Student-run Clinic at RFUMS Expands Health Services
Founded in 2013 by four medical students at Rosalind Franklin University of Medicine and Science (RFUMS), the Interprofessional Community Clinic provides healthcare to underserved men, women, and children while offering early clinical skills training to medical and health science students who work in teams, under faculty supervision, to diagnose and treat patients.

Volunteer student clinicians come from a broad range of RFUMS programs including medicine, pharmacy, podiatric medicine, clinical psychology, and physical therapy. They work side by side, learning respect for each other’s professions and collaborating and leading in areas that also include administration, community outreach, clinic management, fundraising, and research.

AAPSM Awards the Dr. John Pagliano Golden Foot Award to Paula Radcliffe
American Academy of Podiatric Sports Medicine Board Members Drs. Amol Saxena and Brian Fullem awarded the newly-named Dr. John Pagliano Golden Foot Award to marathon world record holder Paula Radcliffe. The AAPSM Golden Foot Award is given to honor those individuals who help to promote a healthy lifestyle and the importance of podiatric sports medicine. Dr. Saxena organized a U.S. contingent, under the auspices of the AAPSM, to attend the 22nd Annual meeting of the German Association for Foot Surgery that was held in Munich, Germany last week.

Paula Radcliffe is the current women's world record holder in the marathon with a time of 2 hours, 15 minutes, and 25 seconds. She is a three-time winner of the London Marathon (2002, 2003, 2005), three-time New York Marathon champion (2004, 2007, 2008), and won the 2002 Chicago Marathon. She has represented Great Britain at the Olympics four times consecutively (1996 to 2008)

Gruber, Tavenner Apologize for Obamacare Miscues
“Inexcusable” was the word of the day on Capitol Hill. Both MIT economist Jonathan Gruber and CMS Administrator Marilyn Tavenner invoked it to describe their actions during a more than four-hour hearing Tuesday before the House Oversight and Government Reform Committee. Tavenner apologized for including nearly 400,000 dental-only plans in the tally of health insurance enrollments through the state and federal exchanges during a previous appearance before the committee. “It was an inexcusable mistake,” Tavenner said. “It should not have happened in the first place.”
Gruber sought penance for a series of videos in which he spoke in extremely injudicious terms about the passage of the Patient Protection and Affordable Care Act. Most notoriously, Gruber stated that Democrats exploited the “stupidity of the American voter” to shield the costs of the law and gain support.

ICD-10 Delay Appears DOA in Congress This Year
A proposal to delay implementation of ICD-10 diagnostic and procedure codes by an additional two years appears to be going nowhere in the current lame duck session of Congress. “That's not going to happen,” said a veteran healthcare consultant who tracks the issue closely, speaking on background. “The reports of them ever getting traction were overrated.” The Texas Medical Association has been lobbying for the two-year delay. The nation's largest state medical society for physicians, with 48,000 members, wants to push back the adoption date for the oft-delayed change to 2017.
The federal government is set to run out of money Dec. 11 if Congress doesn't authorize additional spending, making a funding bill a perfect vehicle for attaching special causes such as an ICD-10 delay since the overall bill must pass to keep the government open. But the proposed ICD-10 delay, which would infuriate other interested healthcare parties that have been moving forward with plans to implement the coding changes next year, doesn't appear to be going anywhere.

Query: Medicare Challenging Home Visits to Patients
We have been receiving letters from Medicare challenging home visits. In all three cases, they asked for justification as to why the patients had to be seen in their homes as opposed to coming to the office. In all of these cases, there are underlying physical problems that make it very hard or even dangerous for the patients to come to the office. One is on hospice with pulmonary support, another has gangrene and dementia, and another is being treated for agorophobia and develops anxiety attacks when she is with people. All three of these patients are housebound with the exception that they will take ambulettes to see their primary care doctors, and going to the doctor in each case is a major project for these very sick people. Has anyone else received these letters and if so, how have you handled this problem?

Editor's comment: PM News does not provide legal advice. These individuals appears to have severe mobility problems, however, this query begs the question, "If these patients can take ambulettes to see their primary care physicians, why can't they take ambulettes to see a podiatrist?

Query: Return Switch Repair on PDM Chair
I have a PDM chair that needs some help. I tried to replace the return switch and must have crossed some wires. When I plugged the foot pedal into the chair, it shorted out a circuit. Would anyone with knowledge of the electrical circuits on the PDM chair respond with some help with this problem?

Query: Billing for Custom Orthotics
Are we required to bill at time of dispensing the foot orthotics? Or can we bill at the time of casting for foot orthotics?

Response: All items of DME are or should be billed at time/date of dispensing - NOT time of casting, or evaluation. In fact, if your medical and billing records disagree on date of billing/dispensing, most payers (especially Medicare) will reject and/or audit the claim, then reject the claim.
On post-payment review (with Medicare), if your records do not reflect a billing date as the same date of dispensing, they consider the service invalid, and "ask" (a kind term for what they actually say, and do) for the money back.

A silent icon of podiatry, Art Helfand continues to work with amazing energy, promoting the profession to the world of geriatric medicine. He does not get the attention or accolades he deserves but then he does not seek them out in any way. He does not self-promote; he promotes the profession with his humility, intelligence, and passion. He has been directly and indirectly responsible for many of the privileges we all enjoy and take for granted. I have the privilege of continuing to work with Dr. Helfand on several projects, and it is one of the great joys of my life. He was my residency director, now he is my mentor, colleague, and most importantly…my friend. I am one of the proud St. Luke’s 440!

I am very appreciative of Dr. Willner's remarks. For your records, the total number of residents we had from 1957-1989 were 440. The real credit belongs to the late James C. Giuffre', MD. He was a professor of Temple in the 1950s and believed that there was a need to train our practitioners in a manner equal to and compatible with the training of other doctors. Our license to practice has never been limited. We are defined as to scope by our state practice acts as public health policy. That is no different than the delineation of privileges for all others practicing medicine and/or dentistry.
The key issue in our profession is to be very proud of who and what we are. In the 1960s, there was a healthcare project at the Queensbridge Housing Project in New York. The article about the study was published in Public Health Reports. It was the former and late Mayor of New York, Robert Wagner, who stressed the need for our profession, especially for the elderly. We just need to remember his words from time to time.
