NY Podiatrist Receives 6th Annual Ronald L. Soave Award
 
On December 8, 2022,  Corrine E. Renne Landau, DPM was presented the sixth Ronald L. Soave, DPM Integrity and Excellence in Podiatry Award at the New York State Podiatric Medicine (NYSPMA) Downstate Meeting. The inscription on the award reads as follows: “In recognition for your time and dedication to the podiatric profession and for your outstanding leadership in the NYSPMA.” The award was presented by the Kings Division president Joseph Santi, DPM, Nicholas D'Angelo, DPM, and Dr. Soave.

Dr. Renne Landau is a past president of the Kings Division, and founder of the award. She was lauded for her ability to bring people together, unselfishly help others, and get seemingly impossible tasks done. She sits on several state committees and is the first female podiatrist to receive this award. In attendance were APMA President Laura Pickard, DPM and many members of NYSPMA, as well as her husband, Laurence Landau, DPM, step-daughter Samantha Landau, DPM, and other family members.

BakoDx Hosts Resident Dermatopathology Rotation for 12th Year
 
Over the past 12 years, Bako Diagnostics has offered podiatric medicine and surgery residents a unique opportunity to spend two weeks at their Alpharetta, Georgia laboratory to gain knowledge and hands-on experience with lower extremity dermatopathology. Through active participation in case sign-out, instruction in all aspects of the anatomic pathology laboratory functions, and time spent in the microbiology laboratory, residents gain educational exposure to the most common nail, skin, soft tissue, and bone pathologies of the lower extremity.

This experience includes lodging during the resident’s visit, a nominal stipend for travel, and fulfills the CPME 320 requirement for podiatric residents to participate in a pathology or subspecialty rotation during their residency. Michael E. RESPONSES/COMMENTS (MEDICARE ADVANTAGE)
RE: Double Standard for Medicare Advantage Plans

The HHS OIG Fall 2022 report was recently released to Congress. On page 20, there are many referrals to seven inappropriate payments to a variety of Medicare “Advantage” Plans. Topping the list is Humana. The OIG claims that Humana in the time period studied falsified records to receive $34.4M worth of payments they received from CMS for risk diagnosis code risk assessments. If even half this amount is true, it is unconscionable that Humana is not severely fined, their executives terminated and subjected to criminal proceedings, and they should be banned from the Medicare program for ten years. This is no different from how other healthcare providers are criminalized, so the question is, why is the insurance industry treated different and preferentially when they commit fraud?
 
These OIG studies are great reads, but up until now, they have done nothing to stop the insurance industry’s abusive practices of denying “clean claims”, denying claims after prior authorization, ignoring CCI edits, “losing” charts sent for review and then claiming higher error rates to Congress, paying providers often less than 50% of Medicare, and this the last draw... falsifying data so they can be paid more from CMS. When will this madness stop? When will providers have the gumption to actually act out the famous quote, “I’m mad as hell and I’m not going take it anymore!” (from the movie Network), and Peter Finch it!

RESPONSES/COMMENTS ( PODIATRISTS IN THE NEWS) - PART 1A

Looking at a bunion deformity as an isolated entity and making the determination whether it should be surgically corrected or not is, in my opinion, a mistake. How often do we see associated painful deformities such as hammertoes, metatarsalgia, neuromas, 1st MTPJ degenerative arthritis, or arthritic sesamoids in individuals with the bunion being the underlying pathology? I opine more often than not. All of those associated deformities could possibly have been avoided by early correction of a mal-positioned and poorly functioning 1st MTPJ.
 
I understand that with surgical intervention, whether with a realignment osteotomy or a 1st tarsal-metatarsal fusion, comes the potential for complications. But as surgeons, it behooves us to mitigate these complications and not wait until a patient has a severe debilitating hallux valgus deformity which could then possibly cause the patient to have other problems requiring multiple surgical procedures, increasing disruption in a patient’s life, and potentially increasing the chances of surgical complications.
 
There is nothing normal about a hallux valgus deformity; however, not all bunion deformities have to be operated on. As experts though, I am sure there is agreement that the deformity must be addressed in most cases.

When I retired at the end of 48 years, my practice was exclusively private foot surgery for cash. I found that with performing mostly open incision Scarf bunionectomies, Weil osteotomies, and arthroplasties with modern fixation, almost all of the patients experienced very little or no pain post-operatively. Other than an NSAID, there was no pain medication necessary. Therefore, almost all cases were performed bilaterally and ambulatory.
 
Of course, post-op they mostly sat with their legs elevated with minimal walking, but there were no crutches, casts, or boots and they could take care of themselves. The patients were grateful that they could have all the work done on both feet in one operation. Most surgeons would not...

RE: A Valid Reason for Missing an Appointment (Keith Gurnick, DPM)

I had a patient who came in for his appointment. When I asked how his wife was, he said, "she died last night." I asked, "Shouldn't you be making arrangements or be with family now?" and he remarked "it's no big deal. I'll take care of it later."  
 
I told this to an opthamologist friend who said he had one even better.  A patient came in for her morning appointment and said her husband had just died about an hour earlier. "Oh doctor, it's so hard to get an appointment with you that I didn't want to miss this one. My husband will still be there when I get home!"
