Cuts in Insurance Coverage are Killing People: AZ Podiatrist
 
During the last recession, Arizona cut [insurance] coverage for tens of thousands of people. This year, the state expanded Medicaid again, although the decision faces a legal challenge. Arizona's on-again, off-again approach makes it a useful place to look at the impact of a program that covers more than 1 in 5 Americans. At the University of Arizona Medical Center in Tucson, Dr. David Armstrong still sees the impact. Nearly every day, Armstrong and Dr. Joseph L. Mills, who co-direct the Southern Arizona Limb Salvage Alliance, don surgical scrubs and step into an operating room to amputate limbs or cut away diseased bones and flesh of patients with diabetes who put off seeing a doctor.

One afternoon, Armstrong's team labored to save the black, gangrenous left foot of a 30-year-old diabetic, sawing out infected bones, stitching healthy tissue and grafting skin in a bid to spare him life in a wheelchair. "If it was just a couple of months earlier, we probably could have stopped some of these problems," Armstrong said. Armstrong and other researchers documented a 37% increase in hospital admissions for diabetic foot ulcers between 2006 and 2011, according to a study they published in the journal Diabetes Care. Patients also stayed longer in the hospital with more severe outcomes, they found. Amputations, surgical complications, life-threatening infections, and deaths increased by nearly half. "These people's feet were literally killing them," Armstrong said.

Podiatric Care by NY Podiatrist Saves Patient's Foot From Amputation
 
Roberto Mosconi was warned. If he didn’t lose weight, get active, and take his medicine, he’d develop diabetes. It wasn’t until a year-and-a-half ago, when complications from the disease resulted in the development of gangrene and possible amputation of his left foot, that he understood the severity of his diabetes. Thankfully for Mosconi, his doctors saved his foot from amputation. Dr. John L. Zboinski, a podiatrist, surgically removed the infected tissues in his foot at Northern Dutchess Hospital in Rhinebeck, but because the operation left a gaping hole, Mosconi then underwent hyperbaric therapy at the hospital’s Wound Care and Hyperbaric Therapy Center.

Each of the 42 treatments, over several weeks, had Mosconi spending two hours lying in an acrylic tube at the center, during which time pressurized oxygen spurred the wound’s healing. Beyond that, Mosconi had intravenous antibiotic treatments, plus his wound was treated with both a wound vacuum to remove excess fluid and an antibacterial honey paste, all to promote healing. It worked. Mosconi’s foot healed and he’s glad to be back at work assisting the chef at Rhinebeck’s Liberty Tavern.

NJ Podiatrist Celebrates 25th Year in Practice With Housecall to 1st Patient
 
Dr. Lori L. Lewis knew as a Woodbury sixth grader that she was going to be a doctor. She just didn't know what kind of doctor. A family doc, perhaps. Or a dentist. By the time she entered junior high, she knew. Watching her dad, Arthur Lewis Sr., struggle with the ravages of diabetes, she decided on podiatry as her chosen field. He passed away at age 49.

The bubbly, energetic woman who made the U.S. Olympic team trials as a sprinter in 1976 celebrated 25 years as a podiatrist and surgeon recently. Dozens of clients and friends crowded into her office on Route 45 in south Woodbury to hail her career. Her first patient, Helen Harrison, couldn't make the party, but Lori showed up with a corsage for her at her doorstep.

Query: Can an EMR Company Hold Data Hostage?
 
I have become disenchanted with the EMR that I have had since 2010. I have received all the money associated with having and using this software to date; and for their help I am grateful. But this program is expensive (approximately $600/month plus the $9,000 to set it up) and, for me, difficult to navigate even after four years of 100% use. I have found and tried another that is easier and is FREE—not only free but with 24/7 help. If I now change to the free program and forego the previous paid one, can my old EMR hold hostage my records? I have heard horror stories of having to pay upwards of $20,000 to have access.

How long do I have to store my old x-ray films?

Response: X-rays are the same as any other part of a patient's chart and must be kept the same length of time that the rest of a chart should be maintained. 
 
Most experts recommend that charts be held for (7) years. X-rays (and charts) for minors should be held for a variable length which is calculated as until the child reaches the age of majority (usually 18) plus the statue of limitations for malpractice in your state.  Click here for state by state requirements.

I have been involved part-time in the VA system  for over 25 years and have been using CPRS and VistA since it was implemented at my VA facility.  In reference to all the posts about CPRS (VistA),  the EHR System used by the VA Systems, I would like to point out a few things: 
 
*It is freely available for download to anybody. In fact, the very first EHR I installed at home when playing with systems was VistA. 
*The backend runs on Linux servers and was not  built for Windows computers. 
*The front end CPRS is written in Delphi and is built to run on Windows computers.

We have to do that. We have to use this form for this. No form for that. This requires authorization. We can’t hold the member responsible. We can do this in 61 days, but not in 60. This is no guarantee of payment.  On and on for eternity.
