CA Podiatrist Recommends 3 Exercises for High Heel Wearers 
 
Wearing heels regularly can affect your leg muscles over time, says podiatrist Dr. Bobby Pourziaee, a Los Angeles-based foot and ankle surgeon and the founder of the Spa on Rodeo. When you’re walking on tiptoes, your calf muscles are shortened—“and they can stay that way if heels are the only shoes you wear,” he explains. But that doesn’t mean you need to toss your peep toes. Try to alternate between heels and supportive flats, says Dr. Pourziaee. “When the foot is in a non-heel shoe, it’s allowed to stretch and recover.”

Another simple way to stem the damage: Add these three easy exercises to your regular workout routine. They're designed to target muscles in the calves and feet and around the ankles that can help protect against pain. 1) Seated calf stretch with resistance band, 2) Standing calf raises, 3) Towel scrunches.

FL Podiatrist Faces Charges of Sexual Assault and Video Voyeurism
 
A Palm Beach County podiatrist is facing charges of sexual assault and video voyeurism. A woman testified in court Thursday that Dr. Scott Strolla fed her drinks during a brunch in downtown West Palm Beach and then took her back to his place. She claims that she was drugged and woke up during the middle of intercourse and found Strolla, 50, taking pictures. Strolla’s brother Cory Strolla is representing him. After court, he said the acts were consensual and claims the alleged victim only called 911 after her boyfriend called to check on her. Bond for Strolla was set at $100,000.

PCPs are Skittish When it Comes to Discussing Medical Errors: Study
 
According to a new survey conducted by researchers at Georgia State University, whereas 77% primary care physicians would inform their patients about a medical error in some capacity, only a minority would provide a full disclosure that included specific details.
 
The Georgia State University researchers asked 300 primary care physicians about how they would disclose a medical error involving two hypothetical cancer diagnoses. Most of the respondent physicians said they would offer limited or no explanation and limited or no apology, which falls short of both national guidelines and patient expectations.

Patient collections are much more than collecting $10 co-pays: those days are long gone. Patients are paying a greater percentage of what you get paid and will be paying an ever higher percentage in the years to come. Smart and consistent patient collections tactics must be a part of every practice's financial strategy. Though the rate of healthcare spending may have slowed, the share absorbed by patients has done the opposite. Here are a few basics for improving your patient collections.

Query: Digital X-Rays and EMR
 
I just upgraded to digital x-rays and had a question for the old pros. The system will store the images on my computer, but there is also an easy way to upload them to my EMR (Practice Fusion) as a static image. Is there any advantage or disadvantage to adding them to the EMR? If stored locally, I can manipulate the images - measure angles, change contrast / sharpness. For the EMR - you can't,  so should I forget about uploading them to the EMR?

Query: 2017 Medicare Digital X-Ray Requirements
 
If we are taking x-rays digitally as of January 1, 2017, no change is required to our current billing for x-rays. If we are NOT taking x-rays digitally as of January 1, 2017, then we need to add a modifier onto the technical component. Is the modifier actually "XX" or is that just used as a placeholder above?

Response: Beginning January 1, 2017, CMS will be reducing by 20% of the Medicare fee schedule allowance the TECHNICAL COMPONENT (TC) allowance for any x-rays billed that still use film. From January 1, 2017 on, doctors/practices taking and billing for x-rays that use film will be required to append an "FX" modifier to the global x-ray code to indicate they are still using film. 
 
Beginning 2018, practices using "computed radiology" (CR) where imaging is obtain via a cassette or imaging plate, the final rule "provides a 7% reduction in Medicare x-ray payments. CMS promises more details prior to the 2018 implementation date. So, to answer your question, if you are using a digital x-ray system in 2017 and NOT obtaining your radiologic image on film, you do not have to do anything. You would bill as you previously have. If you are still using film, you add the "FX" modifier to your global x-ray code. For example, CPT 73630-FX-LT. Medicare will "automatically" reduce by 20% the technical component portion allowance for you.

RE: Understanding the Quality Payment Program (AKA MACRA)

CMS is starting the Quality Payment Program AKA MACRA on Jan 1, 2017. There is much information on this subject. I receive multiple emails daily from many different companies promoting their webinars and so forth to help educate me on the subject. I know that every Medicare provider can choose to either report for 1 year or 90 days (or calendar year quarter), but what I would like to know is what we actually have to report.
 
This program has had multiple changes (including the name) since its inception and now I am totally ignorant of what to do to avoid a penalty in 2019. I assume many members of the podiatric profession are in the same boat. Those who are going to respond to this are probably going to direct me to the APMA website and their MACRA information and webinars, but I would just like a simple answer if there is one.
 
I understood Meaningful Use and have spent tens of thousands and spend thousands annually to attest for Stages l and ll  to avoid getting a Medicare fee reduction. I have also successfully participated in PQRS to not lose any further monies, but this Quality Payment Program is a horse of a different color completely. With that said, if someone has a plain answer to my question, then please post it so I can sleep well knowing that a pending possible 9% fee reduction in my Medicare payables will not occur in the future.

The x-rays that Dr. Cohen provides show interesting results. I was wondering if Dr. Cohen also did a lengthening procedure on the second metatarsal which is longer than the third on the post- op x-ray compared to the pre-operative x-ray. Perhaps showing the entire foot x-ray would be more revealing.

When sending in pre- and post x-rays, it would be nice if the submitted films be the same view. The Katzen Wilson bunionectomy films are hard to evaluate as the post-op foot is in such an oblique position. The metatarsals are all overlapping while the pre-op foot is splayed. It's only fair to evaluate apples to apples.
