Non-Resolving Edema Could Indicate a DVT: NY Podiatrist
 
"Foot swelling that persists despite efforts to reduce or resolve it should be taken seriously and evaluated by a medical specialist," says Nelya Lobkova, DPM, a New York City-based podiatrist at Step Up Footcare. Specifically, if you find that one foot starts to swell suddenly, seek immediate care as this could be a sign of a serious issue called deep vein thrombosis (DVT), Dr. Lobkova says.

DVT happens when blood clots form in the leg veins, preventing the return of blood to the heart, according to Harvard Health Publishing. If these blood clots dislodge and travel to the lungs, they can cause a serious, potentially life-threatening pulmonary embolism. Other warning signs of a blood clot to watch out for include breathlessness and/or pain with breathing.

The Academy of Continuing Podiatric Medical Education (ACPME) held a two-day virtual Seminar this weekend. The seminar which drew a record attendance of over 400 podiatrists was organized by ACPME's scientific director Robert Marcus, DPM and Director of Continuing Medical Education David George, DPM.

2 Women Arrested for Healthcare Fraud Kickback Schemes Involving NY Podiatrists
 
Two New York women were arrested today on criminal charges related to their alleged participation in schemes to pay illegal kickbacks and bribes to Medicare beneficiaries and Medicaid recipients for medically unnecessary prescriptions filled by various pharmacies in New York that resulted in more than $10.5 million in total losses to Medicare and Medicaid. According to court documents and proceedings, Hua Huang, 47, of Fresh Meadows, and Huiling Wu, 40, of Brooklyn, were charged separately. 
 
Huang was an employee of NY Elm Pharmacy Inc. located in Flushing. She allegedly referred an individual to a podiatrist who signed prescriptions – including for durable medical equipment and Diclofenac Epolamine – that NY Elm and other entities with common ownership then billed to Medicare and Medicaid. Wu was an owner and employee of 888 Pharmacy Inc. (888 Pharmacy), located in Brooklyn. She allegedly referred an individual to specific podiatrists who signed prescriptions that 888 Pharmacy then billed to Medicare and Medicaid.

Cost-Effectiveness of Melanoma In Situ Resection and Repair by Dermatology vs Non-Dermatology Specialties
 
When excising a melanoma-in-situ (MIS), other specialties often use operating rooms and general anesthesia, whereas a dermatologist typically performs the procedure in an outpatient setting.
 
In this single-institution study, the average total cost for MIS treated with wide local excision by a dermatologist was $1,089, compared with $2,325 with Mohs micrographic surgery. The cost of excising MIS when performed by non-dermatologists was $5,172.
 
MIS treatment by a dermatologist is more cost-effective than a similar treatment performed by a non-dermatologist.

How would you code/bill the following situation: A new patient with diabetic neuropathy presents to the office. He states that he feels like he’s walking on a lump in the ball of his foot with mild pain. Examination reveals a small entrance wound on the plantar aspect of the 2nd MTPJ with no signs of infection. Debridement is performed without anesthesia and a portion of a toothpick is retrieved (without making an incision).
