Dr. Brenner shares that this is so dangerous because it will block your pain receptors, and you can damage your feet!

On November 30, 2021 the Michael Ann & Saquon Barkley Foundation came to New York Foot and Ankle, the offices of Laurence D. Landau, DPM and Corrine E. Renne Landau, DPM to provide a Thanksgiving Turkey Giveaway complete with 500 turkeys and pantry boxes.

NJ Podiatrist Re-Appointed to CPME Continuing Education Committee
 
David H. George, DPM has been appointed by the Council on Podiatric Medical Education (CPME) to the Continuing Education Committee [CEC] for a three-year term 12/3/21 to 12/3/24. This committee is responsible for reviewing applications for approval of providers of continuing education, progress reports, petitions for continued approval, and other requested documentation submitted by providers in accordance with CPME publications 720 and 730 and transmitting recommendations for provider approval for continuing education in podiatric medicine for final action by the Council.

HEALTHCARE LEGISLATION
Legislation Stopping Medicare Payment Cuts Clears Congress
 
In a 59 to 35 vote on Thursday, the Senate passed legislation that will prevent statutory Medicare payment cuts from taking effect next year. The bill cleared the House earlier this week. With approval from Congress, President Biden can now sign the bill into law.
 
The law will extend a moratorium on the 2 percent Medicare sequestration until April 2022, then reduce the amount taken from payments to 1 percent for another three months. The law also prevents the statutory Pay-As-You-Go cuts to Medicare, which total 4 percent of Medicare payments, and some of the rate cuts finalized in the Medicare Physician Fee Schedule final rule from November.

One of these bad actors’ chief tools is ransomware. These kind of attacks have been on the rise over the past few years, and in the last year alone have impacted 34 percent of healthcare organizations.
 
2. Be vigilant with a backup strategy and execution.
 
When ransomware attacks impact patient care or practice operations, a backup strategy enables rapid, effective recovery as well as additional protection. Choose and implement a system which delivers continuity. A durable copy of data must always be available.

QUERIES
Query: Medicare Part “D” Approved Antifungal Creams
 
I am curious what others are prescribing as topical antifungal creams that are covered by Medicare Part “D” or supplemental plans. Personally, I got so tired of filling out medical necessity forms that were ultimately denied anyway that I am currently prescribing ketoconazole, ciclopirox, olamine, or nystatin.
 
The challenge that I have is these are rarely listed as medications of choice by the clinical laboratories, yet those are either OTC (e.g., terbinafine, tolnaftate, etc.) or routinely denied when prescribed.

Regardless of whether it is bipartite or fractured, besides palliative treatment with off-weight-bearing devices, the only other choice is take it out. It’s a simple surgery with little to no post-op problems.

Over the years, I have treated many, (mostly tibial) sesamoid fractures. I believe this is because the fibular sesamoid can move laterally in the first interspace and avoid overloading. I feel the important thing is to take bilateral views, since there are humans with unilateral bipartite sesamoids. I always recommend to anyone to get an axial sesamoid view of both feet.
 
Depending on the age of the patient and level of activity, if the pain persists after off-loading and/or injection, then excision of the sesamoid may be needed. My preferred method of off-loading is with an accommodating custom orthotic with a silicone pad under.

During the last year, as workgroup coordinator of the APMA DME Workgroup, I have tirelessly spoken to many suppliers and the APMA's news blasts about RAC denials. We have been asking practices who have had RAC denials which contain change in condition/diagnosis to contact APMA or their professional organizations. So far, those responses and examples have almost led to nothing.
 
In a collaborative effort, I have reached out to AOPA which represents orthotic practitioners and manufacturers and PFA which represents pedorthists; all have also had the same negligible response from their constituents. The same is true for orthopedic foot and ankle specialists, hand specialists, and occupational therapists. I also have voiced these concerns at Jurisdictional Council meetings. The bottom line is all of these organizations have members with the same issues with Same or Similar. But none can go to meetings with the RAC directors empty-handed. The first thing asked is to show us examples; without them, we have no argument.
 
If you have examples which you feel qualify as exceptions to the same or similar rule because of a change in diagnosis or condition, by all means these should be forwarded to Scott Haag at APMA Health Policy and Practice Department, this regardless of your APMA membership status. This will allow the APMA staff and me to review these to be sure they meet the exception criteria. For more information on how to submit your documentation in a HIPAA compliant fashion, I suggest you call APMA and ask for Scott Haag or someone in the Health Policy and Practice.

This is me working a COVID-19 vaccine clinic for 5-12 year olds in MA three weeks ago. I have a second one coming up this Monday. We vaccinated over 200 kids during the four hour session. Our MA Board of Registration wrote a regulation confirming that podiatrists are qualified to administer these.
