Reading Health System (PA) Develops New Podiatry Residency
Pennsylvania has a strong supply of doctors, but is struggling to keep its younger, newly graduated physicians in the state, according to data from the Association of American Medical Colleges. Dr. Kristen Sandel, associate medical director of emergency medicine at Reading Hospital, said retaining younger doctors, especially primary doctors, will be crucial as the state's older physicians retire and the federal Affordable Care Act is rolled out. More than 27 percent of the active doctors in the state are 60 or older, according to the report.
She noted that Reading Health System has developed a new residency program in podiatry and surgery and is beginning recruitment for an emergency medicine residency in addition to its other programs. She said the hope is that some of the "best and brightest" from those programs transition from residency to becoming attending physicians.

JAPMA Special Issue Focuses on Podiatry and Falls Prevention
Many falls among the elderly are related to bad balance caused, in part, by poor foot health. A special edition of the Journal of the American Podiatric Medical Association (JAPMA) explores a promising new virtual reality program designed to address these issues by improving balance and stability, helping to keep elderly patients on their feet. The studies included in the special issue demonstrate multifaceted podiatry interventions that reduce the risk of falling, including lower extremity exercise, non-slip socks, special footwear, and surgical foot deformity correction.
Donna Flenz, a 63-year-old diabetes patient, had become fearful of taking a tumble. Rightfully so—she’s fallen eight times this year. “The worst fall was in the backyard. I broke my clavicle,” she said. Flenz is one of 29 participants in the first-of-its-kind study at the University of Arizona, where researchers are testing diabetes patients with peripheral neuropathy. The innovative virtual reality program uses wearable sensors that provide interactive visual feedback in real-time.

OH Podiatrist Honored by Memorial Hospital
The Memorial Hospital Board of Trustees honored local podiatrist Dr. John Clarke on Dec. 3 for his many years of service to Memorial Hospital and the Memorial Hospital Board of Trustees. Clarke was recognized with a proclamation that was presented to him by chairman of the board Al Mehlow. Clarke served on the board since 2004, and his term expires at the end of this year.

Clarke joined the Memorial Hospital medical staff in 1980. While on the Memorial Hospital Board of Trustees, Clarke served on various committees, including nominating, physician recruitment, joint conference, bylaws, ethics, and professional affairs. Clarke served as chairman of the board during 2009 and 2010.

Dr. Chet Evans was invited to attend and participate in the summit as chair of the Governor's Diabetes Advisory Council.

IL Podiatrist Lectures on ACOs and Metrics
On Friday December 6th, Dr. Sev Hrywnak gave insight into the future of podiatry to a sold-out audience in Chicago. The Illinois Physicians and Surgeon Management Services sponsored this dinner event for podiatric medical professionals and residents. Recent surveys show that when ACOs evaluate podiatry, the cost savings are based not on surgery but more on preventive care. “Currently podiatry metrics do not support a podiatrist becoming part of an ACO. We have to change this,” stated Hrywnak.

The lecture focused not only on metrics, but also on clinical integration of medical home models and quality reporting. Following the success of this event, the Illinois Physicians and Surgeon Management Services are planning to host more of these lectures in order to help give direction to today’s podiatric physicians.

| EHR FRAUD NEWS | |
Feds Eye Crackdown on Cut-and-Paste EHR Fraud
Healthcare fraud can be as easy as hitting Control-C, Control-V. Federal officials say the cut-and-paste features common to electronic health records invite fraudulent use of duplicated clinical notes and that there is a need to clamp down on the emerging threat. That concern is enhanced by the fact that it's too easy to turn off features of EHR systems that allow tracking of sloppy or fraudulent records.
In an audit report released Tuesday morning, HHS agencies confirmed that they are developing comprehensive plans to deter fraud and abuse involving EHRs, including guidelines for cut-and-paste features. The issue arises at a time when critics say federally subsidized digital patient record systems are sometimes being used inappropriately by providers to drive up reimbursement.

Query: Floating Toe
The original complaint of this middle-aged working woman was a painful callus under the second metatarsal head. She had surgery about a year ago by another physician and now has a floating toe, hyperpigmented scar, stress fracture, and claims she is worse off than before the surgery. She also has noticed her big toe drifting laterally since the surgery. She has asked me to do what I can surgically to improve it.

In the past, I have used complete soft tissue release and a silicone double stem implant to keep the toe somewhat stable after at least 4 weeks of K-wire. I have also used GraftJacket as an interpositional graft, but I was not pleased with that over time. Another consideration here is the first metatarsal, which is becoming unstable and drifting. Suggestions are welcomed.

The standard of care for DVT prevention now is a sequential compression device on one or both lower extremities during the surgery (any surgery).
There is no such thing as a healthy almost pack-a-day smoker! You have before you the perfect opportunity to explain to your patient exactly what nicotine does to the capillaries in the body. The fact that she's taking anti-depressants and she smokes nearly a pack of cigarettes a day makes me believe she's obsessive compulsive. If she can't be a partner in her health, I would not do surgery on this unhealthy 24-year-old female!

Although I don't know the laws in your state, don't you think this is a very good time to get a relationship going with your local infectious disease doctor? I have worked with many ID doctors and found that it is much more beneficial for the expert in this field to give you a helping hand. It doesn't mean you don't know what to do. Rather it means you know what the standard of the community will allow. Sending a patient out for care will come back to you ten-fold.
Years ago, when antibiotics were new, family physicians frequently gave injections of penicillin both in their offices as well as at house calls. If you had a sore throat, you got a shot of penicillin, generally in the buttocks. The problem encountered was allergy. Many people died from these injections because the family doc did not have the ability to control anaphylaxis in the office or home setting. Penicillin is one drug that often causes anaphylaxis in those allergic to it. The bigger question, however, is whether giving an IM dose of an antibiotic can do more than oral meds, or should one admit the patient to the hospital and start the patient on IV antibiotics if larger doses are really called for.

I suspect that the child has autism or an autism spectrum disorder. I saw this before in a friend's child who also did not verbally communicate, walked primarily on her toes, and bounced a lot. If I remember correctly, she was instructed to have her daughter wear weighted shoes and do stretching exercises with her. If the parents have not yet considered an autism spectrum disorder, it will probably be a difficult suggestion for them to hear. So be prepared with a list of specialists or websites that have more detailed information.

Once upon a time, there was a young lad who wanted to be liked by all the other kids. The children in the neighborhood would play but never included him in any of their games. Sam complained to his mother who said why don't you get really good grades and become a podiatrist? That'll show those mean boys and girls!
