NJ Foot and Ankle Center Staff
 
New Jersey Foot and Ankle Center's employees participated in their 3rd Annual Holiday Gift Giving for Court Appointed Special Advocates (CASA) for Children of Bergen County. The staff members sponsored six children in the foster care system's holiday wish lists. This year the donation coalesced with #GivingTuesday, a global day dedicated to giving back on Tuesday, December 1, 2015. New Jersey Foot and Ankle Center is a podiatry practice located in Oradell, NJ led by Dr. Daniel Margolin.

Exercise Must Be Started Gradually: Aussie Podiatrist
 
A prominent sports podiatrist has warned beginners about some extreme yet popular exercise programs, such as bootcamp, crossfit and obstacle-race-style workouts, due to the crippling injuries they potentially cause on joints and muscles. With a "no-limits" mindset, such programs can put beginners at risk of overuse injuries, if they let the culture of "no pain, no gain" overshadow the basic principles of conducting a safe workout.

Sydney sports podiatrist Karl Lockett says he has seen an increasing number of patients with injuries caused by intense work-outs with a too-hard-too-soon approach on bodies that are at a beginner level of exercise. "Some have crippling pain from ailments such as advanced plantar fasciitis, to torn ligaments which take a long time to heal, to less serious soft-tissue pain that keeps them out of action for weeks," he said. Lockett's advice for those who still want to join an extreme exercise program is to find a trainer you can trust to build your abilities from a beginner's level - not just drive you hard from day one.

Podiatrist Dr. Jeffrey Adler, who has been working with the HyProCure treatment for about three years now, said it’s one of the most gratifying treatments he has come across in his 36 years practicing medicine. “We used to treat these problems using arches and orthotics … but what we found out is that these undertreat things. Because you have continual motion [in your foot], many people who wear orthotics over a long period of time end up with unnecessary arthritis in their ankle joint and in the subtalar joint,” said Dr. Adler, surgical director of Adler Footcare. He said HyProCure is for foot problems what LASIK surgery is for eye problems—a treatment that fixes the root cause.

According to Dr. Alex Tievsky, a surgeon at Adler Footcare, most foot problems are caused by too much movement in the subtalar joint, which causes the talus bone above it to dislocate. Around 30–40 percent of all people roll their ankles in (hyperpronate) when they walk because of this dislocation, and Dr. Tievsky estimates that over 90 percent of their bunion patients have this problem too. “So the majority of foot problems, combined, can be attributed to having this dislocation,” he said.

EmblemHealth's HIP To Drop Solo Practitioners
 
EmblemHealth will no longer contract with independent providers, opting instead to work with larger groups such as independent practice associations as it pursues a shift toward value-based payments. Members of EmblemHealth's HIP plans received a letter Oct. 29 stating that independent providers would no longer be covered. The insurer pointed to the move as a sign it is at the forefront of the shift to value-based payments.
 
That stance has been supported by state regulators with the Department of Financial Services (DFS) who see payment reform as a way to improve quality and lower health costs.

5. Create "wow" experiences for existing patients.
 
By creating moments of "wow" in your office, your current patients are more likely to talk to and tell their friends about you and share their experiences on social media (the new word of mouth), said consultant John Lynn. Be creative and focus on the patient. A nice office is fine, but something that is personal to the patient can create a much bigger "wow."

Query: Rx + E/M = 99213: Urban Myth?
 
I have heard repeatedly that if you write a prescription, then you should bill CPT 99213, even if your documentation is brief. For example:
S - CC itching feet x 2 weeks, sudden onset, progressive,  
O - erythema and scaling plantar aspect b/l,
A - tinea pedis,
P - Rx Spectazole cream bid, 
 
Can this be CPT 99213 because of the prescription?

Response: The fact that you are writing a prescription alone does not automatically bump up your level or guarantee you a level "3" E/M service. 
 
E/M levels are based on significance/relevancy, medical necessity, and documentation of the history, examination, and decision-making. For an initial visit, all three components need to be documented to validate a level of E/M. For follow-up visits, 2 of 3 of the components need to be documented (and relevant - in most cases representing interval changes or new information from previous encounters).

The Medicaid policy bemoaned by Dr. Gegerson has been in place in MA since 1990, when the state was in a financial crunch. Clever accountants at the State House figured out a way to stop paying part of their bills with creative accounting. I first called Medicare because I believed the gouging was illegal. The CMS rep told me, "It's not illegal, but it is immoral. Why don't you just drop out of Medicaid and then you can bill the patients?" I tried that, sent bills to patients for $3-$5 and got angry phone calls from them advising me that not only would they not pay the bills, but that they would then go to the DPM down the street who would not send them a bill. So much for patient loyalty and concern for quality of care. NY is simply following the example of multiple other states that decided that doctors were rich enough and did not need that piddling 20% anyway.

Dr. Bregman, I have a similar story that I believe resulted in limb salvage for one of my patients. I was referred an uncontrolled diabetic patient with a 6-month limb-threatening sub-3rd metatarsal ulceration. Her circulation was also affected by her diabetes and she had been on Levaquin for 1 month. 
 
For these ulcers, I have employed gastrocnemius recession with the usual good results that most of us get with this soft tissue rebalancing. I was reluctant to perform the ...

RESPONSES/COMMENTS (PM QUICK POLLS)

I have read with great interest the polling results regarding PQRS penalties posted on this forum. The polled responses, I believe, are somewhat skewed to those individuals who were subjected to the penalties, simply because they either chose not to participate, attempted to perform this manually, or used a system which did not track or automatically bill the appropriate "G" codes when required. Certainly, the possibility exists that CMS also made errors. The system is fraught with not having sufficient numbers of trained support staff and the only way to get through is being persistent or calling at the crack of dawn. The appeals system appears to also be fraught with problems, and from those who attempted an appeal, it appears to be a nightmare with no positive results. 
 
In speaking with those who use the same EHR in the same manner as I do (ICS Software - AKA Sammy), I have found no podiatrists who were subjected to the penalty. This silent majority using similar systems thus has...
