NY Podiatrist Receives 5th Annual Ronald L. Soave Award
On December 9, 2021 Sandro Frasca, DPM was presented the fifth Ronald L. Soave, DPM Integrity and Excellence in Podiatry Award at the New York State Podiatric Medicine (NYSPMA) Downstate Meeting. The inscription on the award reads as follows: “In recognition for your time and dedication to the podiatric profession and for your outstanding leadership in the NYSPMA.” The award was presented by the Award Founder Corrine E. Renne Landau, DPM and Dr. Soave.

MO Podiatrist Discusses Morton's Neuroma
One of the most common foot conditions, Morton's neuroma, affects about a third of U.S. adults, according to the Cleveland Clinic. It's an inflammation of a bundle of nerves between the toes, most commonly the third and fourth toe, notes Tim Oldani, DPM, podiatrist at Missouri Foot and Ankle in St. Louis.

"With continued inflammation and entrapment, the nerve tissue can become thickened and enlarged, which can lead to a painful, burning sensation with weight-bearing that is typically felt more on the bottom or ball of the foot," he tells LIVESTRONG.com. "There can also be associated numbness or tingling radiating to the toes." Unlike some other foot conditions, Morton's neuroma is not hereditary, nor is it related to a specific injury. It's often the result of wearing narrow or ill-fitting shoes, Dr. Oldani says. "Many times, people with neuroma pain will feel relief and decreased pain simply by removing their shoes," he says.

7 Ways to Prevent Ransomware Attacks at Your Practice
The COVID-19 pandemic has led to a sea change in the realm of remote employment, but that has opened the door to hackers attempting to take advantage of the situation. One of these bad actors’ chief tools is ransomware. These kind of attacks have been on the rise over the past few years, and in the last year alone have impacted 34 percent of healthcare organizations.
3. Expedite and automate patching.
Delivering patches should be a priority when new vulnerabilities are discovered in clinical and business applications, medical devices, and other patient care and delivery systems. When these vulnerabilities are identified, updates should be automated, and patches should be distributed to third-party applications known to be impacted.

A minimally invasive tibial sesamoid planing (partial sesamoidectomy) might be one of my all-time favorite procedures. I use it for a variety of sub 1st met problems in my office. This includes sub 1st met IPKs, even in the presence of mildly plantarflexed 1st rays or laterally deviated tibial sesamoids in the presence of HAV where the patient is not having an HAV procedure, enlarged or bipartite sesamoids, and even non-responsive sesamoid fractures.
A small medial incision with a 67 blade is made directly to the sesamoid, and after separating it from the soft tissue, a rotary burr is used to remove approximately 1/2 of the sesamoid. A Steri-strip (or one stitch) is applied and the foot bandaged. I have the patient remove the bandage in 3 days, apply a simple Band-Aid, and they are in a shoe before the 1st post-op visit in less than a week. The IPK/pain “magically” disappears. This also avoids the possible complications associated with complete sesamoid excision and the post-op recovery period due to the necessary dissection in order to perform the procedure.

We are experiencing the same denials addressed by Name Withheld. We too meet all the CMS criteria only to have appeals rejected with an LPN reviewing and denying them for lacking information clearly provided but obviously unread. I believe that Name Withheld is correct in that his/her next appeal should be congressional, and this is where the political arm of the APMA should be more strident and vigorous in their demands.
The individuals who list DPM + JD degrees have a unique combination that should give them insight into how podiatrists might pursue punitive damages for unreasonable demands and “harassment.” Oh yes, you have an ABN. I wonder how the patient will react to being billed 2 years after delivery of DME. And will it help your social media reputation?

Kudos to Dr. Simmons for bringing up an issue that has long affected and annoyed all of us. When we write a prescription for a topical antifungal, we never know if the patient's insurance company will either allow it, reject it, or hit the patient with a "million dollar" co-payment for it. The latter will generally result in either the patient and/or the pharmacy chasing after us to try to find an alternative antifungal that is covered with a reasonable co-payment.
Since many topical antifungals are OTC products and many of our podiatry suppliers are happy to stock our shelves with them, dispensing these products is a good option. If the patient must get the medication via his or her pharmacy, one method that has worked well with us is to write for a specific cream and give the pharmacist permission to substitute an alternative topical antifungal that is covered by the patient's insurance company. This avoids calls back and forth from the pharmacy while the pharmacy staff searches for the product that will be covered.

Podiatrists in the great state of Texas can administer vaccines. I do, but I don't advertise that I do, and after I get through my first batch of 100 I'm done. It's really quite a hassle to get set up, get the required calibrated double thermometers that are hooked up to the cloud. For every freezer and or refrigerator, you need two thermometers. If you have the personnel and you can do a couple of hundred in a week, it may be worth it. You are supposed to get $40 to give the vaccine. You are to bill the patients’ insurance first and hope to get paid, and then the Government if you don't, and for the uninsured patients. Then you will need someone to keep track of that.
The vaccines only last so long refrigerated, and you need special freezers with Moderna and Pfizer (which you have to mix) to keep them longer. Once the vial is punctured, you have 6 hours to use it, and then if you don't, you have to report the waste in 24 hours. Once you give the vaccination, you have to enter the data within 24 hours.
