NY Podiatrist Receives 8th Annual Ronald L. Soave Award
 
On December 3, 2024 Jeffrey Falcone, DPM was presented the eighth Annual Ronald L. Soave, DPM Integrity and Excellence in Podiatry Award at the New York State Podiatric Medicine (NYSPMA) Downstate Meeting. The inscription on the award reads as follows: “In recognition for your time and dedication to the podiatric profession and for your outstanding leadership in the NYSPMA.” Dr. Falcone is a past president of Kings Division, and currently serves on the Board of Trustees of NYSPMA.

MA Podiatrists Promote Diabetic Foot Care Before State Legislators
 
David Alper, DPM, a member of the APMA Board of Trustees, and Anthony Tickner, DPM, President of the MA Foot and Ankle Society (MFAS) were two invited panel members representing podiatric physicians at an American Diabetes Association (ADA) event held at the MA State House on December 3.

The ADA event, titled “A Case for Amputation Prevention in Massachusetts”, is one of a series held for State governments across the country. Their mission is to educate state legislators about the dangers of diabetes that lead to amputation, and the role that early detection and intervention can have to save limbs, lives, and healthcare dollars. Suggestions were made to improve care, including covering annual foot exams and further access to the latest treatments and testing. Both podiatrists gave presentations regarding the value of podiatric care in a diabetic patient’s life.

TX Podiatrist Sentenced for $8.5M Compounding Fraud Scheme
 
A podiatrist and a patient recruiter were sentenced to 45 months and 60 months in prison, respectively, and ordered to pay over $7 million in restitution for their roles in a scheme to fraudulently bill TRICARE — the healthcare program for U.S. service members and their families — for compounded creams that were medically unnecessary and procured through kickbacks and bribes.
 
According to court documents and evidence presented at trial, Brian Carpenter, 58, of Paradise, Texas, was a podiatrist who signed prescriptions for compounded pain and scar creams for TRICARE beneficiaries to whom he never spoke and whom he never examined or treated.

RE: Treatment of Onychomycotic Onycholysis

The toenail depicted is something we see commonly. It may be caused by trauma to the matrix and/or  the underlying bone. Severe onychomycosis which damaged the root of the nail may be a culprit, or it may be an arthritic spur on the bone which is pushing up and not allowing the nail to grow out. Because the patient was living with this for a long time, conditions beneath the underlying nail present a perfect medium for all sorts of fungi and bacterial growth. 
 
Culturing that area is akin to culturing the bottom of a garbage can. You will see lots of things growing which may not be clinically relevant. Treatment options: Debride the non-attached nail back to where it is still attached or remove the entire nail and give the patient a slim hope that it will regrow normally. Removal of the matrix is also a possibility and then the "sick" nail will be history. X-rays of the toe should also be taken to rule out a dorsal exostosis, and if this exists, it may have to be addressed surgically. If you do, opt to remove the entire nail with or without a matrixectomy; the specimen should be sent to a  dermatopathology lab where they will examine the attached skin and make sure that you are not dealing with a neoplastic disease.

It seems that Dr. Tomczak may be a bit unaware of the differences between APMA and PM News. Having  said that, just last night at a meeting, I had the opportunity to review the clear differences between APMA and PM News with my dear friend, colleague, and mentor Dr. Barry Block. It is clear to me from our discussion that PM News serves a very vital role in allowing our profession to voice their opinions and concerns about various issues within the profession. However, while complementing each other, PM News and APMA serve separate and distinct roles in podiatry.
 
Does PM News file formal complaints against third-party payers? Does PM News meet and foster or finance meetings with third-party payers? Does CMS meet with Medicare officials? Does PM News meet with legislators.. .

RE: Orthotic Modification for Hallux Limitus

In the 1980s,  I treated a wide receiver from the Seattle Seahawks. A Morton’s extension was tried and failed. So I had an orthotic made with the apex of support at the first cuneiform-navicular joint. Back then, a first ray cut-out was not an option, so I modified the orthotic to allow plantarflexion of the first ray upon weight-bearing, supporting rays 2-5. This solved the football player’s problem. I utilized this technique on my subsequent patients over the many years in practice
