MGAS Presents Heroes Walking Among Us Award
Chester W. "Chet" Galaska has been living with diabetes since 1981, and he is helping others with it to live healthier lives. In recognition of this work, he was recognized by the Massachusetts Foot and Ankle Society (MFAS) with the annual Heroes Walking Among Us Award. This award was presented by Walter Wolf, DPM, President-Elect of MFAS, at the society's annual meeting.

Unions Go to Court to Stop VA Changes in ‘Official Time’ Policy
The Veterans Administration's (VA) announcement that it would eliminate paid union-related official time for medical professionals, as of Nov. 15, has drawn a lawsuit from unions representing the agency’s employees. The American Federation of Government Employees along with the National Federation of Federal Employees and the National Association of Government Employees filed their lawsuit the day before implementation was planned.
The move came after the VA said it would be “repudiating” certain provisions of master collective bargaining agreements it had previously accepted with the following unions: American Federation of Government Employees, National Federation of Federal Employees, National Association of Government Employees and National Nurses United. According to the agency, the repudiation will apply to all of the VA’s nearly 104,000 Title 38 employees “eliminating all forms of taxpayer-funded union work” for many professionals including podiatrists.

Strategies for Better Billing and Collections - Part 3
Here are some resolutions that can improve your billing and collections in the new year.
3. Resolve to stay on top of claims.
This is one of the most important things you can do to improve your bottom line, says Tammie Olson manager of coding and compliance at Management Resource Group, a firm offering financial management and support services for the healthcare community. File claims on time, daily if possible, and have a routine system for checking open claims. Be sure to file amended claims when necessary and never miss a resubmission deadline.

| QUERIES (NON-CLINICAL) | |
Query: Podiatric Cancer Registry
Are all DPMs in a risk group? Almost every week, we learn of a fellow DPM who has passed on due to some type of cancer. I wonder if there is something we are all exposed to that might be increasing the risk of specific cancers among DPMs. Many have been reported with lymphoma and pancreatic CA which makes me wonder.
My request is that one of our researchers, schools, or the APMA consider developing a registry so that real data can be generated. When I started podiatry school in the '80s, there was a thought that testicular cancer incidence was higher among DPMs and the thought at the time was that the use and grinding of Rhoadur (that we all used in orthotics) was responsible, causing this material to be removed from the market.

From: Tip Sullivan, DPM
I have always used a 50/50 mixture of local anesthetic and Renographin under "C" arm guidance. Use a small needle and inject dorsally until the joint is full. Any leakage from the medial/lateral/plantar aspect is evidence of a capsular tear. Make sure you visualize it in multiple planes.

From: S. Jeffrey Siegel, DPM
If it’s a true plantar plate tear, the combination of a positive modified Lachman test in combination with a thin slice (2mm) sagittal plane MRI will confirm the diagnosis. A 3T MRI is the optional magnet size for best resolution. I also recommend that you have the tech call you when the patient arrives so you can confirm the exact location of the 2nd MTPJ pathology.
Regarding arthrograms, you should use 1/2cc lidocaine mixed with 1/2cc hypaque in a 23g needle or whatever radiology has in stock. Use an ultrasound or fluoroscopy to ensure that you’re in the joint and to observe extravasation which, depending on the severity of the tear, would confirm the diagnosis.
S. Jeffrey Siegel, DPM, Bensalem, PA

I am so sorry to learn of the passing of past NYSPMA president and NYCPM Board member Hank Merletti, who besides being extremely active, was one of the really super nice guys in podiatry! My condolences to his enormous family, including his eight kids, TWO of which, my Podiakids Ted and Michael, are podiatrists and former students of mine.

RE: Changes in Sports Medicine Licensure
From: Linda McSmith
As part of the healthcare and podiatric community, I want to share pertinent information regarding recent changes that could affect your practice.
The “Sports Medicine Licensure Clarity Act of 2018”, established by H.R. 302, provides clarity for sports medicine physicians, athletic trainers, and other healthcare professionals. This Bill extends the medical professional liability insurance coverage of a state-licensed healthcare professional to a secondary state when the professional provides covered medical services to an athlete, athletic team, or team staff member pursuant to a written agreement. Prior to providing such services, the professional must disclose to their medical professional liability insurer the nature and extent of the services.
This Bill requires the professional to be licensed in their primary practice state but allows the professional’s license to be deemed valid in a secondary state when traveling with sports teams. Licensure requirements of the secondary state must be substantially similar to the licensure requirements of the primary state. We recommend being aware of the scope of practice in the state that the treatment occurs. This law is specific in that it only applies to professionals who are traveling with an athlete or team and rendering sports medicine. Be sure to contact your medical professional liability insurance company before engaging in sports medicine care.

| RESPONSES/COMMENTS (NEWS STORIES) | |
From: Bryan C. Markinson, DPM
Dr. Levy's history in our profession speaks for itself. He has been a staunch advocate for an unrestricted DPM license for many years. However, in this forum several times I have asked what has become of graduates from his DPM/DO program in Florida that was supposed to "solve" this problem. There has never been a response either from Dr. Levy or any of the graduates that I can recall.
I had opined at the time it was started that it would be unlikely for any graduate of that program, with full access to any medical or surgical (MD/DO) residency they could get into, to choose podiatry as their specialty, directly opposite of his intention to enhance the podiatrist. So I ask again, what has become of those graduates? Do they practice as podiatrists? Does the program still exist?
