FL Govermor Appoints 3 Podiatrists to the Board of Podiatric Medicine
 
Governor Ron DeSantis announced the appointment of Mark Block, DPM, Donald Popper, DPM and Soorena Sadri, DPM to the Board of Podiatric Medicine. Dr. Block is a Past President of the Florida Podiatric Medical Association and is Chair Emeritus of the American Podiatric Medical Association’s Health Policy Committee.

Dr. Popper is a podiatrist at the Veterans Affairs Medical Center in West Palm Beach. Query: Inappropriate Use of a Modifier with Bunionectomies
 
I have recently done bunionectomies on two separate patients insured with Anthem and received denials. One was a combination of an Austin procedure and an Akin procedure. I billed CPT 28299 -RT. On another patient, I did an Austin procedure and I billed CPT 28296 -RT. Both claims were denied for “inappropriate use of modifier.” I have called the customer service twice and even sent a corrected claim and removed the modifier but the claim was still denied. Has something changed with Anthem that I don’t know about?

Response: Modifiers are used to clarify a billing line(s). If you used the RT modifier, the computer could have been searching for some other procedure to separate from the bunionectomy. If none was found, the claim could then get denied. So if you are only billing one side, do not use the RT/LT modifiers.
 
Did you call the customer service line after the corrected claim you submitted was denied? Was it denied for a modifier reason or some other reason? Were there other services performed recently? Were there global issues from another procedure(s)?

RE: The Passing of Keith Kashuk, DPM
 
Today was a very sad day when I learned of the passing of my residency director, Keith Kashuk, DPM. Dr. Kashuk represented the best in cutting edge podiatric foot and ankle surgery for his time. He was a pioneer in moving forward podiatric surgery at a time when our profession was just getting traction in the field of foot and ankle surgery. 
 
Thanks to him and his vision, the Barry University School of Podiatric Medicine became a reality -  a reality that my co-resident Chet Evans, DPM ushered into becoming a premier school that we are all proud of. I’m certain all who knew him extend our deepest condolences to his wife, daughter, and his grandchildren. May his memory always be a blessing to them and to all future podiatrists.

All DPMs, especially those in Florida, should join me in mourning the passing of Dr. Keith Kashuk. From my 30+ years in our profession in South Florida, Keith stood out as the beacon of whom to emulate in our profession.  He was an incredible practitioner and businessman and loved training future doctors.
 
While a student, I had an incredible externship with him, where his professionalism and knowledge combined with the incredible way he communicated with patients. He helped me understand the incredible gift each of us could give as podiatrists.

Use a custom molded orthosis with a forefoot filler in an extra-depth shoe with a rigid rocker sole (or a fiberglass shank) if the sole is not rigid. If the rocker sole is not rigid, it will bend at the level of the amputation, increasing pressure there and increasing the risk of ulceration.

I see almost no value in stuffing a shoe with who knows what materials on the advice of either a general or orthopedic surgeon as I have seen over the past forty years. This almost never helps patients and almost always creates more harm than good. I have seen this overly simplistic attempt, causing everything from mild inflammation affecting the distal stump to frank ulceration.
 
To say nothing of the fact that this does nothing to offer the patient any assistance in any gait abnormalities which the patient may have. This leaves the reasonable practitioner to investigate whether a custom toe filler with or without a carbon plate to stiffen the shoe is best, or does the patient require an AFO with...

I am sorry but Dr. FitzPatrick seems to have missed the point. Leapfrogging to a Congressional representative without first allowing the system to respond will no doubt beg the question: what proof do you have that this is a conspiracy (though I agree it may exist)? I have worked with those who are JDs, both with and without the DPM degree. All ask the same question prior to wanting to take this to the next level. Prove you are right. Let us see the evidence supporting what you say. I have had my fill of going to meetings with DME MAC medical directors or the RAC, and sheepishly have to admit that I will get the proof of my contentions and then only have to respond to their inquires that I can't get them.
 
From my own personal practice perspective, I have had 2 RAC audits in my practice lifetime and passed both without any issues. I know of others who have done the same, and yet I have heard from many others who repeatedly fail. With some, the documentation is perfect; with others not so, and many...
