Congress Must Protect Patients from Hospital Greed: NM Podiatrist
 
"As a podiatrist in Albuquerque, I hear every day from patients struggling with the rising costs of medical care," says Janet Simon, DPM. Many of them share the same story: The care they’ve been receiving for years suddenly costs double or even triple — without any change in the quality or location of their treatment. The only difference? The logo on the office door. Patients are left feeling frustrated, confused, and deeply worried about how they’ll afford the care they need."

"Here’s what’s happening: Large hospital systems are buying up independent physician practices and rebranding them as 'hospital outpatient facilities'. This allows them to charge higher prices for the same routine services patients have always received, often from the same doctors in the same offices. These extra charges can quickly add hundreds, even thousands, of dollars to patients’ costs. Importantly, these fees aren’t improving patient safety or quality of care — they’re simply padding hospital profits. Consider the cost of an ultrasound. In an independent doctor’s office, the price may be $164, but once the practice is reclassified as a hospital outpatient facility, that same ultrasound now costs $339," says Dr. Simon.

Podiatrists Ranked #30 on Highest Paying Jobs List
 
As the scramble to retain employees eases, fewer companies plan to give raises in 2024 compared to 2023. Of all the companies surveyed by Payscale for its 2024 Compensation Best Practices Report, 79% said they would offer salary increases, a drop from 86% and significantly lower than the 92% reported in 2022. In 2024, the average pay increase was also down 0.3 percentage points to 4.5%. Big pay hikes became common during the coronavirus pandemic as companies sought to keep workers and replace those who had left.

Now that employees are no longer in the driver's seat as the job market cools, what jobs are the highest paid in the country? Stacker ranked the 50 highest-paying jobs in America using wage estimates from the Bureau of Labor Statistics, updated in April 2024.

PRACTICE MANAGEMENT TIP OF THE DAY
Beware of These Malpractice Landmines
 
EHRs are increasingly leading to malpractice lawsuits. While EHR-related lawsuits still make up only a small number of lawsuits, user error is the cause of 64 percent of EHR-related malpractice claims, according to the Doctors Company. Practices need to focus on eliminating several common problems, mostly tied to user actions, which have led to the majority of EHR-related suits.
 
5. Clinical Decision Support Alerts
 
Another prompt that goes ignored, often to the determinant of the patient and the doctor, are clinical decision support (CDS) alerts. These are clinically relevant educational materials that come up as the practitioner is documenting in the EHR. Practitioners should ignore these at their own peril, David Troxel, MD, medical director and secretary of the Doctor's Company board of directors says. If anything, physicians should document the reason why they overrode the CDS alert.

RE: Can a Nail Like This Ever Regrow Normally?

The short answer is “YES”, a gryphotic/mycotic nail CAN grow back normally if the root causes are properly addressed. Over the past 20 years, I’ve had more successes than failures in tackling these nails. In my experience, osteochondromas are only a small part of the overall solution. We find these growths need to be removed about 74% of the time.

As an aside, I no longer perform radiographic screening for these deformities because intraoperatively I was often getting surprised by chondromas once the offending nails were avulsed. These firm, protruding radiolucent chondromas must also be removed because they are just as problematic to nail attachment as true “bone spurs”.

Every time I see an Ingmar Bergman film, I’ve already seen a few times before, like “The Seventh Seal”, I don’t understand it again, but in a different way from the previous viewings. So it is with podiatry and all the issues we are now facing. I don’t understand these problems again, but in a different way.
 
The enrollment at our podiatry schools is declining as Dr. Jacobs states, while we continue to open more schools with enrollments in each class numbering in the 20s and 30s while the MD, DO, PA, and NP programs increase enrollment. I have two daughters who are NPs. One is a nurse anesthetist making an unbelievable salary working four days a week and taking call one weekend day every couple...

Dr. Jacobs points out a similarity between dentistry and podiatry. Both professions have limited licensure. It is important to note major differences between the two professions. I remember sitting on a bench when I was in school having a discussion with Gary Fields, DPM, who was my attending. Gary pointed out to my chagrin, that everything we do as podiatrists can and will be duplicated by another medical professional. I left that discussion with a bellyache. Unlike podiatry, dentists do not have competition from any other healthcare professionals. 
 
Is there any nurse practitioner, MD or PA who can fill a cavity or make a bridge? Another contrast is the marketplace. Not every person is or will ever be a podiatry patient. On the other hand, I don't know of anyone who does not see a dentist from time to time. As Dr. Jacobs points out, podiatry schools may be having problems with enrollment. Does anyone have any data on enrollment in Dental schools?
