IN Podiatrist is Chairperson for Ballet Fundraising Event
 
Indiana Ballet Conservatory (IBC) has announced that it will present a sizable check on behalf of Pointe to the Cure to Team JOEY, a Heroes Foundation Program, at its matinee Nutcracker performance on Sunday, December 15th, at the Indianapolis Museum of Art’s Tobias Theater. Pointe to the Cure is an IBC initiative led by Dr. Adam Perler, a member of IBC’s board of directors, to raise awareness and funds for pediatric cancer research.

IBC’s mission includes connecting with the community through artistic outreach to serve others. Dr. Adam Perler, the chairperson for this year’s event noted that, “Pointe to the Cure gave us a tremendous conduit to teach our children to pay it forward by dedicating their talents and passion for dance to other children impacted by cancer. I think it is fair to say that no one left that night unaffected.We are so happy to present the check during this season of giving.”

FL Podiatrist Volunteers as Medical Director of 3-Day Cancer Event
 
Dr. David Allen and Pat Straub, office manager for Dr. Allan Rothschild, recently participated as medical crew at the Tampa Bay Komen for the Cure Breast Cancer 3-Day, 60-mile event. This is the 8th year they have worked together with Allen serving as podiatrist and medical director, and Straub serving as medical crew captain.

This is final year for the event in the Tampa Bay Area. "It's the Olympics of blister care, and all for a cause that affects so many families. We are really going to miss the event in our area" says Allen.

OUTSIDE INTERESTS - PART 1
MI Podiatrist Named Boxing Commissioner Emeritus 
 
Dr. Stuart Kirschenbaum has been called a lot of colorful names during his long and passionate association with boxing in Michigan: agitator, troublemaker, rabble-rouser. I’d prefer to describe Doc as a boxing activist or guru, who for many years sought change in efforts to protect a fighter’s well-being and clean up the Sweet Science.

Kirschenbaum, a leading podiatrist in Detroit and former confidante of Joe Louis and his family in the Motor City, was state boxing commissioner in 1981-92. The 68-year-old was a former amateur light-heavyweight champ in New York City. Recently, Kirschenbaum, a close friend of the late Emanuel Steward, laced up the gloves at the request of the Governor’s office in Lansing. In June, he was appointed “boxing commissioner emeritus and adviser to the office of the governor on boxing issues in Michigan.”

OUTSIDE INTERESTS
Distance is No Deterrent for MI Podiatrist Spartan Fan 
 
There are roughly 1,800 miles between Genesee County and Pasadena, CA. It's three time zones away, about a four and a half hour flight. None of that's stopping local Michigan State University Fans who are hell bent on seeing the Spartans in person on Jan. 1 take on Stanford in the Rose Bowl. Dr. Steve Neirink was a freshman at MSU during the '88 game. Now a podiatrist in Burton, he said he's not planning to miss the opportunity to see the game in person. "When you wait a quarter of a century, you sort of have to jump on it," he said.

Neirink remembers watching the Spartans play in their last Rose Bowl visit. He was home on break watching the game with a couple of friends who also went to MSU. The guys are still friends, and a few are part of the group joining Neirink in Pasadena in January. They're working out travel arrangements and plane tickets right now. "Come heck or high water, we're gonna get out there," he said.

ON THE INTERNATIONAL LECTURE CIRCIT
NH Podiatrist Hold Biomechanics Workshop in Taiwan
 
Howard Dananberg, DPM recently presented a two-day seminar on sagittal plane biomechanics and lower extremity manipulation in Taipei, Taiwan to an audience of orthopedists, physiatrists, physiotherapists, chiropractors, osteopaths, and doctors of traditional Chinese medicine.  Formal lectures and workshops were provided.

Prior to the meeting, Dananberg traveled south to Taichung to visit Dr. Tsai Yung Yuh and his rehabilitation clinic at the Taichung Medical Center. Many of the principles presented were new to Taiwan, and discussions were held to have additional meetings in the future to continue to teach the concepts of podiatric biomechanics. The meeting was organized by Ig Loh, part of the worldwide Vasyli network.

MEDICARE NEWS
Lawmakers Express Hopes for Permanent Medicare 'Doc Fix' 
 
Committee leaders in both the House and Senate expressed optimism Thursday that Congress could soon reform Medicare's flawed physician payment system. The chairmen of the Senate Finance and House Ways and Means committees urged quick progress on bills to repeal the sustainable growth rate (SGR) formula, a goal that has eluded Congress for more than a decade. "Enough is enough. After a decade of Band-Aid solutions, it is time for this committee to act," said Finance Committee Chairman Max Baucus (D-MT). 
 
