PODIATRISTS IN THE NEWS
CA Podiatrist Warns Against Walking Barefoot at  Home During Pandemic

Working at home has led many people in America to go barefoot, in socks or slippers. In New York City the trend can be very large, as in Manhattan only 28% of workers have returned to their offices. And although it seems like a very comfortable option, doctors are finding a problem related to this: injured and sick feet. If you are on your feet all day while working from home, consider wearing a shoe that is not only supportive but also shock absorbing. Ami Sheth, DPM, a representative of the American Podiatric Medical Association, suggested in an interview with Health that people try to wear athletic shoes at home.

Dr. Ami Sheth

Dr. Sheth recommends keeping track of when your activity peaks during the day and making sure you are wearing shoes during that time. At the end of the workday, if you’re not active, it’s okay to remove your shoes. But especially during the workday, Dr. Sheth says that wearing “anything is better than nothing.” Dr. Sheth also recommends that people who are vulnerable to foot problems, such as those with diabetes or poor blood flow, also be very vigilant about their foot care.

PRACTICE MANAGEMENT TIP OF THE DAY
7 Ways to Prevent Ransomware Attacks at Your Practice

The COVID-19 pandemic has led to a sea change in the realm of remote employment, but that has opened the door to hackers attempting to take advantage of the situation. One of these bad actors’ chief tools is ransomware. These kind of attacks have been on the rise over the past few years, and in the last year alone have impacted 34 percent of healthcare organizations.

5. Perform security awareness training.

With every employee at risk of receiving one of these attacks, they must all be provided with cybersecurity awareness training consistently reinforced with timely threat information. Responsible behavior can prevent ransomware attacks from being successful.

QUERIES (NON-CLINICAL)
Query: Call Center For Practices

Does anyone know of a reliable company offering a call center for practices?

Hal Ornstein, DPM, Howell, NJ

RESPONSES/ COMMENTS (CLINICAL)
From: Doug Richie, DPM

It should be recognized that the current preferred treatment of the post-TMA patient using therapeutic shoes with toe fillers is simply inadequate. Consider a systematic review which showed that one-third of patients undergoing a TMA will require a more proximal amputation of their affected lower extremity within 5 years. (Thorud, Jakob C.; Jupiter, Daniel C.; Lorenzana, Jonathan; Nguyen, Tea Tu; Shibuya, Naohiro. Reoperation and reamputation after transmetatarsal amputation: A systematic review. The Journal of Foot & Ankle Surgery;2016, 55: 1007–1012).

There are several reasons for the high ulceration rate in patients who have undergone a TMA procedure. Studies have documented high plantar pressures in the residual foot compared to the contralateral foot in TMA patients. This may be related to substantial gait disturbances which have been observed in...

RESPONSES/COMMENTS (OBITUARIES) - PART 2A

RE: The Passing of Keith Kashuk, DPM

What a gentleman.

Clare Starrett, DPM, Anna Maria, FL

Just a few words on the untimely passing of my friend and colleague, Keith Kashuk. This is a man that lived life with a zest for all his endeavors. It goes without further stressing that he was the consummate doctor, both towards his patients and his commitment to education.

We traveled together to Mexico as part of a team to perform surgery on the underserved pediatric population. He lectured around the world and here in the United States sharing his knowledge, expertise, and experiences to his fellow physicians. Some may know his passion was boating and fishing, which he attacked with the same vigor, intensity, and love as he felt toward family and friends. To have him as a part of our lives made us better people and taught us the important values of life.

I will miss Keith on so many levels but a part of his spirit will always be engrained in my soul. May his family find comfort in these difficult times ahead.

RESPONSES/COMMENTS (OBITUARIES) - PART 2B

RE: The Passing of Keith Kashuk, DPM

It was with a great deal of sadness that I learned from my co-resident in training, Dr. Ira Baum, that Keith Kashuk had passed away. He had become a true friend and mentor over the many years I came to know him. I remember first meeting him in a residency interview in south Florida and thinking, "No way did I do well in that interview!". He then invited me to watch a case of his in the hospital the following morning and I remember scrambling to change my plane ticket to be there. As I observed the surgery, and not thinking that I had the program, I thought that if I could end up developing half as good "hands" in surgery as I was witnessing, I would be a pretty damn good surgeon!  I ended up getting his program, and at the time, it was unusual for graduates of the California College to get residencies east of the Mississippi, if you got a residency at all!

A notoriously strict but fair director, he held high expectations for his residents and all of us would agree we are better physicians as a result. We became lifelong friends, and as he became involved with the burgeoning new podiatric medical school at Barry, his enthusiasm for the program was infectious. He knew well how my practice was becoming more and more successful, and when the opportunity to become Dean presented itself, he suggested I revisit the decision, as he knew it would not be as financially rewarding. But when I made the decision, he was the first to congratulate me! We became even closer during his tenure on the Barry Board of Trustees, where he was always an advocate for the School. The rest is history. It is an understatement to say he will be sorely missed. He was a true giant among us, and to have tread this earth as his friend and colleague was a distinct honor. Fair sailing my friend.

RESPONSES/COMMENTS (OBITUARIES) - PART 2C

RE: The Passing of Keith Kashuk, DPM

It's very sad to hear of the passing of Keith Kashuk. He always had a vision to advance the science of surgery. He will be missed.

Gene Spector, DPM, MS

I had the privilege of being in the second residency class that Keith directed in Miami. It would be difficult to explain to anyone who has graduated in the last 20+ years what podiatric surgery looked like back then. The term "Wild West" comes to mind. If you were doing Swanson implants and base wedge osteotomies using wire loop fixation, you were at the cutting edge. And then there was Dr. Keith Kashuk.

I was fortunate to be able to assist him in surgery and learn what foot surgery was supposed to look like. His technical skills in the OR were way ahead of the crowd, and his enthusiasm for teaching was obvious to every student who ever had the honor of working alongside him. The term "pioneer" is sometimes used loosely in our profession...but not this time. Rest in peace, Keith.

RESPONSES/COMMENTS 9OBITUARIES) - PART 2D

I had the incredible good luck to have been one of Dr. Kashuk's residents. His guidance and training set me on the course of my career. Keith was always a gentleman and not only taught us how to do surgery, but also how to be a surgeon. He opened doors for training opportunities that had never been opened before and was never afraid to lead the charge to adapt new ideas and procedures, while always remaining friendly and professional. I will miss his kindness, fishing (only on a Bertram!), and enthusiasm for this profession. RIP.  You will be missed.
