PODIATRISTS IN THE NEWS
Getting Healthier in 2024 Starts with Your Feet: CA Podiatrist

Good foot and ankle health is critical for good overall health, so no matter what your wellness goals are for 2024, be sure to start from the ground up. “Feet and ankle issues can be signs of overarching health problems like diabetes, arthritis, and cardiovascular disease, so paying attention to them is vital for early diagnosis and treatment says Michael Coyer, DPM, a foot and ankle surgeon and Fellow Member of the American College of Foot and Ankle Surgeons.

Dr. Michael Coyer

“Additionally, many common New Year’s resolutions related to fitness and weight loss are difficult to tackle if you’re experiencing foot and heel pain, making it important to fix these issues first,” says Dr. Coyer.

PODIATRIC PRODUCTS IN THE NEWS
OSSIOfiber® Bio-integrative Fixation Technology

OSSIO, Inc., a fast-growing orthopedic fixation technology company has announced that the FDA has cleared the company’s OSSIOfiber® bio-integrative fixation technology for use in orthopedic surgery for children and adolescents needing bone fractures fixed, osteotomies, or fusions. As a result, OSSIOfiber Compression Screws and Trimmable Fixation Nails may now be used in children from age two to 21 years in standard clinical practice.

Dr. Mark Solomon

Mark E. Solomon, DPM, Director of the Pediatric Foot & Ankle Fellowship at The Pediatric Orthopedic Center, provided his perspective on OSSIO’s innovative fixation technology, “The emergence of OSSIOfiber implants a few years ago marks an inflection point in the evolution of orthopedic surgery that my colleagues and I in the United States are keen to incorporate into our practice of what is already a highly evolved surgical subspecialty."

PRACTICE MANAGEMENT TIP OF THE DAY
8 Steps to Follow When Firing Practice Employees

Here are 8 things you must do to fire a practice employee.

5.  Know the law
  • The number of employees you have affects which laws affect you. If you have fewer than 15, you are exempt from some federal regulations, and are considered an at-will employer, meaning you can fire anyone for any reason.
  • Once you hit 15 employees, discrimination, harassment, and disabilities become a bigger issue, and once the employee count hits 50, the Family Medical Leave Act and Americans with Disabilities Act kick in.

RESPONSES/COMMENTS (CLINICAL)
From: Stephen Leonard, DPM, Chris Seuferling, DPM

I would be suspicious of a tarsal coalition. Order a CT scan of bilateral feet and ankles to rule it out.

Stephen Leonard, DPM, Spring Hill, FL

Is there any history of trauma, of any type, related to onset of symptoms? Any significant findings with family history? I would be interested to know how he responds to gabapentin. Depending on the response to the two above inquiries, I would consider a second opinion with another neurologist.

RESPONSES/COMMENTS (CODING ISSUES)
RE: New Bunionectomy Language for 2024 (Michael G. Warshaw, DPM)
From: Charles Morelli, DPM

In a recent publication of coding pearls published by Dr. Warshaw in TLD newsletter, he states that effective January 1, 2024, there will be changes to the definitions/descriptions of the CPT codes for Correction of Hallux Valgus deformities, and he goes on to list those changes. My question is about the CPT code 28297 (Lapidus-type of procedure), which states the following: "28297 Correction, hallux valgus with bunionectomy, with sesamoidectomy; when performed, with first metatarsal and medial cuneiform arthrodesis, any method."

This is pretty standard language, but at the end of his article he goes on to say the following, "The bottom line is that in order to bill any of the above CPT codes and support their performance with the proper documentation, a “bunionectomy” needs to be performed in addition to any other corrections/procedures that are stated within the definition of the CPT code. A bunionectomy is the removal or the excision of the medial aspect of the head of the 1st metatarsal. If this is not performed, then it would be inappropriate to bill any of the CPT codes listed and defined above."

It is not unusual when performing a Lapidus-type of procedure to not always have to address the first metatarsal head, either with an osteotomy or with a simpler excision of the medial eminence. Are we now being told that we are being forced to do work on the first metatarsal head when doing a Lapidus-type of procedure, even though it may not be necessary in order to properly bill CPT code 28297?

RESPONSES/COMMENTS PM ARTICLES
From: Bret Ribotsky, DPM

After reading Paul Kesselman‘s incredible retrospective review of podiatric medicine over the last four decades, I’d like to nominate him for the podiatric equivalent of a Pulitzer Prize. I’m very proud of Paul. Everyone should read this article whether you’re young or old. It will strike a note within each of us.
