US Foot and Ankle Specialists (USFAS) has expanded with its recent acquisition of Foot and Ankle Specialists, adding five podiatry offices in Largo, Tampa, New Port Richey, and two office in Clearwater, Florida. Bruce Levine, DPM is a graduate of the Ohio College of Podiatric Medicine (OCPM) and completed his surgical residency at Southeastern Medical Center, North Miami Beach. Ted Couluris, DPM is a graduate of the California College of Podiatric Medicine and completed his residency at Washington Hospital Center in Washington DC. Jason Levy, DPM is a graduate of  OCPM and completed his residency at JFK Medical Center in Atlantis, FL.

Query: Billing E/M and Injection Codes
 
A new patient presented to my office for heel pain. I diagnosed plantar fasciitis and taught the patient posterior calf stretches. We discussed shoes and over-the-counter arch supports. I injected the plantar fasciitis at both feet and at the same visit, prescribed an NSAID. Is billing an E/M code acceptable along with the injections appropriate in this case?

Response: To bill an evaluation and management code, you not only have to evaluate (perform an examination) but also manage the problem. Educating the patient about what the problem is, treatment options such as stretching, education on proper footgear and supports, and prescribing an NSAID is management.
 
You also happened to inject the heel. So you should be able to bill for both. Remember, it ultimately boils down to what you documented!

The word “aching” brings to mind a couple of thoughts:
1. Chronic venous insufficiency. I know you said arterial status was intact, but what about veins? I’ve seen this cause aching pain even without obvious swelling or varicosities. Send for venous mapping/testing. Try compression stockings.
2. Any alcohol use/abuse? I’ve seen this cause generalized leg aching symptoms, particularly at night.

Within the Coding Pearls article that I wrote that was published in the most recent TLD Newsletter, I addressed the changes that were going to take effect on January 1, 2024 with respect to the CPT codes for the Correction of Hallux Valgus Deformities that specifically address CPT codes 28292, 28295, 28296, 28297, 28298 and 28299. The specific issue at hand that was brought up by Dr. Charles Morelli was in reference to CPT code 28297.
 
Prior to January 1, 2024, the definition of CPT code 28297 has been the following: Correction, hallux valgus [bunionectomy], with sesamoidectomy; when performed, with first metatarsal and medial cuneiform arthrodesis, any method. Based upon this definition, the terms correction, hallux valgus, and bunionectomy appear to be...

Back in 2015, I decided to sell my main practice. I did a good deal of research, including talking to a couple of consultants as to the valuation of my practice. One of the conversation’s fee was around $5,000. I was surprised at that number because it did not include his coming to my office to see the actual physical make-up and getting a feel for the type of practice I had. If you are going to pay a consultant that kind of fee, getting to know what type of practice you have including staff, patients, physical office, working environment, and other practice traits should be taken into account. 
 
I went a different route. I advertised my own practice by putting together a detailed description, including many pictures. My feeling is that nobody knows your practice better than you. As for valuation, to me it was simple. Start off with what you grossed for the last few years and work from a percentage off of that. My valuation was 100% of my average three year gross. 
 
The current market value will determine what you end up getting. Believe me, potential buyers are not intimidated by what your valuation is and they will not be afraid to make an offer they feel is right. In my case, I had a buyer within six months and the sale was completed within ten months. My selling price was 70% of my valuation. Don't let the so-called experts start giving you formulas that will make your head spin and ending up with a valuation that is ridiculously low. Take the time to market your practice. You will come out ahead.
