MI Podiatrist Discusses Compresssion Therapy for Venous Diseases
According to Conway McLean, DPM, "Compression is the mainstay of therapy for many vein diseases. A difficult question is what type of compression, with numerous methods available, some “old school” and others hi tech. By far the most commonly prescribed method of treating the condition before there is breakdown of the skin are compression stockings. These thick, heavy stockings that go over the foot, ankle, and lower leg are made of an elasticized material which results in compression of the limb. Unfortunately, if these garments are tight enough to be effective, they are too tight for most seniors to don. If the patient is successful (usually only with assistance), the garment is not comfortable to keep on."

"Powered pneumatic systems (think of giant socks that inflate) are available and some of them are quite effective, but if they work well, they are costly. The most exciting development in the world of venous insufficiency therapies is a new compression device, basically a sleeve for the lower leg. Velcro tabs allow the patient to control the amount of pressure on the leg. They are significantly more comfortable than prescription compression stockings and are more effective at healing the skin and reducing the swelling," says Dr. McLean.

Dr. Peterson also practices at the group's location in Celebration, FL.

APWH Holds Their Highest Rated Annual Virtual Symposium
The Academy of Physicians in Wound Healing (APWH) held their Annual Virtual Symposium this past weekend on the lerEXPO platform. The event was organized by Steven Kravitz, DPM and facilitated by Rich Dubin, CEO of Lower Extremity Review magazine and lerEXPO. This multi-specialty conference was attended by podiatrists, dermatologists, primary care physicians, vascular surgeons, physician extenders (NP, PA), and wound care nurses. The theme of the meeting was not only how to treat wounds, but how to manage their underlying etiologies. “The event was an immense success with an overall approval rating of 4.96 out of 5 stars. Our highest attendee rated event thus far,” said Dubin.

Medicare Advantage Plans Dodged Auditors and Overcharged Taxpayers By Millions
In April 2016, government auditors asked a Blue Cross Medicare Advantage health plan in Minnesota to turn over medical records of patients treated by a podiatry practice whose owner had been indicted for fraud. Medicare had paid the Blue Cross plan more than $20,000 to cover the care of 11 patients seen by Aggeus Healthcare, a chain of podiatry clinics, in 2011.
The Medicare Advantage plan issued by Minnesota Blue Cross won its appeal after it relied on Aggeus Healthcare for diagnoses of vascular disease for 11 of its patients who got podiatry care. Dr. Yev Gray, a Chicago podiatrist who owned the Aggeus chain that operated in more than a dozen states, was indicted on federal fraud charges in Missouri in October 2015. Gray pleaded guilty in May 2017 to charges of conspiracy to defraud the United States and making false statements related to healthcare matters. He was sentenced to 90 months in prison.

3. Telehealth visits will continue to grow as insurers cover more services.
The COVID pandemic led to a dramatic rise in the use of telehealth services, and that trend is expected to continue into 2023. This expected increase is due to 3 main factors:
Patient Acceptance - A survey conducted by Accenture showed that 62% of patients prefer virtual visits.
Advances in Technology - Many EHR systems now integrate with telehealth platforms with user-friendly features for both patient and provider.
Insurance Coverage - According to United Health Care, telehealth options are more economical, saving $130 per urgent care visit. This has led to many HMO and PPO plans giving patients the option of a virtual appointment for an increasing number of visit types.

It is good to see stories like this in the mainstream media and to see U.S. Senators sufficiently irate to call CMS to question the seriousness of these allegations. The $1,000 annual overcharge per patient quoted by the anonymous poster is actually quite higher, with one well over $5,000 per year. Senator Chuck Grassley has called for “aggressive oversight” to recoup the overcharges, further commenting that CMS must aggressively use every tool at its disposal to ensure that it's efficiently identifying Medicare Advantage fraud and working with the Justice Department to prosecute and recover improper payments.
The article further states that CMS has estimated the total overpayments to health plans for the 2011-2013 audits at $650 million, yet how much it will eventually claw back remains unclear. It is unfortunate that...

I rarely operated on both feet together, unless the patient pushed the issue and had help at home. A few years back, one of the malpractice carriers stated that doing bilateral complex cases increased the chances for a problem, and of course, the chances for litigation.