Both panels were marking up bills Thursday that would repeal the SGR and replace it with a system moving Medicare toward value-based payments. While some lawmakers expected to approve a permanent "doc fix" by Dec. 31, that agenda was overtaken by the government shutdown and several other must-pass bills. Now, a three-month "doc fix" could be passed alongside the budget agreement announced this week by Sen. Patty Murray (D-WA) and Rep. Paul Ryan (R-WI), setting the stage for SGR repeal next year.

Query: ICD-10 Question
 
The example given in a previous post thread outlining how to determine an ankle sprain ICD-10 diagnosis code (S93.491A) was easy to follow. That said, is one ICD-10 code specific enough to get paid, or do you need the external causes and place of occurrence codes, too? How does one know if one needs one or more ICD-10 codes in order to be reimbursed?

Response: I am a member of the APMA Coding Committee, which is committed to preparing members for implementation of ICD-10. This is a question we specifically submitted to CMS. It is my understanding that CMS stated that Medicare will NOT require providers to submit additional coding to reflect external causes of injury under ICD-10. This can change with another payer. 
 
Presumably, most any Workers Comp payer will want such information, as that is a fundamental element of what they are tracking. Check with the policies of the payers you are billing.

Put her in a CAM boot until the 3rd met fracture heals, and try to talk her out of any further surgery. If you feel the need to try to fix it surgically, a 1st MPJ fusion with 2nd MPJ fusion (with graft) is an idea, but you (and her) have to understand that her foot will never be normal. You are only trying to lessen her pain. I assume the bunion was the issue in the first place and a Lapidus procedure likely needed to be done. 
 
If her only issue is the floating toe, find a shoe with a bigger toebox after the 3rd met fracture is healed (Nike Free makes shoes with a soft toe box).

The hallux doesn't appear to have drifted far enough laterally to grossly interfere with the 2nd toe. If this is clinically true, and no pain exists about the 1st MTP, I advise leaving the 1st ray alone. The last thing you need is to consider additional surgery on a different ray that is asymptomatic. 
 
On the other hand, if the 1st MTP is symptomatic, a distal 1st metatarsal osteotomy w/ PASA correction, if needed, and lateral release of the fibular sesamoid and...

Forget about DVT prophylaxis for the moment. Smoking affects bone healing, and you can end up with poor healing and bad results. She must stop smoking one month before going to the OR; otherwise don't do it. With reimbursements so low and malpractice premiums going up, what's your rush? Protect your patient and yourself. Doctor do no harm.

I'd be reluctant to suggest autism as a diagnosis to the parents of this toe walker. If a thorough physical examination reveals no abnormalities (neurologically sound, no evidence of musculoskeletal problems, etc.), it might be simply a behavioral problem that the child outgrows. I was a toe walker until about age 11, and I tended to keep my mouth shut because to speak out of turn might result in a whack to the head. I've encountered patients like this, and have recommended stretches and shoe modifications. I followed them closely. Eventually, they "outgrew" the behavior.

This proves that podiatrists never learn to protect each other. They sell themselves to the legal profession and injure themselves. I have been out of podiatry for 30 years, and things have never changed. 30 years ago, it was Jim Lawton who was the lawyers' prostitute and lost cases that were tried. He made money as an expert against podiatrists, even though he had limited experience in surgical techniques and earned his living as an expert witness. I defended a case against me, pro se, and won. He is now working with a large practice in Illinois and still testifying. When will this profession grow up?

Kudos to the editorial staff of JAPMA and to APMA for selecting  leading experts in the world to produce this landmark issue of the journal (J Am Podiatr Med Assoc. 2013 Nov-Dec;103(6)) which is devoted to the role of the podiatric physician in the prevention of falls in the elderly. The peer-reviewed articles describe interventions which are evidence-based and tested by rigorous scientific methodology. 
 
Nowhere in this entire publication is there any evidence that an AFO device can prevent falls and no preventive programs centered around so-called "balance bracing" are mentioned by the credible authors of the articles. While the significant role of the podiatric physician in falls prevention has been elegantly demonstrated in this recent issue of JAPMA, we must be wary of gimmicks which have no valid role in this important public health issue.
