Podiatry Ranked 6th Most Likely Job to Damage Your Health
Some jobs intrinsically have more health risks than others. A nurse working in a hospital is far more likely to catch an infectious disease than a lawyer working in an office. To rank the most unhealthy jobs in America, Business Insider used data from the Occupational Information Network (O*NET), a U.S. Department of Labor database full of detailed information on occupations.
In order to analyze jobs by their impact on workers' health, Business Insider took measures of six health risks in each of the 974 occupations in the database: exposure to contaminants; exposure to disease and infection; exposure to hazardous conditions; exposure to radiation; risk of minor burns, cuts, bites, and stings; and time spent sitting, since studies show that frequent inactivity shortens your lifespan. These factors were scaled from 0 to 100, with a higher score indicating an increased health risk. For a complete list of job risks, click here.

DMU and TUSPM Students Team up to Collect Food for Hungry Children
Two AAPPM student chapters participated in the second annual Trick or Treat to De-Feet Childhood Hunger on Halloween. Students from Des Moines University and Temple University went door-to-door in their respective communities asking for canned food items that they could donate to hungry children. The DMU volunteers collected 277 food items, while Temple students collected 60.

The volunteers raved about how enjoyable the event was, especially because the people in the community were so receptive to their cause. According to Ricky Kaufman, a Temple podiatry student, “nearly every single person wanted to know how else they could help.” In addition to seeking food donations, the volunteers left flyers with information on our year-long De-Feet Childhood Hunger (a national initiative developed through the efforts of Drs. Les Appel and Hal Ornstein).

KY Podiatrist is Finalist to Win Super Bowl Tickets in NFL Contest
A Louisville cancer survivor who's also a proud Trinity football dad is getting national attention. Dr. Gary Degen took part in a National Football League contest and from thousands of entries, he's one of ten finalists. Degen saw the contest on NFL.com to win tickets to the 2014 Super Bowl. He held up his cell phone and shot a one-minute video about what football means to him. Soon, an NFL film crew was at Trinity High School telling his story that football is about much more than a game.

"People I don't know are praying for me and are asking me what's going on? What can we do?" he said, "Regardless of the story, the story is that this is a family based on faith, family and football; this is my family." That's why Gary Degen loves football. Women Taught to Bear Pain from High Heels: CA Podiatrist
When women wear heels, they change the way their feet work. The worst combination, according to Dr. John Ciccone, is a high stiletto with a pointy, closed toe. Then about 80 percent of the woman’s entire weight is shifted to just 20 percent of her foot, and her toes are squeezed at the front.

Ciccone has had his own San Diego practice, Pacific Beach Podiatry, for 35 years. He said he treats far more women than men, and much of the gender difference can be attributed to footwear. Men get to walk around in roomy, comfortable shoes while women are taught to bear the pain for the sake of fashion.

Query: First Ray Stability
This 26 year old healthy nurse presented with a painful bunion. She has no other complaint except bunion pressure on her left foot with related pain not relieved by conservative measures. She has no sub-second met pain, but a mild functional hallux limitus which is improved with orthoses. Her x-rays show unusual findings at the base of the first metatarsal and associated cuneiform, as well as the rounded met head with a slight dorsal ridge. My impression is that this due to the first ray instability.

If only a neck osteotomy is performed, she is at a greater risk of recurrence. It's an easy procedure now with higher risk of recurrence later vs. more difficult surgery now with potentially less chance of recurrence later on. Options: neck osteotomy with orthoses follow-up or a first ray stabilization with a distal McBride-like procedure. Constructive comments are welcomed.

Query: Patient Access to EMR Medical Records
The recent and upcoming Stage II meaningful use has a requirement of an EHR having the capability to allow a patient to view one's medical records online. If patients request a copy of their medical records, and they have that capability to view their medical records on-line with the EHR we are currently using, do they still have to sign a release to get or email copies, or can we just tell them to view their records on-line and copy whatever they want or need?

Query: Footwear and Deformities
One of the arguments that I have heard which discounts the effect that footgear may have on structural deformity of the foot goes like this: If shoes caused structural deformities of the feet, then if one evaluated the incidence of foot deformities in societies (usually third world nations) where shoes were not worn regularly, then the incidence of foot deformities should be less. This makes sense, but when I set out to actually find data to confirm or deny this, I could find no reasonable data. Does anyone have this data?

RE: The CARe Approach to Medical Mistakes

The CARe approach (Communication, Apology, and Resolution for Adverse Medical Events) is based on a model developed in Michigan, where the practitioners funded their own malpractice insurance and actually saved money. Their idea was to recognize when a mistake was made, acknowledge the mistake to the patient, and take action to correct it, most likely with the patient's involvement. Here is a link with more information on the Michigan model.
I heard about this only recently at a CME lecture at a nearby hospital (Beth Israel Deaconess Medical Center) where it is being implemented. Here is a link to an article about this.
I think that it would be common sense to acknowledge a mistake as soon as you realize it occurred, because if you ignore it, it will only get worse and could lead to a malpractice suit. Apparently, there aren't many places that agree with my line of thinking as this approach has not been adopted nation-wide as of yet.

Many of us have been led to believe that when the new scope of practice laws in New York State take effect, it will not just affect bone surgery of the ankle, but the treatment of soft tissue problem in the adjacent areas.
I, for one, have no intention, at this stage of my practice, to study for or to become certified to perform ankle surgery, however I don't see why the new law would require colleagues like me to complete a residency in ankle surgery in order to treat a verruca which happens to be above the lateral malleolus.

I have been critical of some of Dr. Bijak's remarks in the past, but this time, I want to applaud him for saying what needed to be said. We don't need to practice "homeopathychiropody". There are practitioners of other "healing arts", who cannot write scrips, so they instead "deal" in supplements, and come up with some crazy things. Food causing plantar fasciitis sounds like that kind of pseudo-science. It's bad enough that we do not get the respect our education and training deserve, but to start pushing "quack science", is not what our profession needs.

Dr Kornfield, despite what you think, New York state law forbids podiatrists from the treatment of inflammatory diseases by diet. You risk your license if caught. There is a big difference treating minor, local inflammation and treating inflammatory diseases. NY state law says you can treat local manifestations of systemic diseases only, not systemic diseases themselves. NYS law won't trust you to touch a wart on the ankle and you think you can treat and diagnose inflammatory bowel disease, etc.? You can treat the initial flare of gout and give a starter prescription. Then you have to refer the gout patient to a doctor of medicine. Your holistic and vitamin therapies are not the standard of care in NY. Real treatment and prescriptions need an MD/DO, not a DPM.
