NY Podiatrist Explains Why Feet Smell Worse in Winter
Summer is all sweat and exposed toes, so you'd think foot odor would be at its peak in that season. Not so. As the weather turns cooler, feet often turn smellier; at least, that's what anecdotal evidence suggests. Blame shoes and socks. Enclosing the foot increases heat and traps sweat, said Robert Kornfeld, a doctor of podiatric medicine who runs a holistic and restorative foot and ankle practice in New York.

"If the foot is allowed to [air out] efficiently, you don't build up the breeding ground for bacteria," Kornfeld told Live Science. Kornfeld said he advocates dietary changes, including getting rid of refined sugars, to strengthen the immune system and keep pathogenic bacteria at bay. He also suggested a roll-on anti-perspirant applied to the foot. People who are chronically stressed also tend to sweat more, he said, so foot odor is sometimes a sign of a wider systemic issue.

IL Podiatrists Return from Mission Trip to India
Lalita Turaga, DPM (PGY-2) and Martin Yorath, DPM of the Advocate Illinois Masonic Medical Center recently returned from a one-week medical mission trip to India (Nov 19- Nov 26). This was their third visit in three years. Dr. Turaga reached out to an orthopedic hospital for the poor and disabled in the city of Tirupati, Andhra Pradesh, India. The hospital director gladly embraced their interest and extended an invite. Yorath and Turaga delivered CME lectures for 2 days, including live surgeries.

Fifty patients were screened and twenty six surgeries were performed. Surgeries included correction of vertical talus, CTEV, multiple congenital anomalies, malunion ankle fractures, and a Haglund’s exostectomy. The CME live surgeries were very well perceived and Dr. Yorath gave lectures on multiple topics including Charcot reconstruction and total ankle arthroplasty. The hospital has expressed an interest to conduct these on a recurring basis, annually, and has invited Drs. Turaga and Yorath to come back in 2017.

The Basics of Incident-to Billing - Part 5
Employing non-physician practitioners (physician assistants, nurse practitioners, clinical nurse specialists, etc.) is an effective way to increase productivity in a physician office. However, under Medicare rules, covered services provided by non-physician practitioners (NPPs) are reimbursed at a reduced rate (85 percent of the fee schedule amount). The “incident-to” billing rules provide an exception, allowing 100 percent reimbursement for non-physician services that meet the requirements detailed in the Medicare Benefit Policy Manual, Chapter 15, Section 60 (Services and Supplies Furnished Incident To a Physician’s/NPP’s Professional Service).
There are six basic requirements to meet the incident-to guidelines for Medicare payment:
5. Both the credentialed physician and the qualified NPP providing the incident-to service must be employed by the group entity billing for the service (if the physician is a sole practitioner, the physician must employ the NPP).

I will assume that there is adequate circulation. Have any x-rays or a culture been taken? Try using a Darco or other wound shoe and try to sleep on your right side. In the worst case scenario, have a wide excision and have them perform a primary closure.

It's refreshing to finally see MIS get some positive press. It is a technique that I have used for 30 years with great success and numerous referrals from it. It most definitely has an important place in our profession. Many thanks to Dr. Peacock for writing the article.
I would like to add a few of my personal applications of the Akin and some observations I've seen over the years. There is obviously more than one way to skin a cat. I routinely use a Silver/Akin bunionectomy (all MIS) to correct bunions with IMs even in the 18-20 degree range. I know what many of you are thinking, but let me present an often encountered...

I had to laugh at the survey this week. Having left podiatry practice 9 years ago, I remained blissfully ignorant about what MACRA stands for. Only the U.S. healthcare system could come up with an acronym that contains, nested within it, another acronym (The C in MACRA stands for Child Health Improvement Program)!
I read what Wiki says about MACRA and found it to be the same kind of garbage that prompted me to leave practice, only much worse. I'm predicting more early retirements or career changes.

In addition, get rid of the last alphanumeric character in ICD-10! We should be leading this charge and getting other organizations to join us in this request.

As a former proud member of the NYS Podiatry Board prior to moving out-of-state, I recall going to Albany for an interview conducted by the Board Chairman and another Board member. I do not know if this is still a requirement, but if Dr. Karman was interviewed, I wonder how she managed to get past this hurdle. I strenuously oppose her appointment for all the reasons previously discussed.
