OH Podiatrist Discusses When to Consider Bunion Surgery 
 
If your bunion is causing pain, your podiatrist may recommend surgery. Georgeanne Botek, DPM, Head of the Section of Podiatry and Medical Director of Cleveland Clinic’s Diabetic Foot Clinic, says it’s time to consider surgery when you’ve felt pain for at least a year. Surgery can be done on mild or severe cases. “When patients come in at an older age, usually it’s because the bunion is causing other problems,” Dr. Botek says. “For these patients, the pain is more constant or is creating problems with the second toe.”

The goal of surgery is to return the big toe to its correct position. A surgeon puts bones, ligaments, tendons, and nerves back into correct order, and removes the bump. There are more than 150 types of bunion surgery, but surgeons typically choose one from about a half-dozen commonly used procedures, Dr. Botek says.

All bunions are permanent unless surgically corrected. But there are some measures you can take to be more comfortable or to slow a bunion’s progression, says podiatrist Dina Stock, DPM, who sees patients at the Solon Family Health Center and Willoughby Hills Family Center.

“For many people, it may simply be a matter of wearing properly fitting shoes,” Dr. Stock says. “Be sure to choose low-heeled, comfortable shoes that provide plenty of space for your toes and the widest part of your foot.”

Twenty-two-year-old Joanna Allan, BSc (Hons) Podiatry MChS (Member of the Society of Chiropodists and Podiatrists) diagnoses and treats conditions in the foot and lower limbs. Joanna said: “Podiatry is a fascinating science which affects the whole body. “Knowing I am helping to improve someone’s quality of life makes my job hugely satisfying. As they say ‘if your feet are sore, you’re all sore’.”

Artist Finds Inspiration from Famous Foot Sign 
 
Strip mall signs. Taco trucks. Small business mascots. Many an artist has documented the visual landscape of Los Angeles’ streets, but few have felt it as strongly as Billy Kheel. For his last show, “Sunset Stitches,” at Trencher, a cafe in Echo Park, the 40-year-old Silver Lake artist crafted painstakingly detailed felt-applique soft sculptures based on local commercial signage -- including the revolving podiatry clinic beacon at Sunset Boulevard and Benton Way known colloquially as “Happy Foot, Sad Foot.”

Kheel, who has an Etsy shop, will be exhibiting at the Renegade Craft Fair in Los Angeles on Saturday and Sunday. Among his other handmade works, which he calls “Vegan Taxidermy,” Kheel will be offering ornament-sized versions of Happy Foot and Sad Foot.

CMS Announces Next Phase in Medicare DMEPOS Competitive Bidding
 
The Centers for Medicare & Medicaid Services (CMS) today announced the bidding timeline for Round 2 Recompete and the national mail-order recompete of the Medicare Durable Medical Equipment, Prosthetics, Orthotics and Supplies (DMEPOS) Competitive Bidding Program, as required by law. CMS also launched a comprehensive bidder education program. This program is designed to ensure that DMEPOS suppliers interested in bidding receive the information and assistance they need to submit complete bids in a timely manner.
 
The DMEPOS Competitive Bidding Program changes the amount Medicare pays for certain DMEPOS while maintaining beneficiary access to items and services and quality of care. The program replaces the outdated, inflated fee-schedule prices Medicare paid for these items with lower, more accurate prices to help Medicare and its beneficiaries save money while ensuring access to quality equipment, supplies, and services. This program also helps limit fraud and abuse in Medicare. CMS is required by section 1847(b)(3) of the Social Security Act to recompete contracts under the DMEPOS Competitive Bidding Program at least once every three years.

QUERIES (DME)
Query: Billing for Custom Orthotics 
 
I've obviously missed something very important here. Since when does MC or DMERC pay for custom orthotics? I know they pay for custom AFOs, but thought they did not pay for foot orthotics.

Response: There has been much confusion recently on the subject of Medicare coverage and custom foot orthotics (CFO). To end the confusion, it's quite simple: In over 99% of cases that a CFO is required, Medicare will not cover them. To repeat: CFOs are usually not covered by Medicare. Hence, in just about every case, one should NOT (and I repeat NOT) ever use the KX modifier when billing for CFOs. If it's that simple, then why the confusion?
 
There is a very ,very narrow margin of cases, where CFOs are covered by Medicare and that is ONLY when:
1) The shoe is an integral part of a brace, that is the shoe is attached to the brace (See Forrest Gump).
2) The orthotic is placed into that shoe which is part of a brace.
3) The supplier who provided the orthotic also provided the brace.
 
In only the above cases, would there be coverage. By using the KX modifier for CFO (L3000-L3020), you are telling  Medicare you met the above 3 stipulations. Since these three almost never happen in a podiatry practice, it would be wise to never think that CFOs are covered by Medicare. To obtain a rejection, use the GY modifier, which will result in a patient responsibility (PR) statement by the DME MAC. For more info on the subject, read your DME MAC LCD on Orthopedic Footwear.

Dr. Kiel may not want to sound snarly or obnoxious, but sorry to say, he sounded both.  I wonder if he is the only podiatrist out there who has never tried to fix something in his office before calling an over-priced repair man who sees dollar signs after the "Dr."
 
As for the chair, I had a similar problem after I had a chair reupholstered (done by a professional) and the wires were “accidentally” removed. The auto return switch uses a series of relays, as does any pre-set” buttons. After trying for a long time to rewire the circuits, I ultimately just disconnected the auto return switch, and connected the up/down, tilt, and back switches directly between the power and the motors.  It all works well and honestly, I don’t really miss it!
