VA Podiatrist Wins National Recognition for Groundbreaking Research 
 
Brandon Brooks, DPM, a podiatrist at the Columbia VA Health Care System, earned first place at the 10th Annual Conference of the American Society of Podiatric Surgeons for his abstract, DREADD: Diabetes-Related Extremity Amputation Depression & Distress, this past month. This achievement marks his third consecutive first place win in national abstract competitions, solidifying his reputation as a leader in podiatric research focused on improving care for veterans.

The winning abstract introduced the DREADD phenomenon, a term coined to describe the emotional and psychological toll experienced by patients following any diabetes-related amputation. The study focused on “minor, non-traumatic” amputations, such as the loss of a toe. While these procedures are often viewed as relatively minor from a surgical perspective, Dr. Brooks’ research reveals that they can have significant psychological consequences among patients with Type 2 diabetes mellitus.

PFA Celebrates 50th Anniversary 
 
Robert Schwartz, the executive director of the Pedorthic Footwear Association (PFA) has a message for podiatry: Do not forget the importance of footwear in your treatment plan. "We walk around on concrete surfaces in boxes and there are better designs for those boxes that can compensate for a pathologic foot. Footwear forensics is like a fast x-ray for foot pathology," said Schwartz.

While celebrating the PFA’s 50th anniversary at the Run Event in Austin, TX, Josh White, DPM was recognized for his partnership with PFA. Ben Pearl, DPM said that he benefitted from listening to information and tips that could be as simple as using double-sided tape on the bottom heel of an orthotic to decrease blisters due to friction.

Reserve Wearing High Heels for Special Occasions: IL Podiatrist
 
High heels are the No. 1 culprit of foot pain for women, according to an American Podiatric Medical Association survey. Nearly half of all women wear heels, and 71 percent of heel-wearers say those shoes hurt their feet, the APMA reports. “With many types of heels, like very high stilettos, foot pain is hard to avoid,” says Matthew G. Garoufalis, DPM, a podiatrist and past president of the APMA. “But it is possible for women to find a happy mid-point between great looks and great pain.”

Even if you’re like the average American woman and own nine pairs of high heels, don’t wear them every day. Daily heel-wearing can cause the Achilles tendon to shorten, increasing your risk of an injury while doing activities in flat shoes, including exercise. “Treat heels like dessert,” Dr. Garoufalis says. “Don’t wear them all the time, just on special occasions.”

AMG0001 Advances Healing in CLTI
 
Gene therapy using AMG0001, a plasmid encoding hepatocyte growth factor (HGF), significantly shortened wound healing time and improved ulcer healing rates in patients with chronic limb-threatening ischemia (CLTI), according to featured science at the American Heart Association (AHA) Scientific Sessions 2024.

“We look forward to further work that will confirm this or refute it, but this is a real glimmer of hope in an area that needs it,” investigator David G. Armstrong, DPM, PhD, University of Southern California Keck School of Medicine, told HCPLive. “A lot of times, there haven’t been good therapies for these patients and they’ve been largely ignored. From that standpoint, they’re not going to be ignored anymore.”

QUERIES (CODING & BILLING)
Query: What is Needed in an Encounter Note
 
With the advancing capacity of AI note-taking technology, I have come to question what is needed in an encounter note. The simple answer is that a note should include relevant medical information. But I am not interested in what it should include because that can waste time. My question is what must be in a medical note to pass an insurance audit. The term "relevant" is very subjective. For instance, a patient's family history, social history, dermatological findings, and vital signs are irrelevant to how I treat their plantar fasciitis.

Query: Scanning Paper Records
 
I recently joined a large podiatry group using ModMed EHR. I am transitioning my active patient paper charts by scanning into Mod Med easily enough. However, I need to scan my prior inactive patients and deceased patients' records onto a separate external hard drive with an EHR that is not part of the group practice responsibility.   
 
What would be a relatively inexpensive EHR or alternative method that would allow storage and retrieval of patient records if needed, as I am now legally responsible for the past 7 years for these records?

I read with interest the story of Dr. Markinson diagnosing acral melanoma. Many years ago, I covered a hospital clinic when a patient presented with what they thought was a mycotic hallux nail. The patient had been treating it for several months and I expected it to look better than it did. With a high index of suspicion, I biopsied the nail bed. The next week the chief pathologist was waiting to tell me the patient had amelanotic melanoma. Proof that if we don't see what we expect, we need to look closer.
