PODIATRISTS AND SPORTS MEDICINE
Running a Race With Pain Not Worth the Risk: CA Podiatrist

Whether a runner can overcome an injury by marathon time depends on what the injury is and when it was sustained, said Dr. Kevin Kirby, podiatrist and former UC Davis track team member. When injuries occur a few months in advance, Kirby recommends swimming or cycling to maintain endurance while nursing running muscles back to health.

Dr. Kevin Kirby

If it's two weeks to race day when an injury strikes, the chance of sufficient recovery is slim. And if it hurts at the starting line, it's only going to get worse. "If you get in the race and you can't run normally, you should just stop," he said. "Just stop. Take the bus home. It's not worth it."

SPORTS MEDICINE RESEARCH
Running Does Not Cause Osteoarthritis in the Knee: Study

It's official: not only does running not cause osteoarthritis in the knee, it may even help to keep the problem at bay. That's the verdict from a new U.S. study from Baylor University that looked at data from 2,863 people and found that runners, regardless of the age they were when they ran, had less knee pain and osteoarthritis. The findings, presented at the American College of Rheumatology annual meeting in November, add to growing evidence that running isn't the arch-enemy of knees it's made out to be - providing  knees have no pre-existing problems. One large study of more than 74,000 U.S. runners reported last year, for example, that running significantly reduced the risk of both osteoarthritis and knee replacement.

This is no surprise to Rob Newton, professor of exercise and sports science at Perth's Edith Cowan University. "A knee's worst enemy isn't going for a run – it's the couch and the TV remote. One of the strongest risk factors for osteoarthritis in the knee is being overweight. If only we had more people running, there would be less knee osteoarthritis," he says. "Running may also be protective because it helps to keep the joint mobile and reduces systemic inflammation as well as helping to keep weight off."

TECHNOLOGY AND FOOTWEAR
Hi-Tech Ballet Shoes Trace Dancer's Movements

Trying to keep your eye on a dancer's ballet shoe is not unlike attempting to follow a hockey puck as it glides between packed bodies on ice. A ballerina kicks and twirls, pirouettes and arabesques, throwing his or her body into a flurry of motions guided by the precise movements of carefully trained feet. Designer and amateur dancer Lesia Trubat has a true appreciation for this frenzied action of dancers. Her past projects have sought to illuminate the movements of ballet, transforming the fleeting image of a physical dancer into a whole new visual language (think two-dimensional reliefs that turn the drag of a pointed toe into alphabet-like artworks). More recently, Trubat has taken this fascination with footwork into the digital realm, introducing "E-Traces."

E-Traces

"E-Traces" starts with a small electronic device affixed to the bottom and sides of a dancer's shoe, developed by master of the "sewable electronics," Lilypad Arduino. This device records the foot's contact with the floor, allowing the dancer to effectively "draw" his or her movements in a constellation of data strokes sent to a custom mobile app program. The result is a dazzling mapping of choreography, demonstrated in this video.

MEDICAL RESEARCH NEWS
Adding Lesinurad May Lower Uric Acid

Co-administration of lesinurad, an investigational selective uric acid re-absorption inhibitor, and febuxostat was superior to either agent alone in lowering serum uric acid (sUA) in patients with gout and hyperuricemia, according to results from a phase Ib study.

"Once daily administration of lesinurad 400 and 600 mg in combination with either 40 or 80 mg/day doses of febuxostat achieved sUA-lowering effects that were significantly more pronounced than either dose of single-agent febuxostat in gout patients with high baseline sUA levels (58 mg/dL)," according to Roy Fleischmann, MD, of Metroplex Clinical Research Center in Dallas, and colleagues. The finding is consistent with the superiority of other combinations (i.e., allopurinol and benzbromarone) in lowering sUA levels compared with monotherapy, they wrote.

PRACTICE MANAGEMENT TIP OF OF THE DAY
Diffusing Angry Patients: It's as Simple as ABCD- PART 2

Unsurprisingly, people are more stressed than ever before. According to a recent poll by NPR, the Robert Wood Johnson Foundation, and the Harvard School of Public Health, nearly half of 2,500 adults surveyed admitted to a major stressful event within the last year. More than a quarter tied their specific stressor to a health-related problem.

C. Curtail Confrontation - Confrontation may seem inevitable in these instances, but ideally, it should be avoided at all costs. Although it goes against human instinct, physicians will have greater success at diffusing a scenario if they remain calm and monitor their language, while avoiding raised voices or body language like crossed arms.

D. Don't Forget to Document - As in every other patient-physician exchange, documentation is key. Physicians will want to include the details of angry and violent outbursts in the patient's chart. This not only serves to forewarn other healthcare professionals who encounter the patient, but also may identify an ongoing decrease in mental health status that warrants further intervention.

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Werner Strupp, JD

It is with sadness and deep respect that I inform PM News readers that Werner Strupp, JD passed away on December 7, 2014. He served as General Counsel for the APMA for in excess of 50 years.

For those who were members of the APMA's House of Delegates, who served on the Council on Podiatric Medical Education, on the APMA Board, and as Officers of APMA, we shall always remember his steady hand as he guided us through the legal issues involving healthcare and education. One of Werner's phrases of deep respect was to indicate that the individual was a very decent person. Having known him for some 55 years, I can clearly report that he was one of the most decent people I ever knew.

His words and deeds for our profession will long endure. Rest in peace my friend. Robert Pine said it well when he noted, "What we have done for ourselves is soon forgotten but what we have done for others remains and is immortal." Kol Ha Kavod.
