"Cinderella" Procedure Allows Women to Function Pain-Free - CA Podiatrist
 
Cosmetic foot surgery such as a trademarked “Cinderella procedure is becoming trendy, according to the Daily Mail. And women who want to totter on the highest possible heels à la Carrie Bradshaw are finding out that if the shoe doesn't fit, they can wear it anyway. At the forefront of the worrying trend is podiatrist Dr. Ali Sadrieh, founder of the Beverly Hills Aesthetic Foot Surgery center in Studio City, CA, where women not only can get their toes trimmed or lengthened but also can have problems like bunions and corns fixed.

“It’s unrealistic to tell women not to wear high heels,” Sadrieh told The Wall Street Journal. “I came up with procedures that allow women to function, pain-free, in the real world.” The so-called Cinderella procedure is a preventive bunion correction that results in a narrower foot. The “Perfect 10!”, a toe-shortening operation, is meant to make the toes twinkle in sandals.

CO Podiatrist Donates Time, Expertise in Laos
 
Imagine spending four months as an unpaid medical practitioner and teacher in a third-world country, and then imagine doing it with eight children, ages 1 to 15. Dr. Arlin Peterson, a Pueblo podiatrist, returned last month from such a trip. He and wife Nancy and their large brood sold their home and left for Vientiane, Laos, in July.

Peterson was living in San Diego, where a sizable community of Vietnam War-era refugees had resettled. He learned their language through teaching them his, and fell in love with their simple, authentic approach to life, he said. “It’s such a gift, working with people who are so happy to have you, and so grateful for what very little they have.”

Providers Not Required to Keep EHR Audit Systems Turned On
 
If healthcare providers are using their electronic health records to falsify medical billing or cover their tracks after mistakes, there's an easy way for investigators to find out: Check the audit trail. Unfortunately, federal rules don't require healthcare providers to keep their automated audit systems turned on. A study out this week from HHS' watchdog office finds that many healthcare providers can simply disable their logs or alter them after the fact—and experts say the problem may be far worse than what the study found.
 
Regulations from HHS' Office of the National Coordinator for Health Information Technology define what the makers of EHR systems must do in order for their customers to qualify for federal subsidies. A certified EHR system must have an audit log function that can be disabled only by authorized users, cannot be altered or deleted, and must be enabled by default. However, the federal rule does not require healthcare providers to keep the audit log turned on at all times—something HHS' inspector general urged the ONC and the CMS to change.

You might try injecting 1 cc Sarapin around the area, either alone or mixed with lidocaine and some steroid. This derivative of the piper plant has been used for decades to treat spinal neuralgias, and many of our colleagues have been using it to treat interdigital neuromas. It is available directly from the manufacturer or from many medical suppliers.

Many specialists have a tough time providing meaningful performance measures that are exclusive to themselves or their specialty or done frequently enough to be of any statistical significance relative to performance and bonus compensation. However, there are areas that you strongly participate in, both directly and indirectly, that can prove your value and...

I would like to hear from some of the 17+% of podiatrists who have decided NOT to ask their congressman to support the SGR bill this week. What part of the bill do you not agree with? Do you feel that we should not be recognized as physicians under the Medicaid guidelines or we should not try to expand podiatric care to diabetics and the poor? Or are you simply apathetic?
 
When I heard that members of Congress have a lower approval rating than cockroaches and hemorrhoids, I wondered who the people were who actually had a positive approval of hemorrhoids? Are you in that category as well?

While Dr. Roth and I usually agree, I am still uncomfortable with testifying against a fellow DPM. I think we should all be. The tort/peer review legal systems are the areas where DPMs seem to have true "parity" with medical doctors. But those are the only ones. This means that the playing field is not level. I agree that we have to be responsible and also be held so for our treatment choices and actions. But whenever we are called to action against a fellow DPM, we should tread carefully, with the full understanding that we should avoid over-reacting.
 
In the case Dr. Roth is likely speaking about, there was a really excessive verdict against the DPM, even if you accept that he made a surgical error. The particular doctor who was the plaintiff's expert is known to frequently testify against DPMs, so this was not his first rodeo. I'm reminded of the history of the French Revolution, which clearly demonstrated that justice is more important for the tribunal than it is for their particular defendant at any one time. Their places can be easily "reversed" as a function of time. History can teach us a lot.

Good news for podiatrists - The U.S. House of Representatives passed a bi-partisan budget agreement that will postpone a 23.7% payment cut to physicians that was scheduled to take effect on January 1st, 2014. Okay, the party is at my house. All are invited! Is everyone as excited as I am? No cut, temporary fix, life is good. But seriously, 2014 is going to be THE year! 
 
Can we really continue to be such fools? Are we so helpless as a profession with such little confidence that this is good news. The answer is YES! Did anyone notice the 2014 fee schedules that show what cuts we will get by code. Guerilla Podiatry has reported that they have crunched the numbers and the average cut for us is about 5.6%. That's good news? We only will get a small cut, right?
 
This is a sad day in our profession. Do you think that podiatrists may have to close their doors at some point? Will podiatrists retire early? I'm hoping you will share your ideas on how to revive a profession that has been sucked into the abyss!

My advice: Hire the best story-teller or DPM expert before the prosecution does. The odds are that they have already done this. If you cannot, then you might be best advised by your experienced medical billing and coding attorney to consider plea bargaining. Find out what percentage of cases that go to trial win. Just ask yourself, "am I feeling lucky?". 
 
Meanwhile, pay for an audit and face the facts, even if it is not a legal case and it becomes a factual dispute. I would stick to the legal case, as opposed to fighting factual disputes.
