WI Podiatrist Honored on 9/11 Memorial Fundraiser Run
Terrance Benda, DPM was the honoree on Day 6 of “30 Runs to Remember” to raise money for the Wisconsin 9/11 Memorial. Just 2 months prior to graduating from college, Dr. Benda was met with a choice: be drafted or enlist in the Navy. Dr. Benda decided to enlist in the Navy and served in Vietnam. One of Dr. Benda’s jobs in the Navy included a stint at the Great Lakes Naval Base, where he served in the neuro-surgical ward, helping soldiers with head and spine injuries.

Dr. Benda describes this experience as "sad" and "humbling" as many of the 18-, 19-, and 20-year-old soldiers would never walk again. His service and experience dealing with lower extremity injuries inspired him to pursue a career in podiatry, where Dr. Benda served the West Bend community for over 40 years. He feels that it is important to support the 9/11 memorial because people need to remember history and learn from it.

Query: Denial of Steroid Injection Codes
Medicare continues to deny any and all steroid injections (i.e. CPT 20550, CPT 20551, CPT 20552, CPT 20660, CPT 20605) using appropriate associated diagnostic codes. The denials are usually accompanied by: “REM: 115 not medically necessary.” The denials occur with and without modifiers LT/RT. Any advice from Codingline would be appreciated.

Response: The first thing I would do is check on your Medicare carrier’s website to see if they have a policy on these types of injections. If so, there will be a list of acceptable ICD-10 codes that are payable, as well as other coding/billing guidelines. If you are using an ICD-10 code that is maybe “appropriate” BUT IS NOT on the approved list, then your claim could be denied with the denial remark listed.
You could also ask the provider line for some assistance. They will not tell what the acceptable ICD-10 codes are, but they might be able to dig a bit deeper and see if there is more specific explanation for the denial. It could be even a technical issue with some information not coming across in cyberspace.
Lastly, check with your state association to see if this is a more widespread problem and not just your claim. They may need to contact your carrier about possible policy changes.

As the forum at Codingline comes to a close, I think we all owe them a great thank you and debt of gratitude. Yes, it is a business venture, but it has offered a great service to us and our profession. Thanks to the wisdom and entrepreneurial spirit of our late friend and colleague, Dr. Harry Goldsmith, and his family. His family has soldiered on to provide wisdom and information in the coding world in the absence of our good friend.
We as DPMs are master coders when compared to many specialties. This expertise comes from necessity, but also from those who have continued to assist us in becoming the best coders in medicine. I have been honored to assist in the continuance of Codingline and am grateful to have been part of it. Thank you Harry and to all the Goldsmith clan for allowing us all to have benefited from such service.

"Much of what we have been talking about for the last four decades is the attempt to be treated as equals to our allopathic and osteopathic colleagues," says Dr. Paul Kesselman.
I like to comment on people in our profession referring to MD and DO medicine as allopathic and osteopathic and then putting ‘podiatric medicine’ in a separate category, as if podiatric medicine wasn’t allopathic medicine. The poll in question does this same thing. There are a certain finite number of medical theories out there, including allopathic, osteopathic, homeopathic, chiropractic, native American Indian pan-theistic naturopathy, witch doctors, Eastern Indian Ayurvedic medicine, and a few others. Allopathic medicine has as its basis the idea of pathology from disease state: bacteria, virus, prion, spirochete, genetic dyscrasia, etc.
Unless I missed something critical in medical school, that’s the disease model we in podiatry follow as well. As such, podiatric medicine IS allopathic medicine. Podiatry is often stated as wishing to contribute with "allopathic" providers in the area, as if WE were not allopathic physicians. One of the myopic tendencies in our profession is to separate ourselves in like manner from allopathy, which makes no sense to me. Podiatry follows the allopathic theory of medicine. We ARE allopathic physicians and referring to ourselves as podiatric physicians with similarities to allopathic physicians (as if allopathy means "MD", which it obviously does not) shows either ignorance of what the term means or is a strange form of self-denigration I don't understand.
