MDS IN THE NEWS
Light Therapy May Offer Relief for OA of the Foot & Ankle

Mobility can be an issue as you get older, but the problem may begin with foot pain or soreness which makes it harder to walk, stand up, or climb stairs. It can affect balance, and could involve other parts of the body such as the knees and spine. "Ignoring foot pain is often to the detriment of the patient's life," says Lance M. Silverman, MD, of Silverman Ankle and Foot in Edina, MN. "Living with it is always an option and patient choice. But there are so many non-surgical management and surgical treatment options available to patients to help them live more productive and fulfilling lives."

Dr. Lance Silverman

Osteoarthritis is often linked to genetics or being overweight, but can also stem from a past foot or ankle injury. It damages the cartilage joint surface and can cause pain or swelling in the foot or ankle but can also harm the bones and ligaments of the affected areas. Although not a cure, light therapy can offer some relief by stimulating cells and increasing circulation. “Light therapy has some investigated medical benefits, typically from pain relief of arthritis or tendinitis,” says Dr. Silverman.

STATE PODIATRY NEWS
Proposed Bipartisan Legislation Would Allow Recent WI Vets With Medical Training to Temporarily Practice Without a License

As hospitals in Wisconsin struggle to find enough staff, there are veterans with medical training who could potentially help. A bill recently introduced in the state Legislature would allow recent vets to provide limited healthcare without a license under supervision of a physician, physician assistant, podiatrist, registered professional nurse or advanced practice nurse.

The bipartisan bill sponsored by state Sen. Andre Jacque, R-Green Bay comes as hospital staff face pandemic fatigue, retirements, and an increase in patients while the Midwest sees a rise in COVID-19 infections. Under the proposed Wisconsin legislation, military medical personnel would sign a memorandum of understanding detailing their scope of practice and establish a timeline to acquire a license from the Medical Examining Board, the Podiatry Affiliated Credentialing Board, the Physician Assistant Affiliated Credentialing Board, or the Board of Nursing.

PODIATRISTS AND FOOTWEAR
NY Podiatrist Discusses How to Select Shoes for Bunions

There are plenty of common foot ailments that lead to pain, like plantar fasciitis, flat feet, and high arches. Bunions, however, can be particularly painful. It has sent many people, primarily women, to the podiatrist’s office, as Glenn Davison, DPM, owner of Advanced Podiatry in New York City and New Jersey and a Fellow in the American College of Foot and Ankle Surgeons, can attest. The good news is that, according to Dr. Davison, with early intervention, it is possible to avoid bunions. That means obtaining proper foot support with the best shoes for bunions.

Dr. Glenn Davison

“A good corrective shoe will stabilize the foot,” Davison advises. That means look for qualities like arch support, as well as proper cushioning to stay comfortable. One of the biggest keys for bunion sufferers is to look for a wide shoe fit and a roomy toe box. “Avoid straight or narrow toe beds,” Davison warns, which can result in painful friction.

PODIATRIC PRODUCTS AND SERVICES IN THE NEWS
Topo Athletic Awarded APMA Seal of Acceptance for Select Models

Topo Athletic has announced that the American Podiatric Medical Association (APMA) has bestowed the Seal of Acceptance on all seven of the performance shoe models Topo submitted for evaluation. “Footwear education is such an integral part of my practice and I always recommend shoes that match the shape of a human foot rather than forcing feet to conform to the shape of shoes,” said podiatrist Neil J. Feldman, DPM of Central Massachusetts Podiatry.

Dr. Neil Feldman

“Topo Athletic shoes have always been my go-to – and not just for myself, but also for my patients. I appreciate their gimmick-free design and focus on only the elements that let my patients move naturally and pain free. I’m proud that the APMA has recognized the Topo models on the Seal of Acceptance list with this very important distinction for our industry,” said Dr. Feldman.

PRACTICE MANAGEMENT TIP OF THE DAY
2021 Year-end Financial Planning Checklist

The end of the year is a good time to check in on your financial plan to see if everything is on track or if some things need adjusting. You will be in a better position for 2022 the sooner these items are addressed and adjusted if they are off-track.

3. Money Saved in Flexible Spending Accounts (FSAs)

Utilizing a healthcare, dependent care, or limited purpose FSA is a great way to pay for certain services on a pre-tax basis. Be aware, most FSAs are use-it-or-lose-it accounts, meaning any money remaining at the end of the year gets forfeited back to the plan, not back to you. Some employers allow a $500 carryover to the following year or a grace period into the spring, but most do not.

CODINGLINE CORNER
Query: Extension for Wound VAC Use

My patient is a type I diabetic with a chronic, tunneling wound at the right foot. MRSA osteomyelitis is present at the location. The patient is blind and requires dialysis. The patient is wheelchair bound and ambulation is impossible. We have diminished the MRSA infection with the use of a wound vacuum assisted closure (VAC) device, heretofore paid for by Medicare. According to the VAC device company, Medicare has a rule they will continue to pay for only four months. Neither I nor her PCP can determine how long she has to heal. The wound VAC device has allowed her to remain in her home. She has actually regained her appetite and has a fairly happy life with the assistance of caregivers. I would appreciate any advice on getting an extension from Medicare to use  the VAC device.
