FL Podiatrist Uses SteriShoe to Prevent Fungal Re-infection
Dr. Eric Weinstein clears the fungus using a laser treatment but some patients worry that the condition will come back. "Once we get rid of the fungus with the laser, then we have to do a little bit of maintenance and that means perhaps a topical on the nails to act as a protective barrier, to not get re-infected, but more importantly, we have to address the shoe which the toes sit in on a day-to-day basis," said Weinstein.

Weinstein suggests that patients buy a SteriShoe, which emits an ultraviolet light that kills fungus or bacteria that's in the shoes. "It also helps with foot odor because the bacteria in shoes are a contributing factor to foot odor," said Weinstein. Users only need to put the device into closed toe shoes and turn it on. The SteriShoe automatically shuts off after 45 minutes. "Ideally, it's recommended to use it every day after you use those shoes, so when you put the shoe on the next morning, you can have confidence you won't be getting re-infected by what's breeding in your shoe," said Weinstein.

CA Podiatrist Presents at Italian Orthopedic Meeting
Amol Saxena, DPM from Palo Alto CA, presented at the 2nd Annual Society for Muscles Ligaments & Tendons in Napoli, Italy Nov 30-Dec1. He was invited by Professor Nicola Maffulli, president of the society and meeting chair Prof. Francesco Oliva. Also presenting from the U.S. was orthopedist John Furia, MD.

Saxena, who also serves on The Journal of Muscles, Ligaments & Tendons editorial board, was the first podiatrist to present at the meeting attended by approximately 300 orthopedists and other musculoskeletal specialists, primarily from Italy. He presented on "return to sports after ligament and tendon surgery of the foot & ankle." Other topics included ESWT, PRP, and other soft tissue injuries of extremities.

Device-based Therapies for Onychomycosis Treatment Show Promise
Device-based therapies for onychomycosis show promise in initial clinical studies involving lasers, photodynamic therapy, iontophoresis, and ultrasound-based therapy. Device-based treatments may be advantageous because they are conducted in the clinic and only require short-term patient compliance. These modalities also have the potential to reduce adverse events caused by antifungal drugs, as they are highly localized treatments.
Devices may also be alternatives for patients whose susceptibility to onychomycosis infection arises from a co-morbidity, as these therapies do not interact with the drugs involved in the management of such conditions. In order to substantiate the efficacy of device-based therapies for onychomycosis, randomized controlled trials with mycological evaluation and long-term follow-up are required. We believe this therapeutic area will continue to expand and hope that broader clinical investigations will result in new options for practitioners.

Hiking Medicare Age Opposed by Left, Right
Proposals to increase the eligibility age for Medicare drew fire from across the political spectrum during a Washington meeting of some debt negotiation leaders. Sen. Max Baucus (D-MT), chairman of the powerful Finance Committee, said during the Fix the Debt Conference that he favored targeting the growth in healthcare costs instead of “shifting costs to seniors.”
Neera Tanden, president of the liberal Center for American Progress, agreed with Edmund Haislmaier, a senior research fellow at the conservative Heritage Foundation, that increasing the Medicare eligibility age would just shift the cost of their care to other systems. Tanden said employers would cover much of the shifted cost, while Haislmaier expected a cost shift to younger and healthier buyers in coming health insurance exchanges.

Query: Patient Access to X-Rays
Today, I had a patient come in with a piece of paper she signed in order to take her x-rays for a second opinion. The statement she had to sign read as follows: I agree to return all records that I have borrowed within 30 days of the date of pick up. If the records are not returned within the 30 days, I understand and agree to pay the $100 fee for not returning the documentation that was borrowed. Is this different from state to state?

"Copies: does NOT mean "originals", which should be ratained as part of your records. You are entitled to charge a reasonable fee for a copy of the x-rays.

RE: Botox for Plantar Fasciitis? (David Kahan, DPM)

Botox utilization for treatment of symptomatic plantar fasciitis is very well supported in the medical literature as isolated therapy, and in combination with platelet concentrate.
Examples include Sorensen, et al., JFAS 50 (2) 2011, Logan, et al., American J Physical Med and Rehab 85 (8) 2006, Placek, et al. Clin J Pain 22 (2) 2006, Babcock. et al., Am J.Phys Med and Rehab 84 (9) 2005, Zhang, et al. Inflammopharmacology 19 (1) 2011. There are many more citations supporting the use of Botox for plantar fasciitis. Each article sites a different regimen and dosage schedule.

RE: CMS to Now Cover Obesity Counseling (Robert Bijak, DPM)

Having been the recipient of a fortuitous heart stent placement in my left anterior descending coronary artery which was 95% blocked along with being a licensed physician as well as having a primary care physician for a father to consult, coupled with a close relationship with my cardiologist, I feel more qualified than Dr. Bjiak to speak on the subject of CAD and obesity. I practice in a rural area of far Northern California where obesity is rampant. Diet and exercise are both acceptable means to reduce morbidity and mortality with respect to CAD. Of course, one would get an electrocardiogram and perform a complete physical exam on anyone at risk for a cardiac event before suggesting to them to undertake an exercise program for weight reduction.

Dr. Creighton suggests that biomechanics “can’t be practically and effectively communicated to patients in the clinical setting.” I offer wellness biomechanics to fill the vacuum he describes. Podiatry sits as the foundation of the exercise pillar of the pyramid of care indicated to treat obesity (diet, exercise, medication).
I do a functional foot typing, profiling every obese patient’s pedal weaknesses and strengths into one of five common subgroups. I do a muscle engine evaluation and a stance and gait evaluation. I can formulate a custom, professional program of biomechanical care that includes a foot centering orthotic, thus overcoming some of the collapse inherent in most feet.
I then develop a program of muscle engine training that strengthens weak muscles and reduces compensatory overuse of others that enables the overweight patient to make positive custom lifestyle adjustments and train more successfully. Finally, I advocate, support, and monitor their progress.
