MI Podiatrist Discusses the Lost Art and Science of the Physical Exam
According to Conway McLean DPM, "No matter the specialty, there has been an obvious trend in modern medicine: the physical exam is rarely performed. We have come to rely on tests and studies, providing as they occasionally do a definitive answer. Even specialties considered extremely “physical,” like physiatry, orthopedics, as well as podiatry, no longer stress the physical exam in school or clinical rotations. If the problem can’t be tested by some form of technology, then it doesn’t exist. But there is none that can evaluate the quality of motion of a diseased joint. No machine which can appreciate the subtle coloration in the early stages of a developing infection in the foot of a diabetic."

"Part of this trend away from a hands-on approach has been the development of new and better technologies. These allow doctors to examine structures inside the body never before visualized. Still, they can’t give the whole picture, they can’t feel a tender abdomen, test carefully a healing sprain. Admittedly, a physical exam isn’t necessary for every problem or by every specialist. But for most problems, a hands-on exam should begin the investigative process of evaluation for any medical condition," says Dr. McLean.

Beware of These Malpractice Landmines
EHRs are increasingly leading to malpractice lawsuits. While EHR-related lawsuits still make up only a small number of lawsuits, user error is the cause of 64 percent of EHR-related malpractice claims, according to the Doctors Company. Practices need to focus on eliminating several common problems, mostly tied to user actions, which have led to the majority of EHR-related suits.
4. Alert Fatigue
The e-prescribing module in an EHR is a potential landmine for malpractice. Alerts indicating when there is a problem with medication dosage or drug-drug interaction will pop up as practitioners are inputting data into the EHR. As a result, doctors get annoyed and develop alert fatigue, and they just turn the alerts off.
While this is understandable, turning off the alerts could mean a potential significant problem could go undetected.

It is possible but first you must get rid of the fungus. Sometimes you may have to use ½” paper tape at the distal end of the toe to hold down the soft tissue. I am trying something new and that is applying a KeryFlex nail to put pressure on the distal tip to flatten out as the nail grows over. I will report on if this works. Plan on treating for an extended period of time as these nails are difficult to convert to “new nails”, but I have done it on occasion.

The onychomycotic hallux nail can grow back normally if it is caused by an underlying subungual exostosis, either osseous or cartilaginous. I am retired but when I was practicing, I was 3 for 3 on correcting this condition. All 3 patients were in their 40s or 50s, and all 3 nails were normal after 1-2 years. The mycotic nail was removed 2-3 weeks before the bone surgery.

Dr. Freireich poignantly points out that "No other profession would put up with this year after year." And he is right. This is the point that I have been trying to get across for decades now. There is no profession that I know of that is as highly regulated and consistently exploited and abused as doctors. Every one of you who participates with insurance year after year, either by running a practice that depends on insurance reimbursements or are employed by PE, where insurance is still the method of reimbursement for the corporation are actually, perhaps unwittingly, feeding the monster.
As long as you "employ" the insurance companies to run things, they will continue to steal from you. And since they know you don't do anything about it, they steal more and more every...

Mark Twain stated, “Never argue with stupid people. They will drag you down to their level and beat you with their experience." With specific reference to the question of podiatry, there are realities which, in my opinion, are stupid to ignore. Stupid, meaning lack of common sense. Why is there a concerning decline in the application pool to the colleges of podiatric medicine? That is a reality. Those educational facilities awarding an MD, DO, PA, NP degree are overwhelmed with applicants. Why do all these alternative degrees maintain a broader scope of practice including diagnostic and therapeutic interventions than a DPM degree maintains? The reality is that of education. That is reality.

Most of my fellow classmates of ICPM ‘81 knew during our time at ICPM that Bob was destined to accomplish great things. He was always there to assist others and was always in a jovial mood.
I recall his hilarious role one year in the class follies, doing their version of the Village People’s YMCA, and changing the lyrics to ICPM. 40+ years later, I remember those lyrics better than the original. What I didn’t know then is that Bob was trying to teach us to not take things and ourselves too seriously and to try to learn from both the serious and absurd. RIP Bob.
