2021 Year-end Financial Planning Checklist
The end of the year is a good time to check in on your financial plan to see if everything is on track or if some things need adjusting. You will be in a better position for 2022 the sooner these items are addressed and adjusted if they are off-track.
8. Update Beneficiaries
Major life events can result in a need to update your beneficiaries. If you got married, divorced, had a birth or death in the family, you may need to update the beneficiaries on your retirement accounts and life insurance policies. These events may also necessitate updating your Will and Power of Attorney documents. There is no deadline for these changes, but the sooner the better.

Query: Current Telemedicine Rules
Where may I find the current published 2021 Telemedicine document as it directly relates to the Rules and Guidelines of Jurisdiction E – Medicare?

Response: It is quite possible that your MAC does not have an LCD on telemedicine plus the rules have changed with COVID-19. You might try your MAC Home Page and see if there is a link to Telemedicine issues within the PHE.
Whenever you read an LCD which seems illogical or unfair, go where the Medicare Administrative Contractors go for their directions... The Congressional Federal Register (CFR) which provides information on the Medicare fee schedule and many rulings for Medicare for the following year. It is released each November. You should be fair warned this document is over a thousand pages, most of which will not pertain to DPMs at all. Download the PDF and then search for telemedicine. I would go back a few years to where all the CPT codes started and work your way forward.
Another option is for APMA members to simply log onto the APMA website and search for telemedicine. The APMA Health Policy Committee has done a fantastic job of providing a slide show, podcast, etc. on telemedicine both prior to and subsequent to the PHE and has outlined many of the issues in easy to understand language. But if you really want to be comprehensive, get a subscription to the CFR (which is free!).

Richard Austin Nelson, a kind, gentle soul, known for his infectious laughter and compassionate heart passed away peacefully at Serenity House at 99 years of age after a long bout with heart failure.

Query: ARP Rural Payments
I recently received a payment from the HRSA (Health Resources and Service Administration) that was processed by United Health Group. It was part of the American Rescue Plan (ARP) Rural payment through the Provider Relief Fund Application and Attestation Portal. It was part of the American Rescue Plan Act.
Is anyone aware of how these funds are treated from a tax perspective? Is this considered income or is it not taxable income similar to the PPP loans that in many cases were forgiven?

RE: Charcot Marie Tooth and Orthotics

A recent article in the journal Neurological Sciences pointed out that Charcot-Marie-Tooth (CMT) neuropathy score and ambulation index are both predictors of orthotic need for patients with CMT. Further review of the article indicates that not all patients benefit from AFOs and some do just as well with foot orthotics as compared to SMO (supramalleolar orthotics which come just proximal to the ankle), while others do better with full length AFOs.
The main point of this detailed well thought out article is for the prescriber to conduct a full work-up of the patient and not simply prescribe any one type of device for every patient with CMT or for that matter every patient who is at a risk for falling or who has neuropathy. The patient's age, history, other risk factors and other diseases, and the extent of their symptoms must be accounted for in addition to a complete neurological and musculoskeletal work-up.

Borders don’t change this situation. In Ontario, Canada, Pharmacists, RNs, NPs, and RPNs (who after their GED, complete a 4-semester college program) are permitted, by law to administer flu and COVID-19 vaccines but not so for podiatric physicians. A pharmacist said that they are paid $13 per jab.
The container box has vaccines for up to 100 persons. ID is recorded from each individual and a laptop with access to the Internet and a printer are required. A crash cart with Epi-pens is also necessary to have on hand. The gross income made for a day administering 100 shots is $1,300. The real benefit is that it would be good PR for the podiatry clinic. It doesn’t happen. The pharmacy gets the public perception of providing a beneficial medical service.
