Scholl College Faculty Present to AADE
 
Three faculty members from the Dr. William M. Scholl College of Podiatric Medicine provided a live webinar to approximately 250 members of the American Association of Diabetes Educators on Dec. 3. The title of the 90 minute presentation was “The Diabetic Foot Ulcer: Prevention, Treatment and Patient Impact.” Professor Stephanie Wu, DPM, MS; Associate Professor Adam Fleischer, DPM, MPH; and Assistant Professor Ryan Crews, MS, CCRP provided a review of the public health and economic impact of the ulcers, the prevention and treatment of ulcers, quality of life implications, and patient education concepts.

Crews stated, “Tackling the monumental challenge of diabetic foot ulcers requires conscious and deliberate actions on the part of patients. Having such a tremendous patient advocate as the American Association of Diabetes Educators engaging patients about their often neglected feet is sure to greatly benefit patients.” The full 13,000+ members of the association will have the option to view a recording of the webinar at a later date.

Aussie Podiatrist Discusses Rehabilitation After Achilles Tendon Rupture 
 
Football player Darius Boyd recently ruptured his Achilles tendon. Podiatrist Ashley Mahoney is an expert in the rehabilitation of lower leg injuries. A published author and now the coach of Olympic champion Sally Pearson, Mahoney is confident the fullback can return to his best before next year’s finals. To do so, Boyd must be dedicated to a treatment plan which will begin with surgery, then time spent in a compression boot to reduce post-operative swelling.

Daily physiotherapy will encourage healing, reduce strength loss, and promote ankle and calf mobility. “That’s a lengthy rehab stint,” Mahoney said. “As strength improves, then there is walking again and then building him up to running before he gets to stepping and cutting maneuvers, and then finally sprinting and jumping again," said Maloney.

Women, Blacks at Greater Risk for Bunions: Study
 
A large cohort study has confirmed several risk factors associated with hallux valgus: female sex, African-American race, older age, pes planus and knee/hip osteoarthritis (OA). The study, published online in Arthritis Care and Research found the highest prevalence of hallux valgus in African-American women: 70%.
 
"Early prevention and intervention approaches may be needed in high-risk groups," wrote researchers from the University of North Carolina in Chapel Hill and Harvard Medical School in the first study to report hallux valgus by sex and race subgroups. They noted that hallux valgus is a major driver of foot and ankle surgery and is associated with functional disability, foot pain, poor balance, and a high risk of falling in older adults. "Identifying adults with hallux valgus earlier in the life course may aid in preventing later functional limitations, such as impaired balance, in older populations," they said.

Physicians, IT leaders Mull ICD-10 Compromise but Brace for Fight
 
Many, but not all, physicians oppose the federal push to implement ICD-10. One doc who wants an ICD-10 deflection rather than a full stop is Dr. Joseph Schneider, chief medical information officer and medical director of clinical information at Baylor Health Care System, Dallas. “We've got to find some middle ground on this,” he pleaded.  But Schneider's moderate stance on ICD-10 puts him at odds with his own state medical society, which recently turned up the gas on an already simmering ICD-10 cauldron. The association—of which Schneider is a member—posted a call to action on its website, urging its 48,000 members to join with physicians nationwide in writing to Congress, asking it to delay ICD-10 implementation for two years.
 
Meanwhile, the Association of Medical Directors of Information Systems, a professional association of physician informaticists, recently posted Schneider's compromise proposal on the AMDIS listserv, where it garnered both support and criticism. The plan calls for going forward with the current Oct. 1, 2015 compliance date, but making ICD-10 use optional for hospitals and physicians for the next three years. Health plans and other claims handlers, however, would be required to process claims in either the currently used ICD-9 code sets, if that's how they're received, or in ICD-10. A multidisciplinary panel—he likened it to the 1990s' military base-closing commission—would lead a national discussion on clinical coding and come up with a transition plan.

Diffusing Angry Patients: It's as Simple as ABCD- PART 1
 
Unsurprisingly, people are more stressed than ever before. According to a recent poll by NPR, the Robert Wood Johnson Foundation, and the Harvard School of Public Health, nearly half of 2,500 adults surveyed admitted to a major stressful event within the last year. More than a quarter tied their specific stressor to a health-related problem.  
 
A) Acknowledge the Anger- Aaron Braun, medical director of PhysiciansER Mission Bend, a free-standing emergency room located in the Houston area, tries to find the path of least resistance. "If the patient is angry, is there some quick fix? If the answer is simple," said Braun, "I would capitulate and get them what they would like within reasonable bounds." 
 
B) Be Benevolent - Marc Leavey, board-certified in internal medicine and a primary-care specialist at Maryland-based Lutherville Personal Physicians, says he has seen finances become a catalyst for angry behavior too. Given the drastic changes to healthcare recently, and the misinformation that has inevitably followed, patients are rightfully concerned about being able to afford ongoing treatments. "It doesn't seem to matter if you say to the patient, 'Don't worry about [the cost] or we will deal with it [later],' they begin getting a little bit testy," said Leavey. "Being creative and trying to come up with a solution that meets the patient's needs while still maintaining good medical care [is ideal]," said Leavey. If a patient is most upset because they can't afford the medicine as prescribed, simply offering a generic medication — at a fraction of the cost — can resolve the entire dilemma.

Regarding Dr. Stempler’s post about the use of sterilized tubing for the cord of his Osada drill: If he’s going to be using this regularly, he might wish to consider purchasing an ethylene oxide gas sterilizer such as the one manufactured by Anprolene so that he can safely sterile the entire handpiece. If he does so, it will not be necessary for him to enclose the cord in other sterile tubing.
