CA Podiatrist Delivers Keynote at the 7th International Wound Congress
David G. Armstrong, DPM, MD, PhD, Distinguished Professor of Surgery and Neurological Surgery at the University of Southern California (USC), delivered the opening keynote address at the 19th National & 7th International Wound Congress. The event, held at the Titanic Mardan Palace in Turkey from December 4–7, 2025, gathers global leaders to discuss the latest innovations in tissue repair and limb preservation.

Dr. Armstrong opened the scientific program on Thursday evening with the keynote address, “The Power of Multidisciplinary Approach in Wound Management”. In addition to the keynote, Dr. Armstrong is presenting two invited lectures focusing on surgical intervention and technological advancement: “Diabetic Foot Surgery to Prevent Amputations and Save Lives,” focusing on prophylactic surgical techniques and “Artificial Intelligence and Digital Health Applications in Wound Monitoring and Treatment,” exploring how AI is streaming healthcare directly to the patient.

Hydrogen Sulfide: New Hope for Nail Fungus Treatment?
Forget everything you think you know about foot care. That stubborn, unsightly nail fungus you’ve been battling for months – or even years – might finally have met its match. And the unlikely hero? Hydrogen sulfide, the gas responsible for that unmistakable rotten egg smell. “Patients are often left feeling hopeless,” explains Sarah Chen, DPM, a podiatrist based in New York City, who wasn’t involved in the study but reviewed the findings. “The current options are either risky or ineffective, and the recurrence rate is high. We desperately need something new.”
That “something new” might just be H2S. A recent study published in Scientific Reports by researchers at the University of Bath and King’s College London (KCL) revealed that hydrogen sulfide doesn’t just reach the fungus under the nail – it actively kills it. The gas appears to disrupt the energy production within fungal cells, essentially shutting them down. What’s more, it’s effective against fungal species that have become resistant to traditional antifungal medications. Researchers estimate a topical H2S treatment could be available within the next five years, pending successful trials and regulatory approval.

Strategic Options for Physicians Facing Challenges in Private Medical Practice
Here are some strategic options for physician practices to consider in this evolving, competitive market:
7. Contract with a reputable physician practice management services organization (MSO) that provides services for a fee (without buying the practice’s assets or equity). Another option that should be explored (assuming physicians are not looking for a partnership involving some cash proceeds) is a specialty focused practice management company that charges a fee for seasoned “C-suite” administrative services and strategic advice, without buying the practice or its assets.

Year-End Tax Planning Can Protect You From a Big Tax Bill
On individual tax planning, look at the overall impact on 2025 and 2026. The goal is to reduce your federal tax liability over both years, not just one.
Use your annual gift tax exclusion. You can give each person up to $19,000 this year, $38,000 if you are married, without paying gift tax, filing a gift tax return or tapping your lifetime estate and gift tax exemption, which is $13,990,000 for 2025. Also, recipients aren't taxed on gifts.
Any unused amount is gone forever. You can't give extra next year to make up for it. The 2026 gift exclusion amount stays at $19,000, and the 2026 lifetime estate and gift tax exemption soars to $15 million.
Here's an example. Say you're married with two kids and one grandchild. The two of you can give each of your children and your grandchild up to $38,000 ($114,000 total) this year in excludible gifts.

Is it okay to upload patient chart info to Chat GPT if the name of the patient is removed? For example, if a podiatrist had a dermatoscopic image to upload or a chart to upload with the patient’s name redacted, can a podiatrist use a local language model on your local computer or Chat GPT? How does a podiatrist comply with HIPAA regarding this choice?

Dr. Kesselman brought back fond memories of when I first started my podiatry practices. I had asked the former owner of what is now Henry Schein, where to open up. "Should I open up in an area where there are no podiatrists or in an area that may have many podiatrists?" He strongly advised me to open my first office in an area with many podiatrists, for the very reasons Dr. Kesselman stated. If there are no podiatrists, the community does not know what a podiatrist is. But in a community with many podiatrists, there might be some competition, yet the community is at least aware of how a podiatrist can address their foot problems.
I opened in three locations. I opened two where I was the only "kid on the block" and one where there were lots of colleagues. The adviser was right: I did better in the podiatry-busy area and had to wait many years to build a practice in the two areas where I was the only podiatrist.

I graduated NYCPM in 1980. Back then, podiatry wasn’t at all happy with what it was but also had no clear plan to make things different. I won’t go into what didn’t happen over the past 45 years. Suffice it to say there is a large number of podiatrists today (way too many) who are loathe to identify themselves as podiatrists. They are “foot and ankle surgeons”. That says a lot about the morale of this profession and how our colleagues feel about their status among MDs and DOs.
Now there’s talk about the DO degree as if that will solve all the problems our profession faces. I’m not going to say it’s a bad idea. In fact, I think it would be a good one. But it won’t change things for many years to come and does not address our challenges now. We have an obligation to every DPM practicing in the U.S. today to do something now. Something that will change things for the better. I opine we have wasted decades worrying...
