MA Podiatrist Still Makes House Calls
Lynn podiatrist Dr. Mark Sanphy has been practicing medicine on the North Shore for 25 years, but he also is truly a welcome throwback to an older era. Sanphy, who heads the four-physician practice of the Sanphy Podiatry Group, still makes house calls. “We all make house calls a priority here,” Sanphy said. “It’s important to be able to service patients who struggle to leave home and are in need of a doctor’s care. Housecalls can prevent many patients from being hospitalized.

To Avoid Foot Pain, Opt for Wider Heels: UK Podiatrist
Wearing heels for extended periods of time can leave the wearer with blisters, aching legs, and pinched toes. The shoes can also cause long-term problems for wearers, including bunions, back problems, and can even increase the risk of sprains. However, one podiatrist believes she has the formula to avoid foot pain when purchasing heels. Laura Gleich says that in order to avoid walking home barefoot, we first need to pay careful attention to the shoes we buy.

Laura revealed to the lifestyle site Mamamia that those looking for comfort should go in search of a wider heel - this means ditching a particularly popular style. She says: "Stilettos with a very small base are the most dangerous kind of heels - they certainly won't feel so great when you roll your ankle walking down the street." She advises investing in some wedged styles rather than a pointed heel for more comfort.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
The Basics of Incident-to Billing - Part 7
Employing non-physician practitioners (physician assistants, nurse practitioners, clinical nurse specialists, etc.) is an effective way to increase productivity in a physician office. However, under Medicare rules, covered services provided by non-physician practitioners (NPPs) are reimbursed at a reduced rate (85 percent of the fee schedule amount). The “incident-to” billing rules provide an exception, allowing 100 percent reimbursement for non-physician services that meet the requirements detailed in the Medicare Benefit Policy Manual, Chapter 15, Section 60 (Services and Supplies Furnished Incident To a Physician’s/NPP’s Professional Service).
Remember: Incident-to applies only to Medicare. Further, the requirements do not apply to services with their own benefit category. Diagnostic tests, for example, are subject to their own coverage requirements. “Depending on the particular tests,” the Benefit Policy Manual explains, “the supervision requirement for diagnostic tests or other services may be more or less stringent than supervision requirements for services and supplies furnished incident to physicians’ or other practitioners’ services.”

Angelo "Ange" Monaco, DPM of Broomall, PA passed away on December 4, 2016 surrounded by his family. Dr. Angelo Monaco graduated from the Temple University School of Chiropody in 1952. He was one of the original founders of the Pennsylvania College of Podiatric Medicine (PCPM) and served as a Trustee of PCPM from 1968 until 1998, when the School was re-acquired by Temple University.

The non-healing site is due to a foreign body response to skin pushed subdermal from the original excision. A deep curettage without suturing will remove the inclusion cyst that has formed, causing this clinical picture. Simple topical care will close this in another few weeks.

I would do a wound culture of the skin. Non-invasive arterial Dopplers could provide pertinent information as well. Blood work, including a total lymphocyte count and albumin levels, will also demonstrate your healing potential. Has the biopsy site been properly off-loaded to decrease pressure over the course of the 6 weeks?

I attended the Global Foot and Ankle Conference in Cleveland this year. As advertised, it included foot specialists from around the world. The lectures were timely and well presented by podiatrists, orthopedists, and others involved in foot care. Insights from specialists from around the world showed different perspectives on treatments. I highly recommend the conference.

This is a legitimate meeting unlike some predatory conferences and spam emailings that we all receive from time to time. Current and past presidents and leaders from foot and ankle societies throughout the USA, UK, Europe, and the Asia-Pacific rim, founded this organization in 2015. The inaugural event was held in Cleveland, OH this past June. As a global organization, GFAC will rotate the annual meeting to a different host country each year. Much like the Olympics, each host country will bring its own flare and cultural contributions to the annual congress. 2017 will be in Chongqing, China; 2018 will be in Milan, Italy; 2019 will be in Delhi, India. Faculty and council members are comprised of ACFAS, AOFAS, EFAS, APSFAS in addition to many other national/international organizations. GFAC is an organization designed to be inclusive (MD, DPM, DO, PhD, PT, RN, etc.), not exclusive. Thus, the organization’s name: Global Foot and Ankle Community.

I'm in the midst of opening a practice and have found a number of ways that technology can make for a nimble startup. With recent advances in cloud computing, an entirely cloud-based office is possible. Gone are the days of expensive server installations with costly monthly IT service contracts. With a modern EHR like Medi-Touch, much of the practice is handled via web browser. Patient registration and check-in can be handled entirely through the EHR, saving time and administrative costs over a traditional paper system. In addition, charting, surgical consent signing, and patient instruction on everything from stretching to wound care can all be done electronically.
Medi-Touch is what I've elected to go with, but there are a number of others out there like DrChrono and Practice Fusion that can be considered. The decision is entirely personal, but can be overwhelming. The same goes for digital x-rays. I recommend a good consultant to help you decipher between all of the options out there and aid you in negotiating competitive pricing. Again, the choice is entirely personal, but I've retained Cindy Pezza with Pinnacle Practice Achievement and have been very happy with the service I've received so far.

Check out Cardfellow.com. They are an ISO (like a broker), but they do not take commissions from the merchant. They have pretty strict standards as to their agreements. For example, where most credit card processors structure tier pricing (so you wind up paying much more than their advertised rate), all Cardfellow processors must offer Interchange Plus pricing, so there's no hidden tier "upcharging". Cardfellow processors basically compete for your business, and so they offer better rates and terms.
That said, I always review any processor's agreement before committing. Sometimes they'll have a cancellation fee in the agreement but not mentioned elsewhere, which should be negotiated out. Some processors want a reserve bank account you must fund, but they control, to cover their potential losses. I consider that a deal killer.
Always review the fees the processor lists, like PCI compliance, or marketing add-ons that are actually optional, but may not look like it. They should offer an easy, very inexpensive PCI compliance done-for-you, or a free DIY PCI compliance site. Cardfellow has a fair bit of non-biased, fairly and simply explained educational material on their site, so you can ultimately make a very educated decision without having your head blow up with credit card complexity.

RE: Medical Marijuana and Podiatry

As per my previous posts regarding prescribing medical marijuana for such conditions as diabetic neuropathy, ishemic pain, and debilitating arthritis, I again wish to raise my concern that as podiatrists, we de-value our profession by not retaining parity with the other medical professions.
As stated in Newsday last weekend, as of November 29th, nurse practitioners may certify patients for medical marijuana in New York State. Physician assistants will no doubt eventually be included, as they are already licensed to prescribe...

It is nice to hear from another perspective regarding indications and application of foot surgery techniques. I simply ask Dr. Lee to let us see the volume after 30 years of doing this, clinical pictures, and the x-rays of these 18-20 degree IM angle bunions that he has done using this technique at 5 years post-op. In my opinion, this technique in a person with a high IM angle is a “quick fix” and in my experience, it is doomed to long-term failure.
I suppose that if one explained to the patient that the need for future surgery is very likely, it would be more palatable or if your goal was short-term (i.e.-with an older minimally ambulatory patient), I could understand the rational of using it. On a younger patient with a significant structural deformity, I cannot rationalize its use. I will say that I do not have 30 years of experience using this technique and am willing to learn something new from someone with more experience.
