CA Podiatrist Discusses Shoes to Wear Around the House
 
Ultimately, your feet will determine what is best to wear around the house. “If someone has foot problems and pain, it’s best to wear supportive shoes with socks, even inside the home,” said Wenjay Sung, DPM, a podiatrist at Global Podiatry Partners at Methodist Hospital in Los Angeles. “If someone has no foot problems, barefoot is acceptable.” He added that for most Americans, wearing slippers around the house is the safest option, as a well-designed slipper will provide support and protection for the foot, in addition to providing flexibility and comfort.

Dr. Sung added that house shoes that lack support will put pressure on the toes and muscles in the foot and cause excessive gripping from the toes, leading to pain and other issues. Indicators that you’re not wearing proper footwear around the house include pain, blisters, and calluses. “If a corn forms at the end of the toes, there could be excessive gripping by the toes. This indicates that the shoe is causing the foot to overexert itself,” he said. “Blisters are another warning sign, especially when there is excessive sweat and rubbing on pressure points to the foot.” If you’re experiencing either of these, it’s time to get rid of your slippers and find something else.

New Neuro One Topical Cream Promises PN Relief
 
California-based Neuropathy Research Inc. announced the release of its L-arginine-based topical cream designed to relieve the symptoms of peripheral neuropathy (PN). PN refers to pain, tingling, and numbness in the feet or hands suffered by as many as 30 million Americans, the majority of them diabetics. Causes of PN include diabetes, vitamin B-12 deficiency, excessive alcohol consumption, and age.

All that was what company co-founder Tony Cook learned from his doctor, Arlene F. Hoffman, DPM, PhD, a veteran podiatrist and medical researcher in San Francisco. “She told me my peripheral neuropathy was idiopathic – meaning cause unknown,” recalls Cook, “and the doctor dispensed a topical formulation that she said had helped the many PN patients in her care.” It worked, explains Dr. Hoffman, because the L-arginine could reach the damaged nerves causing the symptoms, since these spidery or “small-fiber” peripheral nerves were close to the surface of the skin and could therefore be readily nourished.

QUERIES (CLINICAL)
Query: Fractured Bipartite Sesamoid
 
I am treating a friend and fellow golfer in his 70s who is having significant pain around the tibial sesamoid area. He originally had x-rays and most recently an MRI that was ordered by his internist. I was able to get the reports and it was diagnosed as a "fractured bipartite sesamoid bone" along with adventitious bursa due to pressure on the area. Clincal findings of pain and erythema mostly around the tibial sesamoid area are consistent with the diagnostic testing. I was not able to see the actual x-ray to confirm the fracture. I assume the radiologist did see a fracture within the bipartite sesamoid and was not referring to a bipartite sesamoid as a fracture. 
 
I am trying to get a copy or disc of the x-ray to confirm this. Possible bilateral x-rays will be ordered to further confirm a bipartite sesamoid. I injected the bursa area with cortisone with approximately 50% improvement noted two days later. Further treatment with insoles and accommodative shoes was discussed. I am thinking of Hoka Bondi 7 athletic shoes because the rocker sole provides some off-loading to the sesamoid area. I would appreciate any input concerning the treatment of this condition.

QUERIES (DME)
Query: DME Denials
 
Our office is being inundated with CMS DME denials from Performant and Celerian, going back to 2018. These are for shoes, night splints, and AFOs. All of our "i"s are dotted and "t"s crossed...and we have appealed multiple times, correctly! We know that these appeals are not being looked at because they say we don't have the information that is clearly included in the documentation. We are strongly considering not offering DME for CMS anymore. What other choices do we have...my congressperson?

Dr. Meisler asked about rural payments but he mentioned something even more interesting. I assumed that the PPP loans if they were forgiven are counted as ordinary practice income. Dr. Meisler mentioned that they may not be. If the PPP loan is forgiven, is it income or non-taxable? Is there an IRS determination on this?

Q. May a healthcare provider that receives a payment from the Provider Relief Fund exclude this payment from gross income as a qualified disaster relief payment under section 139 of the Internal Revenue Code (Code)?
 
A: No. A payment to a business, even if the business is a sole proprietorship, does not qualify as a qualified disaster relief payment under section 139. The payment from the Provider Relief Fund is includible in gross income under section 61 of the Code.

This issue has been discussed for quite a few years in this forum. Why is it that pharmacists can administer many vaccinations and podiatrists and dentists who give injections all day long, cannot? During the height of the first wave of the COVID-19 pandemic, some states in the U.S., including my own, issued temporary permission for podiatrists to administer COVID-19 vaccinations but only under the supervision of one of the other professionals.
 
A number of our colleagues took advantage of this and generously volunteered their time giving shots at mass vaccination centers. The fact that this did not lead to allowing us to give flu and COVID-19 shots permanently, does not do justice to the public, especially in areas where a podiatrist or dentist is the only healthcare professional that some elderly people see.

Reinforcing the notion of a Cinderella service since time immemorial, the podiatric physician-surgeon is not 'permitted' to administer COVID-19 vaccinations. There is an extensive, and growing, online debate about the fact that those staff, with lesser training and education, yet legally empowered to do so, have no idea about aspiration, prior to injection. 
 
Increasingly, the intravascular injection is now linked to adverse health sequelae, such as increased coronary syndrome with a spike in myocardial infarctions. This important aspect of injection technique is second nature to all podiatrists. This is a global problem. Our 'leaders' are again failing the nations of the world adding to their catalogue of shame in managing this 'crisis'.
