CA Podiatrist Discusses Common Injuries in New Runners

The most common running injuries are shin splints, runner's knee, plantar fasciitis, Achilles tendonitis, and some type of foot fracture, according to Dr. John Pagliano, a Long Beach sports medicine podiatrist. And there's more bad news - they'll probably keep popping up for the first year for new runners.

Dr. John Pagliano

Pagliano recommends strength workouts three times a week on alternate days, which should include upper body strengthening. And if the pain lingers for more than two weeks, that's when you should go see someone.

15-Minute Daily Walk Can Extend Life 3 Years: Aussie Podiatrist

Dr. Phil Vasyli, a podiatrist and founder of Orthaheel and Vionic Performance Footwear, notes that a 15 minute bout of exercise each day can increase your life expectancy by three years, even if you have cardiovascular disease risk factors. Daily physical activity is especially important for older adults, because maintaining an active lifestyle into the senior years has been shown to reduce blood pressure, inflammation, blood sugar and other markers of chronic disease.

Dr. Phil Vasyli

Exercise can also boost immunity. Additionally, getting outdoors and exposed to some natural sunlight at any point in the day can help regulate the body’s production of melatonin, a hormone that controls the sleep cycle, says Dr. Vasyli.

AT THE COLLEGES

Noted NY Endocrinologist to Speak at Barry

Barry University’s Paul & Margaret Brand Research Center recently launched its new national lecture series highlighting vital topics in podiatric medical research. The second lecture in the Distinguished Lecture Series, titled "Emerging Trends in the Management of Diabetic Lower Extremity Complications", will feature Peter Sheehan, MD, a widely known and acclaimed specialist in the field of diabetes. Sheehan’s practice has focused on the cardiovascular and lower extremity complications of the disease. He has a particular research interest in peripheral neuropathy, wound healing, Charcot osteoarthropathy, and peripheral arterial disease.

Dr. Peter Sheehan

The lecture will be on Jan. 12, 2012 at 6:30 p.m. at Barry University’s Andreas Hall. There will be no cost for the two-hour lecture, and two Continuing Medical Education (CME) credits will be offered to licensed podiatric physicians.

NY Podiatrist Displays 450 Sq Ft Building at Model Railroad Show

Hundreds of people showed up to watch the display of model trains by the Westchester Model Railroad Club at the Greenburgh Nature Center on Saturday. A replica of the Greenburgh Nature Center building was constructed by Dr. Paul Greenburg, an 84-year-old former podiatrist. "It took me a couple of months to build," said Greenburg, who said he had a 450-square-foot display at his home in Hartsdale. "This hobby is good for recreation, socialization, and corollary education," said Greenburg. "As a small kid, I was crazy about trains."

QUERIES (NON-CLINICAL)

Query: Timer for Whitehall Whirlpool Fillers

I am trying to find a source for an electronic timer that can be used in place of the mechanical timer in a Whitehall whirlpool filler. I contacted Whitehall directly and was given the name of a person who does a complete reconditioning of the filler. They do not seem to be in business anymore. Does anyone have any sources for this application?

RE: Botox for Spastic Contractures (Ray McClanahan, DPM, BS Ed)
From: Jay Helman, DPM

I have referred patients to a neurologist for Botox injections in cases of spastic contractures. The results worked for approximately 3 months. I have found the results to be satisfactory.

RE: Life-Long Maintenance of Onychomycosis
From: Paul Kruper, DPM,Stephen Peslar, BSc, DCh

SteriShoe shoe horn, Lysol, tineacide, ultraviolet shoe sanitizer. Mix 6 tablespoons liquid bleach with one quart of water and spray inside shoe QD and the shower/tub floor after use.

Paul Kruper, DPM Kingsburg, CA

Lysol spray contains ethanol & has to be a food safe disinfectant for use in kitchens, etc. I doubt that it can inactivate fungal spores in shoes or socks.

I did learn about a product called bioTEXT spray. The manufacturer claims that it kills fungus, viruses, and bacteria. It is available in a 10 pack of 60 mL travel size pump spray bottles. Contact the manufacturer for distributors.

RE: Allscripts Vendors are Not Created Equal
From: Scott Hughes, DPM

We purchased MyWay from a local vendor, who has turned out to be incompetent. After a failed ‘go live’ date and after all the recommended training, we were told that ‘this system won’t work for you’ by their trainer. No solution or refund was offered. Unfortunately, I learned the hard way that all Allscripts vendors are not created equal.

However, thanks to a fellow podiatrist, Brandon Macy, DPM of New Jersey, there is a happy ending - or at least so far, so good. We found Podiatry Billing Specialists (podbillspec.com/templates/index.htm) who have created outstanding podiatric content for MyWay.

RE: Starting A Practice (Nicole Rubackin-Hayward, DPM)
From: Edmond F. Mertzenich, DPM

I'm glad to hear you are starting out in practice and that you are curious on how to make it work. I have been in practice, both private and clinical, for over 20 years. My wife and I work with practitioners who are interested in beginning or expanding their practice. When speaking with practitioners, some of the information we require to assist in their endeavors is the same information you need to make your decisions.

"What is your business plan?" This should be where you want to be in 1, 3, 5, and 10 years. "What steps do you need to take to reach these goals?" Part of this would be how to brand yourself. Most times, this is enough to get them thinking in the correct direction. There are many other steps practitioners need to take to achieve success. You can’t build a house without blueprints. Building a business without a plan is a bad route to take. Using the suggestions in the other posts could be included in the steps to reach your 1, 3, 5, and 10 year goals.

RE: Why This Country is Going Broke
From: Barry Mullen, DPM

Okay, so subprime mortgages are a major economic factor. Now, let's turn our attention to what we know best, healthcare. Do we simply forget the hundreds, perhaps thousands of third-party insurance carrier CEOs, each making seven figure salaries who, through lobbying, "legally" bribe politicians to leave the healthcare death spiral status quo, instead of allocating needed healthcare dollars for...healthcare! What a novel concept!

Insurance premiums constantly rise while covered services dwindle...the healthcare dollar expenditure paradigm has grossly shifted from carrier to consumer, including FDA-approved healthcare services based upon EBM...how does an entire industry get away with this? It's VERY simple...

Editor's note: Dr. Mullen's extended-length letter can be read here.

RE: MN Podiatrist Supports Statewide Single-Payer Health Insurance System (Michael DeBrule, DPM)
From: Jim Shipley, DPM

I lived in Canada for 18 years and England for 2 years. Both have single-payer systems in place. I personally did not care at all for the English healthcare system due to their extremely long waits for even the most basic things. I think that the Canadian healthcare system is awesome as long as you're not really sick.

By my last count, the province of Alberta had 9 MRIs within its borders (almost all within university settings for educational purposes). Alberta's population as of April 2011 was just shy of 4 million people. I want to let those two facts sink in a little. The Canadian government voted to decrease reimbursement for MRIs drastically one year, and all of the MRI companies left the province.

Single-payer systems can drastically bring down a country's ability to provide complete and advanced healthcare. Once again, a large pay cut looms over our heads this coming January decided on by politicians, not by the free markets. Having personal experience with two countries that have them, I'm very much against single-payer systems.
