NYCPM Announces Commencement Speaker
Todd J. Albert, MD, surgeon-in-chief and Medical Director and Korein-Wilson Professor of Orthopaedic Surgery at Hospital for Special Surgery, son of Earl A. Albert, DPM (’41), will be the keynote speaker at the 2015 Graduation. The younger Dr. Albert is a Professor in the Department of Academic Orthopedic Surgery at NYCPM, and is also Chairman of the Department of Orthopaedic Surgery and a Professor of Orthopaedic Surgery at Weill Cornell Medical College.

Query: Provider Enrollment Requirements for Writing Prescriptions for Medicare Part D
The Centers for Medicare & Medicaid Services (CMS) finalized new rules which require physicians and professionals who write prescriptions for Part D drugs to be enrolled in an approved status or have a valid opt-out affidavit for their prescriptions to be covered under Medicare Part D. This article was revised December 5, 2014, to emphasize that form CMS-855O is appropriate for use by prescribers. All other information remains the same. [SE1434]
I not sure what this means. I don't know what they mean by "approved status." Does being a participating provider equal "approved status"? Does any of this apply to us? It appears that if you are non-participating, there is an opt-out affidavit that is necessary in order to write prescriptions. If Medicare is setting this precedent, we may next see that health plans may not honor out-of-network prescriptions.

I also received three of these letters from Medicare. I found this in the guidelines from Medicare: " For home services provided by a physician using these codes (99341-99350), the beneficiary does not need to be confined to the home. The medical record must document the medical necessity of the home visit made in lieu of an office or outpatient visit."
United Healthcare Medicare has this in the guidelines: Home Visits (House Calls)
o Home visits made by a physician to a member’s private residence may be covered when medically reasonable and necessary and not for the convenience of the provider or the member.
o Although the member need not be confined to his home for the physician’s home visit services to be covered, the member’s condition must satisfy Medicare’s definition of ‘homebound”, and the medical record must document the medical necessity of the home visit made in lieu of an office or outpatient visit.
o Generally speaking, a patient will be considered to be “homebound” if he/she has a condition due to an illness or injury that restricts his/her ability to leave his/her place of residence except with the aid of supportive devices such as crutches, canes, wheelchairs, and walkers, the use of special transportation, or the assistance of another person or if leaving home is medically contraindicated. (For details, see the Medicare Benefit Policy Manual, Chapter 7 - Home Health Services, Section 30.1.1 - Patient Confined to the Home.)
o For more specific Medicare guidelines, refer to Medicare.
All of my housecall patients are elderly and cannot get to my office by themselves without assistance, so I feel they qualify. I Will soon find out if Medicare agrees!

RE: Union Membership for Federal Podiatrists

How many Federally employed podiatrists know that we are eligible for Union assistance during personnel actions? It's my understanding that all Federal professional members who are not Senior Executive Service (SES) are eligible. The American Federation of Government Employees (AFGE) Union is required to provide services to all professional class employees. They are currently having an end of year membership drive. They support us, so I think we should support them and join. I have joined; will you?

Start over by having "a hospital grade duplex receptor" and at least an independent circuit. I recall seeing this as an electrical requirement to Midmark's M417 models. If you are using other outlets on the side of the exam chair or in the wall outlet plug, then I suggest that you consider changing your circuit; especially if you are also using other medical equipment on patients. I really do not have independent "knowledge of the electrical circuits." Manufacturers sometimes say that the equipment has an end life. When I was trying to get another light bulb for our Burton outpatient surgical suite light, for example, they wanted to only sell me another entire assembly. Try a hand control for the same features, including auto return, as Midmark offers.

I hate to seem snarly or obnoxious, but we, as podiatrists, don't recommend that patients make their own orthotics or operate on their ingrown nails. We tell them that their problem requires professional help. This situation is no different. Because the Dr. didn't know what he was doing and screwed it up, just like our patients, asking us to fix it is like going to the Internet to fix the complications of self-treatment of a medical problem. Call a professional and get it done right.

RE: PQRS Registry

There is a situation that is very unsettling for (probably) most podiatrists. It’s called PQRS – Physician Quality Reporting System. Mike Brody has lectured extensively along with the APMA’s Jim Cristina, but I’m finding that MANY of the doctors have not registed with PQRS. In an era of fees going south or not keeping up with expenses, podiatrists are going to be penalized 2% in 2016 for not completing the requirements for 2014.
In a nutshell, forgetting about any incentive “payment”, doctors had to report on 3 measures in 2014 to escape the 2% penalty. Most doctors who did PQRS, did it via claims reporting and they will continue that until the end of this year. Their perception is that if they did not do this, they have “locked” in the 2% penalty. That’s not true. They can report via a REGISTRY and yes, ICS Software is a registry. This means we can take the data from 2014 and convert it to the data CMS wants to have – even without sending in claims or anything else. All reports need to be received by CMS by the end of February 2015, and we will cut off requests to submit PQRS data in the middle of February or so.

Thank you, Richard Willner, DPM. Dittos on your comments regarding Dr. Arthur Helfand. Many of the younger practitioners do not realize the icon this giant is in the field of the diabetic foot, and secondarily opening up the field of wound care to podiatrists. It is also noteworthy that he has authored and co-authored over 400 publications in national and international journals and textbooks. He is past president of the APMA, the founder of two major podiatric medical boards, and served as the first chair of the podiatric health section of the American Public Health Association. This is only a tiny example of hundreds of his achievements.
In March 2014, Dr. Helfand was honored by the Academy of Physicians in Wound Healing (APWH), receiving the organization's first "Outstanding Achievement Award." This is another terrific achievement from an organization that has a 50-50 balance of allopathic to podiatric physicians. He is a world opinion leader who blazed new trails and set the stage for successors such as Larry Harkless, DPM.

He truly loved our profession and our profession reciprocated. One of my daughters reminisced about him after his passing by stating, "He loved his work, everyday, always. He loved his life."
