'Tis the Season for Stress Fractures: DC Podiatrist
In the winter months, slippery walking conditions and seasonal sports like ice skating, snowboarding, and sledding are some of the leading causes of foot-related injuries. Even standing on a hard floor for too long can produce a stress fracture. “I’ve treated several patients with stress fractures in November after they volunteered at the polls and stood on a hard surface for hours,” says Timothy Swartz, DPM, FACFAS, a Fellow of the American College of Foot and Ankle Surgeons and Chief of Podiatric Surgery at DCSM Kaiser Permanente, in the Washington, DC area.

It’s important to be aware of the signs of a stress fracture, especially following a potentially stressful activity or fall, and to seek appropriate care. Pain, swelling, redness, and possibly bruising can all be signs of a stress fracture. “The pain typically occurs on the top of the foot,” says Dr. Swartz. “The symptoms usually pop up quickly and then subside if the person stops the activity. Beginning treatment as soon as possible is very important and may shorten your recovery time,” says Dr. Swartz. “Treatment can take four to six weeks if you catch the problem early.” During this time, it’s important to rest the foot and possibly wear a surgical shoe or cast boot. In a small percentage of patients, surgery may be needed to stabilize the fracture.

Podiatrists Among Winners in Analysis of Provider Pay
Podiatrists are among the clear winners in a four-year analysis of provider pay by specialty under the Medicare Physician Fee Schedule, thanks to the tireless efforts of APMA’s representatives to AMA’s Relative Value Scale Update Committee (RUC) and Current Procedural Terminology (CPT) Editorial Panel. According to the analysis, conducted by Health Leaders Media, podiatrists have seen a 3-percent increase in pay over the past four years. That growth places podiatric medicine firmly in the top 20 specialties. Other specialties saw significant decreases in pay, up to a whopping 23 percent, over the past four years.

NY Podiatrist Lectures on Forensic Podiatry to UK School of Health
Forensic podiatrist Bryan B Kagan, DPM recently lectured at the School of Health of the University of Northampton, Northampton, England. He presented a lecture on forensic podiatry to second year podiatry students and also lectured on "Personal Perceptions of the American Healthcare System" to faculty, administration, and the student body of the School of Health.

DE Podiatrist Warns of Carbon Monoxide Poisoning
With temperatures dipping to freezing, home heating season has begun. Now is the time to protect homes against carbon monoxide, a colorless, odorless gas that is produced by fuel-burning heating sources, appliances, and engines. Furnaces, vehicles, snow blowers, portable generators, stoves, lanterns, gas ranges, and charcoal or wood grills are all potential sources of this dangerous gas. Symptoms of carbon monoxide poisoning include feeling dizzy, light-headed, or nauseous.

“Each year, more than 400 Americans die from unintentional carbon monoxide poisoning, more than 20,000 visit the emergency room, and more than 4,000 are hospitalized due to poisoning,” said Ming Lau, DPM, environmental health toxicologist at the Division of Public Health. “Unborn babies, infants, elderly people and those who suffer from anemia, respiratory or heart disease are most susceptible.” Carbon monoxide poisoning is entirely preventable. Battery-operated carbon monoxide detectors should be checked monthly.

New CMS Rules Increase Medicare Provider Oversight
CMS Administrator Marilyn Tavenner announced new rules that strengthen oversight of Medicare providers and protect taxpayer dollars from bad actors. These new safeguards are designed to prevent physicians and other providers with unpaid debt from re-entering Medicare, remove providers with patterns or practices of abusive billing, and implement other provisions to help save more than $327 million annually.
New changes announced allow CMS to:

Deny enrollment to providers, suppliers and owners affiliated with any entity that has unpaid Medicare debt; this will prevent people and entities that have incurred substantial Medicare debts from exiting the program and then attempting to re-enroll as a new business to avoid repayment of the outstanding Medicare debt.

Deny or revoke the enrollment of a provider or supplier if a managing employee has been convicted of a felony offense that CMS determines to be detrimental to Medicare beneficiaries. The recently implemented background checks will provide CMS with more information about felony convictions for high risk providers or suppliers.

Revoke enrollments of providers and suppliers engaging in abuse of billing privileges by demonstrating a pattern or practice of billing for services that do not meet Medicare requirements.

RE: PCA Pumps and Common Sense

For 20 years, I have used home PCA pumps for post-operative pain management for non-compromised healthy individuals following complex rearfoot or midfoot reconstruction. Up until recently, I thought that by avoiding a hospital stay, I was doing my part in containing peri-operative costs. I have recently been notified by the last two patients and the nursing service that sets up the PCA that reimbursement for the nursing service that the PCA pump is being denied by Blue Cross/Blue Shield of Mississippi. This same insurance company has covered this service in the past. It seems as though the company would rather pay for a hospital admission and stay rather than cover a home PCA. I guess I will just have to bite the bullet and do these cases in the hospital. Where has common sense gone?
