PODIATRIC ECONOMICS
Podiatry Listed as Second Best Healthcare Job in 2015

CareerCast, a nine-year-old job search website in Carlsbad, CA, just released a list of the 10 jobs it deems to be the best in healthcare. These jobs don’t pay in the mid-six figures commanded by, say, a heart surgeon, but the top jobs like pharmacist and podiatrist pay more than $100,000, and they don’t involve excessive stress or harsh working conditions, according to CareerCast. CareerCast doesn’t rank the 10 jobs it deems best, but we’ve listed them starting with the highest salary, putting pharmacist, with an average annual salary of $116,670 at the top.

The second-highest-paying job "is the only MD slot" on the list: podiatrist. Though working on people’s feet all day seems like it could be difficult and even onerous, Tony Lee of CareerCast insists that podiatrists are trained to perform surgery so they can mix up their routines. The field has also had some attrition, in part because the pay is not as high as that of specialties like radiology or neurosurgery. “There aren’t enough podiatrists to fill all the positions,” says Lee, yielding a projected growth rate of 23% by 2022.

PODIATRISTS IN THE COMMUNITY
CT Podiatrist Speaks for 33rd Year at High School Career Day

At the most recent West Milford High School Career Day session, Dr. Ira Schiowitz spoke to 37 students and invited those interested to stop by his office for further questions or information. Schiowitz became involved in the township, and for each of the last 33 years, he has been present and accounted for. “I think the most important thing is to let the kids know that I enjoy what I do and I’m able to help people on a daily basis,” he said.

Schiowitz advises people to give their feet a good once over, especially those who are diabetic. It’s time to see the doctor if you are experiencing, among other symptoms, continuing pain in the foot or ankle, have cuts, bruises, deformities, or injuries, notice changes in skin or nails, a sore that doesn’t heal, or you can’t bear weight on your foot. A visit to his website goes further into patient education.

PODIATRIC PRODUCTS IN THE NEWS
BME Launches Speed Arc Nitinol Bone Fixation Implant For Sloping Surfaces

BioMedical Enterprises (BME) announced the launch of the Speed Arc Nitinol Fixation System, the world's first super-elastic shape memory fixation device that conforms to the sloping surface of the metaphyseal anatomy. This addition to the current patented BME Speed Fixation System is available in three different sizes to ensure a proper fit in varying patient anatomies.

Dr. John Anderson

Dr. John Anderson, DPM, FACFAS, Podiatry Associates of Florida stated, "BME, a leader in Nitinol fixation in extremity surgery, is leading the way in developing implants that not only allow for continuous compression, but also contour to the slope or position of the bone."

HEALTHCARE NEWS
U.S. Healthcare Spending Growth Hit 53-year Low in 2013

U.S. healthcare spending apparently grew more slowly last year than at any time in the past half-century—including the Great Recession—as Medicare squeezed outlays, millions of Americans continued to go without health insurance, and those with health plans spent at a slower pace on hospitals, clinics, and pharmacies.

The nation spent $2.9 trillion on healthcare last year, an increase of 3.6% from the prior year and the weakest growth since 1960, after federal actuaries and economists revised recent estimates. That spending remained weak in 2013 was not surprising: U.S. health spending growth fell below 4% in 2009 with the recession that stripped private health insurance from millions of individuals. But newly revised numbers show an acceleration in 2012 to 4.1% before a slump last year.

QUERIES (MEDICAL-LEGAL)
Query: Does Sterilized Tubing Meet Standard of Care

Is the Osada surgical drill acceptable regarding standard of care? The cord cannot be autoclaved and must be covered with sterilized nylon tubing. I plan to use it for simple exostectomies only.

Mark Stempler, DPM, Staten Island, NY

Editor's Comment: PM News does not provide legal advice. Standard of care is an amorphous term. It generally refers to what is the generally accepted practice in a field. Not every  surgical instrument can be sterilized. It is common practice in the medical community to use sterilized tubing. The key is maintaining a sterile field. Fortunately, there many way to achieve and maintain a sterile field.

CODINGLINE CORNER
RE: CAM Walking Boot Denial

On June 13, 2014, I dispensed a pneumatic CAM walking boot to a patient diagnosed with Achilles tendinitis as well as a posterior calcaneal spur. I coded the device as L4360-RT-KX and coded ICD-9 726.73 (enthesopathy, calcaneal spur) and ICD-9 726.71 (Achilles tendinitis). The claim was denied by Medicare stating it did not meet medical necessity. No records were requested. While I am aware that there are new codes for walking boots, I do not think this has any bearing on this claim denial. Where am I lacking medical necessity?

Jack Reingold, DPM, Solana Beach, CA

Response: I cannot really see anything that you are missing. It would be important to take a look at the Medicare LCD to find out if they have any recent (last 6 months) changes that might apply to the use of the CAM walker. It appears - at least to me - that recently, many Medicare contractors have been subjectively, and without comment, changing the LCDs...sending us ridiculous statements about the lack of medical necessity. I would first take a look at your local carrier LCD, then appeal, resubmitting the claim.

RESPONSES/COMMENTS (CLINICAL)

I have seen this happen a few times with adults. It turned out to be something really simple to fix: the blanket or sheet was tucked in too tightly, so when they were sleeping on their backs, there was too much pressure on the toes. Untucking the sheet/ blanket fixed it.
