New Study Shows Foot Problems Can Cause Weight Gain: MD Podiatrist

"When feet hurt, people don't exercise. When people don't exercise, they gain weight, especially this time of year," says Baltimore podiatrist Dr. Mikel Daniels. "A study done by the American Podiatric Medical Association showed that 72 percent of people don't exercise because their feet hurt."

There are two ways to deal with foot pain, says Dr. Daniels: "You can identify what's causing the foot problem and have it treated, or you can change to a less impact exercise such as swimming, bicycling, or using upper body weights."

Dr. Bijan Najafi, Associate Professor of Applied Biomechanics at the Dr. William M. Scholl College of Podiatric Medicine’s CLEAR at Rosalind Franklin University of Medicine and Science, recently joined the editorial board of Gerontology.

CA Podiatrist Joins Saint Louise Regional Hospital Staff

Saint Louise Regional Hospital announced that new podiatrist Dr. Sandeep Bains has joined the medical team in Gilroy. Dr. Bains offers both comprehensive inpatient and outpatient assessment of the entire spectrum of foot and ankle care.

NY Podiatrist Captures Photo of Snowy Owl with His Camera

Photography and fishing have been central to Dr. Gary Sherman's life. Sometimes, he has to make a decision about which to grab – the camera or the pole. Fishing off of Hoffman Island one day this October, the decision was an easy one.

He was in his boat trying to hook some striped bass when his attention was drawn to the shore of the Island, a few miles off of South Beach. He put down his fishing pole, grabbed his Nikon camera with a 18 x 200 zoom lens, and started shooting. As the boat moved closer, the owl took off. His expertise shows in the dramatic, clear images of the white bird and the intricacies of its barring on a brilliant sunny day. "It was huge and so beautiful," said Dr. Sherman who is chief director of podiatry at Richmond University Medical Center.

Jury Finds TX Hospital and Podiatrist Not Negligent

A Gregg County jury found Good Shepherd Medical Center and a Longview podiatrist innocent of negligence charges in a 2009 civil suit. Jurors deliberated more than four hours before reaching their decision, finding that neither the hospital nor Dr. Mark Witt were negligent in the care of Kathryn Pepper, a county employee who accused Witt of malpractice and Good Shepherd of not providing supervision when Witt performed a surgical procedure on Pepper’s tibia in 2007.

Pepper accused Witt and Good Shepherd of negligence or malpractice for failing to provide reasonable care following a surgery to repair Pepper’s ankle and remove a lesion from her ankle. It was Witt’s first time to perform the surgery in private practice, and he used a bone saw during the procedure. “Dr. Witt was clearly trained and competent to perform this surgery,” Dallas attorney Kay Ellington said.

Query: Traumatic Injury

A 56 year old male patient started treatment 8 months after kicking a large, heavy stereo speaker, barefoot. The patient feels pain in the second and third MPJs. A fracture was ruled out. The foot was treated for a suspected neuroma of the second interspace, and for capsulitis. Two injections of cortisone provided minimal help. He was provided with a post-op shoe and a CAM-walker.

An MRI in October showed 1) Degeneration of the plantar plate, 2nd, 3rd and 4th phalanx, with inflammation, but no flexion deformity 2) Degenerative arthrosis with loss of chondral surface and subchondral edema at 2nd/3rd MPJ level 3) Intermetatarsal bursitis between the 2nd/3rd toes. The patient is now 15 months post-injury. He can be on his feet for a few hours daily, but the condition can be very painful.  Seven months into treatment, I am wondering if surgical intervention of the plantar plate or complete immobilization with casting should be considered.

RE: Life-Long Maintenance Regimen for Onychomycosis (Jay Kerner, DPM, Evan Meltzer, DPM)

There has been discussion about life-long maintenance after successful clearing of nails with use of Lamisil. Actually, life-long maintenance is critical after any method of cure. It is true that foci of infection on the skin and between the toes must be aggressively controlled. I do that with daily use of ciclopirox cream .77% after shower. I do the same in diabetics with tinea pedis whether they have onychomycosis or not. The use of Lysol spray in shoes may cause a contact dermatitis, and it is very difficult to know how much to apply. Since most podiatrists agree that shoe disinfection is important, I highly recommend use of the Steri-shoe by Shoe Care Innovations. It employs a cidal form of UV light incorporated into a shoe tree and disinfects in 45 minutes. It is reliable and recent studies soon to be published confirm its efficacy.

I highly recommend that you purchase the book written by Neil Baum, MD Marketing Your Clinical Practice (Amazom.com). This text is considered by many as the marketing bible for any type of practice and has many easy-to-apply ideas to build any practice. Also, be sure to always have business cards with you, and wherever you are, strike up a conversation with someone and give them a card. I saw other posts about how the American Academy of Podiatric Practice Management is of great value to all practices. The best thing you can do to get a great kick-start is to attend our biggest and best meeting in Pittsburgh the first week in March that sells out each year. (aappm.org).

I am in total agreement with the sentiment expressed by Dr. Roth regarding an eight year old filing for disability for flat feet. Although I am not an attorney, the doctor has every right to contact Federal and state authorities regarding suspected fraud and this should be reported as a suspected case of child abuse on the potential grounds of "Munchausen by proxy." However, disclosure of such medical information to a "newspaper" may be viewed as a professional violation of unauthorized disclosure of medical information of the patient in question. Such action, may create a larger and unintended problem for the reporting practitioner. My advice in regard to contacting the media is to "tread carefully" on that piece of advice!

RE: MN Podiatrist Supports Statewide Single-Payer Health Insurance System

I just received my family health insurance renewal premium bill which increased 10% to $1800+/month with no ceiling in sight. I also think a single payor, not-for-profit, (i.e. Medicare) should be available to those still working as well as those retired. It makes sense to me that healthy younger families who don't utilize healthcare as much can subsidize the system with their higher monthly premiums. I realize the present for-profit system is too well entrenched to change.

In his recent post, Dr. DeBrule discusses the benefits of a single payer system and begins by claiming that healthcare is a basic human right. If healthcare is a human right, then surely food and shelter would also be a human right. Why not transportation? Why wouldn't everyone be entitled to a car, insurance for the car. and gasoline?  Certainly in a high stress society everyone should be entitled to an annual vacation. Why should you as a doctor, be entitled to live in a better house than me.

As I see it, Dr. DeBrule's basic premise is flawed. Adding other incorrect things to the mix like a truly unsubstantiated number of 50 million uninsured, the lowly position of U.S. healthcare on the global stage, and the number of people who die each year from lack of healthcare only further complicates a disturbingly bad argument. I would be glad to discuss the overall concept of the government running anything efficiently, e.g., post office, Medicare, Amtrak, etc., should  you be interested. In the meantime, I suggest that Dr. DeBrule place a sign on his office door that states that he will provide free podiatric care for all, or he should move to a country that already provides wonderful free healthcare to its people, i.e. Cuba.  Oh..when you go.. how about leaving me your car and credit cards?
