INTERNATIONAL PODIATRISTS AND DIABETES
Diabetes-Related Amputations Preventable with Regular Foot Exams: Aussie Podiatrist

Every week in Australia about 70 people undergo a diabetes-related amputation. "The cost is around $25,000 for an amputation, then $8000 every year after that to the health system," Dr. Peta Tehan, a podiatrist and researcher based at the University of Newcastle, said.

Dr. Peta Tehan

"But the biggest problem is that after someone undergoes a major amputation, their risk of death is around 50 per cent within five years. They don't survive it a lot of the time because they tend to be quite sick and have a lot of co-morbidities." Dr. Tehan said it costs more than $600 million a year to care for people with diabetic foot ulceration - a largely preventable problem with the appropriate care and support.

PODIATRISTS IN THE COMMUNITY
FL Podiatrist Plans to Retire Next Year

Podiatrist Turner O Houston, DPM, Jr. has long had a unique approach to his practice. He gave up his Jacksonville Beach office in the late 1970s. His new goal was to become a simple, old-fashioned doctor who only made house calls. Most of his current patients are elderly and many live in retirement communities or nursing homes. Turner’s gentle bedside manner is just what they need. He simply talks and listens as he works carefully on their feet.

Dr. Turner has been in continual practice for about 47 years and plans to retire next March when his license expires. He is 74. He now hates driving anyway due to the traffic and crazy drivers he encounters on his travels to see patients in Mandarin and Orange Park. His many patients will miss the relaxed casual visits, where his full attention is on them and their needs.

PODIATRIC PRODUCTS AND SERVICES IN THE NEWS
APMA Grants Seal of Approval for JUBLIA® Topical Solution, 10%

Bausch Health Companies Inc. ("Bausch Health") and its dermatology business, Ortho Dermatologics, one of the largest prescription dermatology healthcare businesses, has announced JUBLIA® (efinaconazole) Topical Solution, 10%, a treatment for onychomycosis, a fungal infection of the toenails, has received the American Podiatric Medical Association (APMA) Seal of Approval. The APMA Seal of Approval is granted to products that promote good foot health and are of significant value when used in a consistently applied program of daily foot care and regular professional treatment.

Dr. Jeffrey DeSantis

"Toenail fungus is extremely common, but it is an infection—and a highly contagious infection," said Jeffrey R. DeSantis, DPM, president of the American Podiatric Medical Association. "APMA is proud to bestow its Seal of Approval on products such as JUBLIA® that meet our physicians' highest standards for safety and effectiveness."

PRACTICE MANAGEMENT TIP OF THE DAY
2021 Year-end Financial Planning Checklist

The end of the year is a good time to check in on your financial plan to see if everything is on track or if some things need adjusting. You will be in a better position for 2022 the sooner these items are addressed and adjusted if they are off-track.

7. Harvest Tax Losses

Tax loss harvesting can be an effective tool to reduce taxes. If any investments in your taxable accounts have lost money, those investments can be sold, and the loss captured. This loss can then be used to offset gains in other investments or potentially lower your 2021 tax bill. Up to $3,000 ($1,500 if you are married filing separately) of net capital losses can be deducted against ordinary income, self-employment income, and interest income. If you have captured losses above these amounts, the excess can be carried over to future years to offset gains or deduct against income. Tax loss harvesting must occur before 12/31/2021 to count towards 2021.

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Robert Fleischner, DPM

Robert Fleischner, 92, died peacefully in the company of his beloved wife of over 57 years, Dorothy, on November 18, 2021. During the Korean War, Robert was awarded two Purple Hearts and a Silver Star for his valor on the field. After the war, Robert received a battlefield promotion to captain.

Dr. Robert Fleischner

Following discharge, Robert attended Temple University and graduated in 1958 as a Doctor of Podiatric Medicine. Robert began his practice in Springfield shortly thereafter, building a lifelong bond with many of his patients. Robert was an avid competitive golfer who, over the course of his career, won several individual and team championships at his lifelong club Twin Hills, and at other courses.

RESPONSES/COMMENTS (RELEVANT RESEARCH)
RE: Alcohol Consumption and Diabetic Peripheral Neuropathy
From: Richard Mann, DPM

A recently published paper in the journal Diabetes Therapy, examines the impact of alcohol use in diabetic patients with distal symmetrical polyneuropathy (DSP). In the paper, “Alcohol Consumption as a Causator and/or an Accelerator of Neuropathy in People with Diabetes is Regularly Overlooked,” the authors note that the most common cause of DSP next to diabetes is excessive alcohol intake, which is almost never evaluated, and that peripheral nerve damage from these two distinct pathologies is likely to be additive.

Authored by two board certified endocrinologists, the paper makes the point that alcohol use is often underreported by patients. They recommend that in addition to taking a detailed medical history, clinicians should investigate the possibility of...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: No Sane Rationale for Prohibiting DPMs to Vaccinate
From: Name Withheld (TX)

I do not understand the world we live in. In Texas (and perhaps other states), DPMs cannot give flu shots or COVID-19 vaccines. Is this insane? RNs, LVNs, NPs, and mid-levels can give these injections but not podiatric physicians. DPMs are trained in administering injections and dealing with adverse reactions much better than the local pharmacist. The reason given for not allowing DPMs to give these vaccines? Flu and Covid are systemic illnesses. Crazy!

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: In-Person Conferences and the Omicron Variant
From: Jeffrey Kass, DPM,

Medical necessity are words podiatrists grapple with on a regular basis. Recently, three New York podiatrists (Drs. Chaskin, Jaffe, Udell) discussed whether NYSPMA should livestream their 2022 Clinical conference in order to reasonably accommodate those immunocompromised or at high risk the opportunity to participate. Dr. Jaffe informed us that he would not attend. With the recent outbreak of the omicron variant, and New York State under a state of emergency, is an in-person conference medically necessary and prudent? Why take unnecessary risks of potentially causing colleagues to possibly get ill or transmit the virus to others?

This past weekend, I participated in Desert Foot’s online conference. Anyone who attended can verify that one can learn from an online conference as much as from an in-person conference.
