Holidays Shopping Particularly Stressful on Diabetic Feet: CA Podiatrist
"Shopping trips, parties, and family gatherings can take their toll on our feet during the hustle and bustle of the holiday season. For those with diabetes, this increase in activity can place the foot at increased risk for complications like ulcers and infections," says Diane Koshimune, DPM, a podiatric physician and surgeon on staff at Kaiser Permanente, San Jose, CA.
|
|
Dr. Diane Koshimune |
According to Dr. Koshimune, people with diabetes must pay special attention to their feet. "Diabetes puts tremendous strain on the vascular system, which often leads to a narrowing of the blood vessels, significantly decreasing circulation in the lower part of the legs and feet. Poor circulation contributes to diabetic foot problems by reducing the amount of blood, oxygen, and nutrition supplied to the feet - resulting in increased injuries and hard to heal wounds. In addition to poor circulation, diabetics often suffer from diabetic neuropathy.

TX Podiatrist Resigns from Area School Board
A local doctor who served on the Pottsville Area school board for nine years handed the board a resignation letter at its reorganization meeting Wednesday night. “After countless sleepless nights and endless hours of thought, I have made a most difficult decision to tender my resignation from the Pottsville Area School District Board of Directors effective immediately,” Dr. Gary A. Cortese said in a letter he presented to the board at the Howard S. Fernsler Academic Center.
|
|
Dr. Gary Cortese |
“This difficult decision is the product of recent events within the boardroom that have led me to conclude that I can no longer be an effective board member because we are no longer working as a cohesive unit,” Cortese said in the letter dated Dec. 7.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
10 Collection Tactics for High-Deductible Patients
Patient collections are much more than collecting $10 co-pays: those days are long gone. Patients are paying a greater percentage of what you get paid and will be paying an ever higher percentage in the years to come. Smart and consistent patient collections tactics must be a part of every practice's financial strategy. Though the rate of healthcare spending may have slowed, the share absorbed by patients has done the opposite. Here are a few basics for improving your patient collections.
1. Have a financial policy: Every practice should have a financial policy and every patient should sign it. This document is in essence a contract. In signing it, the patient agrees to follow your rules regarding payment. Fees for no-shows, co-pays, deductibles, the forms of payment you accept: all should be included.

| QUERIES (CLINICAL) | |
Query: Surgery or No Surgery?
I have a patient scheduled for bunion surgery in about 2.5 weeks. I repaired her contralateral foot last year. She called me today and told me that she just had a cardiac stent placed 3 days ago. She discussed this planned surgery with her cardiologist, who basically told her that the surgery would be fine with him and that the only issue should be that he did not want her off her anticoagulant. Typically, he will leave patients on an anticoagulant for a year and then re-evaluate. She does not want to put off her surgery for financial reasons. This patient understands the risks and has asked me to proceed with her bunionectomy. Surgery or no surgery?

| CODINGLINE CORNER | |
Query: Tarsal Tunnel Injection Coding
The doctor is injecting a steroid/lidocaine combo to treat the tarsal tunnel complaint. What code would be best to use for this injection?
Katherine Sharp, Woodbury, TN
Response: I suggest CPT 64450 (injection, anesthetic agent; other peripheral nerve or branch). You can also bill for the steroid product.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 | |
From: Eric Edelman, DPM
If Chase/JP Morgan Chase is available in your area, they are likely to be the lowest cost option for credit cards processing. Chase Paymentech is one of the 5 direct processors in the U.S., so there is no third-party involvement. Almost everyone else you talk to is going to be a third party which will add another layer of service fees. Also, if you move your operating account to Chase, you will have your daily settlements in under 24 hours (Monday's transactions have funds available Tuesday, etc.). Their equipment cost is reasonable too; we just got new chip- compliant terminals for $350 that can go to CAT-5 cable and your Internet router for fast processing times.

From: Wenjay Sung, DPM, Tom Fusco, DPM
I understand the question of these Chinese vendors contacting various doctors to attend and speak at illegitimate conferences. I too have been solicited by various outlets for GFAC in China. Because I was unsure if some Chinese company hijacked Dr. Hardy's conference, I checked his website, but there isn't any updated info.
So I implore Dr. Hardy to update the GFAC website, so there's no confusion. Right now, the only site with info for GFAC 2017 is bitcongress.com/GFAC2017/. This company BIT is notorious for fake conferences throughout China.
Wenjay Sung, DPM, Los Angeles, CA
I was at the conference held in Cleveland in 2016 and I can assure you it was very much a real conference. Great talks and a much better run conference, better talks than I have ever attended. Dr. Mark Hardy is the one still heading this up and they announced at the 2016 meeting that the 2017 would be held in China. I highly recommend anyone who wants to do some traveling to attend this conference. I have no affiliation with GFAC.
Tom Fusco, DPM, Fort Walton Beach, FL

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 3 | |
From: Jeffrey Kass, DPM
Dr. Kerner states in his post, "we de-value our profession by not retaining parity with the other medical professions." I concur. That's why I, as a NYSPMA member, contacted a member on the executive board and voiced this concern. NYSPMA is now actively pursuing having podiatrists added to the list of those eligible to prescribe (I can't answer why it wasn't looked into when the original bill came into law.).
NYSPMA must pay attorneys/lobbyists to achieve this and other goals that ultimately benefit all podiatrists practicing in this state. This is one of the reasons why it is important to belong to an association, to achieve common goals that further our collective profession.
Jeffrey Kass, DPM, Forest Hills, NY
