PODIATRISTS AND SPORTS MEDICINE
DC Podiatrist Discusses 3-Step Treatment of Plantar Fasciitis

Dr. Stephen Pribut, a Washington, DC-based podiatrist and sports medicine and biomechanics expert, says that the main goal of all products targeting plantar fasciitis should be to reduce the amount of bending at the heel and arch to help give your plantar fascia a break so it can heal. That’s what supportive sandals do, and what compression sleeves and socks may also help with.

Dr. Stephen Pribut

Of course, some of the best things an athlete can do to alleviate plantar fasciitis pain are free. Pribut often prescribes athletes toe curls with a towel—curling up the towel for 20 seconds per foot—as well as gentle calf stretches done one to two times per day when your muscles are warmed up. For many athletes, combining these exercises with compression gear and supportive footwear may turn out to be the ultimate three-step attack plan to address plantar fasciitis as soon as it starts.

PODIATRISTS IN THE COMMUNITY
UT Podiatrist to Serve on County Selection Committee

Three Stansbury Park residents will be the initial members of a council that will select a committee of Tooele County residents who will study the county's current form of government.

Dr. Kim Halladay

On December 2, Tooele County state legislators announced Dr. Kim Halladay to be their designee to the appointment council. Halladay, who lives in Stansbury Park, is a podiatrist at the Tooele Foot and Ankle Clinic. In the past, he served as chief of staff at Tooele Valley Regional Medical Center.

VA Podiatrist Becomes Executive Director of Homeless Organization

Dr. Darlene Sparks Washington is a Portsmouth native, podiatrist, and since Nov. 3, the executive director of Portsmouth Volunteers for the Homeless. There, she does what she says she was made to do: "Help people get through tough times."

She earned a doctorate at the New York College of Podiatric Medicine. But podiatry, she learned, didn't fulfill her. "I believe God ... had a different design for my life," she said.

PODIATRISTS AND THE LAW
Court Revokes PA Podiatrist's Bail After Repeated Arrests

A Centre County physician accused of resisting arrest had her bail revoked Tuesday during motions court in Clearfield County. Dr. Sharon Pletcher, 57, Port Matilda, was also charged by state police with defiant trespass, disorderly conduct, and public drunkenness as a result of an incident in Morris Township on Aug. 23, 2015. She was free on $10,000 bail. Clearfield County District Attorney Bill Shaw told President Judge Fred Ammerman that in June, Pletcher was charged in a second case with simple assault, disorderly conduct, and harassment by Clearfield Borough Police. In addition, she pleaded guilty at the magistrate level in two other cases in Patton Township from August.

PRACTICE MANAGEMENT TIP OF THE DAY
10 Collection Tactics for High-Deductible Patients - Part 2

Patient collections are much more than collecting $10 co-pays: those days are long gone. Patients are paying a greater percentage of what you get paid and will be paying an ever higher percentage in the years to come. Smart and consistent patient collections tactics must be a part of every practice's financial strategy. Though the rate of healthcare spending may have slowed, the share absorbed by patients has done the opposite. Here are a few basics for improving your patient collections.

2. Know in advance what your patients will owe: Most clearinghouses permit real-time and batch eligibility processing. It is important to know in advance whether you should collect more than just a co-pay.

QUERIES (NON-CLINICAL)
Query: Pharmacogenetics Testing

During a pre-operative work-up for a bunionectomy, I gave patient orders for blood work and x-rays. The patient asked, “Aren’t you going to do a DNA test?” Apparently, other doctors in my area are doing DNA testing for “pharmacogenetics.” Is this becoming a popular thing? Is this something you order? And, if you do order it, what do you do with the results that you wouldn’t normally do without doing the test in the first place? t’m just wondering if I am missing the boat here.

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Steven Miller, DPM, Don Peacock, DPM, MS

As I explained two years ago in Foot & Ankle Specialist, potential bleeding events include surgical-site bleeding and hematoma formation. The American College of Chest Physicians Evidence-Based Clinical Practice Guidelines recommends deferring surgery so soon after placement of a coronary stent.

I assume you meant "antiplatelet" (such as aspirin and clopidogrel) instead of "anticoagulant."

Steven Miller, DPM, Skokie, IL

For this patient, it would be in her best interest to put off the surgery for 6 months.

Don Peacock, DPM, MS, Whiteville, NC

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: David Secord, DPM

"Current American College of Cardiology/American Heart Association guidelines suggest one month of dual antiplatelet therapy and a six-week wait before noncardiac surgery following bare-metal stent placement and at least 12 months of clopidogrel and aspirin after a drug-eluting stent is used and before noncardiac surgery. Data from 899 patients were included in the researchers' analysis of bare-metal stent outcomes. The drug-eluting stent study included outcomes data from 520 patients."

Recommendations:
Explain to interested patients that this study suggests that patients treated with bare-metal stents should wait three months before having elective noncardiac surgery.
Note that for patients treated with drug-eluting stents, the study suggests delaying surgery for more than 12 months.
Explain that current guidelines suggest a six-week wait following bare-metal stent implant and at least a 12-month wait for elective surgery following drug-eluting stent placement.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Adam Siegel, DPM

Credit card processors make their money from confusing small businesses about guessing how much they will be on the hook for when they sign up for processing services. If you are interested in flat rate processing, Square is an excellent alternative. They offer 2.75% per swipe for Visa, MC, and Amex with 3.5% +15c for keyed in transactions. There is no contract, no monthly fees, and no initiation fees. This is the simple option.

If you want to be more adventurous, you can search for a credit card processor that may have lower rates, but will also require a contract, monthly rates, and high initiation fees. This may, however, be worthwhile if you do more than 10k a month in credit card transactions.

To simplify the process, go to cardfellow.com. This is a free credit card processing aggregator that will help you find the best rates for your business. You input basics about your business (how much you take in from CCs/month, what types of cards, etc.), and it will match you with a rate that best fits your specifications.

RESPONSES/COMMENTS (OBITUARIES)

It was with sadness that I read the other day of the passing of Angelo Monaco, DPM. He truly was a pioneer with a lasting impact on our profession. Prior to 1984, I had read and heard of his leadership in the state of Pennsylvania and had seen him at the Hershey Surgical Seminars. After my first 3 years in the USAF, the military offered to send me to a surgical residency of my choice and would fully fund me to attend a surgical program. With three years from graduation from PCPM, I was having difficulties competing  with recent graduates for a program. I called Dr Monaco, who at the time was the director of the program at the Hospital of the Philadelphia College of Osteopathic Medicine, Philadelphia, PA. After a phone interview, he accepted me to his program!

After my surgical year, Dr. Monaco worked with the military to provide a one-year surgical program at his hospital for additional military DPMs. In the USAF, we do our jobs with the following "core values" of commitment  to excellence, integrity, and service above self. I know that Dr. Monaco's entire professional life embodied those values. It is times like these, when we hear of the passing of prior mentors, that we wish we talked to them more and thanked them for guiding us through our professional lives. Luckily, I was able to thank him every year since completing his program in 1985 through this time last year. There are dozens of podiatric surgical residents who are better DPMs because we attended his program. My prayers this past week are for his family. On behalf of the military residents who matriculated through his program, we extend our condolences.
