AT THE COLLEGES

Sole Supports Donates to Yucatan Crippled Children’s Project at Barry

Barry University’s School of Podiatric Medicine received a $12,000 donation from Dr. Ed Glaser of Sole Supports, Inc., (Lyles, TN) for the Yucatan Crippled Children’s Project (YCCP). The contribution subsidizes costs for medical supplies and equipment.

For 15 years, the YCCP has been going on medical mission trips to Merida, Mexico to perform surgery on disadvantaged, crippled children from the Yucatan Peninsula. The project has organized more than 70 trips rendering treatment to more than 7,400 children in need. Dozens of doctors and students alike train and assist in surgeries, logging in more than 23,500 volunteer hours – providing their expertise and time to the mission.

SPY Imager Shows Blood Flow in the Skin: CA Podiatrist 

When a doctor is surgically debriding a wound, it is not always possible to see the extent of the dead tissue needing removal. There are now devices to help doctors see exactly where to go during the surgery by revealing which tissue still has blood flow (live tissue) and which does not (dead tissue).

Rogers, Associate Medical Director of the Amputation Prevention Center at Valley Presbyterian Hospital in Los Angeles and Chair of the American Diabetes Association’s Professional Foot Care Council, “The SPY Imager from Novadeq is brand-new in the wound-healing arena. It is a novel device that uses a kidney-safe dye that can shows you the blood flow in the skin when looked at with a laser camera. Additional devices that can help doctors assess the blood flow to your limbs are PADnet from BioMedix, and SensiLase from Väsamed.”

QUERIES (CLINICAL)

Query: Chronic Diabetic Ulcer

I would appreciate some suggestions as to how to cure this ulcer in this 51 yr white female, 5'2", 170 lbs who presented with a diabetic ulcer she has had for the past 10-12 months. She has been a Type 1 diabetic, with accompanying diabetic neuropathy for the past 10 yrs. Her meds include the following: Novarapid, NPH, Cymbalta, Lyica, Synthroid, conjugated estrogen, Apo-Atorvastatin. 

Chronic Diabetic Ulcer

Previous treatments have included 2 sessions of Flagyl 500 and Clavulin 500 together, each session for a 4-week period. She is using crutches to remain non-weight bearing on that foot. She is presently using Amerigel daily. X-rays are normal. Debridement occurs every 3-4 weeks. There is no sinus tract present and the surrounding skin is normal with granulation tissue present. There has been minimal change since the inception of treatment.

Query: Diabetic Shoe Frequency

Could someone please clarify the Medicare guidelines regarding the frequency of dispensing diabetic shoes and inserts? I'm aware that they are covered for one pair of shoes and 6 inserts per year. However, if they receive shoes and inserts on October 30, 2011, would they be eligible for another pair on January 1, 2012? I'm just a little confused as to whether "per year" is 12 months from previous dispensing or from January 1 - December 31.

Response: This clarification comes directly from the article attached to the Therapeutic Shoes for Persons with Diabetes LCD:

"For patients meeting the coverage criteria, coverage is limited to one of the following within one calendar year (January – December):

- One pair of custom molded shoes (A5501) (which includes inserts provided with these shoes) and 2 additional pairs of inserts (A5512 or A5513); or

- One pair of depth shoes (A5500) and 3 pairs of inserts (A5512 or A5513) (not including the non-customized removable inserts provided with such shoes)."

So, yes, given your scenario and based on the calendar year definition, a patient could receive one pair of shoes and the three pair of insoles on October 31st of this year and be eligible for another pair starting January 1st of next year.

RE: Allscripts Vendors are Not Created Equal

I also have been misled by E-transmedia. We were promised a podiatry version of Allscripts Myway which they did not deliver. E-transmedia is not capable of providing the podiatry content as was promised. Instead, they are attempting to modify the medical program in lieu of providing the real podiatry product.

Microwize is a computer company located in Paramus, NJ which created and sold the podiatry content to Allscripts Myway.  However, E-transmedia is unwilling to pay for the podiatry license from Allscripts to permit them to download the data for the podiatry program. I suggest that anyone who would like to use Allscripts Myway go directly to Microwize and to avoid E-transmedia. It has been a frustrating experience that is still unresolved in our practice.

Dr. Ribotsky's spine surgeon friend has figured out how to get tipped, just like when we leave extra for the waiter for great service at dinner. What's the expected gratuity if you need an operation performed correctly? An extra $1,000? He's not going to win a humanitarian award, but this is not unheard of. In the old Soviet world of Eastern Bloc countries, doctors were paid so poorly under the socialized medical system that it was customary for patients or their families to offer gifts or money in order to receive their care.

Perhaps this is where our own medical care system will wind up eventually. You'll have your Medicare card and then you'll need to slip the guy at the blood lab $20 for your blood test, or the fellow at the MRI scanner $100 to get you in "this month." Just like slipping the maitre d' a few bucks to get your table faster. Sarcasm aside, we should probably be better than this.

I agree with Dr. Rosenblatt. Practice management meetings offer many tips on starting and running a practice. One of the greatest tips I ever received was from Bob Levoy many years ago when he asked the audience to stop practicing on Saturdays. Then I eliminated Fridays. Next will be Thursdays.
