Toronto Podiatrists Participate in Special Olympics Fit Feet Screening

On Saturday November 29th, 2014, four Canadian podiatrists participated in a Special Olympics Healthy Athletes "Fit Feet" screening program at a public school in Barrie, Ontario. "Fit Feet" was just one of the 5 health disciplines that participated at this event which screened several hundred Special Olympic athletes. "Many of the participating athletes were here to be screened before their upcoming winter sports competitions," said Dr. Robert Chelin.

(L-R) Drs. Robert Chelin, Bruce Ramsden, Chris Hastings, and Kel Sherkin

Participating in the screening were Toronto podiatrists: Drs. Robert Chelin, Bruce Ramsden, Chris Hastings and Kel Sherkin. The Special Olympics Healthy Athletes "Fit Feet" program has developed a strong foothold in Canada as it has in other countries around the globe.

PODIATRISTS AND DIABETES
Diabetics Need to Take Extra Precautions When Holiday Shopping: NV Podiatrist

Dr. Denise Tropea, a Las Vegas podiatrist whose practice is affiliated with the Preventive Foot Care Centers of America Network, has some tips and suggestions that can help women keep their feet happy during the holiday season. And, while these tips are important for all women, Tropea points out that diabetic women should pay special attention to them, because diabetics are at risk of serious complications that can arise from minor foot problems, even a small blister caused by a tight shoe.

Dr. Denise Tropea

When feet get tired from trudging around shopping, they tend to perspire a lot. So Tropea advises you wear cotton socks to absorb the moisture. Moist skin is easily irritated by shoe friction, so keeping your feet dry can prevent blisters and sore spots. In addition to being painful, these irritated areas can become infected, which can lead to more serious problems, especially if you are diabetic.

RESPONSES/COMMENTS (CODINGLINE CORNER)
From: James W. Clark, DPM

I prefer to be reimbursed for my knowledge and expertise, particularly when it involves surgical patients. Have you ever seen what a general surgeon does with an ingrown nail? We podiatrists are SPECIALISTS when it comes to nail surgery. Try doing both borders on one nail; you will only get reimbursed for one border. Have you had a root canal done lately? What did that cost? When I began in private practice, the fee for a dental root canal and nail matrixectomy were the same. Compare those fees today - you'll be shocked!

In my area, general surgeons continue to perform cold steel matrixectomies under general anesthesic at our surgery center. We routinely perform these procedures in our office and save the insurance companies untold sums of money. Often, nail surgery patients fail to take their prescribed antiobiotics and follow soaking instructions, resulting in post-operative complications. So, do you want to see these patients for free?

A 10-day follow-up on matrixectomies is the accepted policy. After 10 days, it should be fee for service. Don't devalue your medical services. The insurance companies have already done that for us. Remember, we are no longer DOCTORS - we are now "Providers", so say the insurance companies.

RESPONSES/COMMENTS (EMR)
From: Judd Davis

In response to Dr. Gottlieb's posting about the government making an EHR available, I must disagree. The link you provided appears to be for a demo version only, not a complete version for everday use in a private practice setting. I agree that the VA EHR system (VISTA/CPRS) is very efficient and useful. Having completed part of my residency through a VA system 10 years ago, I realize the value of their EHR. I had been told by several sources that I could download and use their system in a private practice setting for free. After extensive research, I determined that the "free" version was only available through private vendor software companies at a cost of $20k or more, so I opted not to do it. If you know of a way we can obtain the real usable version of CPRS, please post it.

I agree with most other postings on this matter. The government should have modified their VA EHR system and made it available for free to any private practices/hospitals who wanted to use it. It would have been interoperable which was the whole point, right? Sharing all this data is where the efficiency will occur. This surely would have been more useful and cost a whole lot less than the mess we see now. Instead, we have dozens, if not hundreds, of privately-designed EHRs, most of which do not communicate with one another, thereby defeating the whole purpose. The EHR vendors are laughing all the way to the bank, and the doctors/hospitals are now wasting their time jumping through the Stage 2 hoops. I call it Meaningless Use.
