Sara Rose-Sauld, DPM, a podiatrist in the Foot and Ankle Division of the Orthopaedic Department at Massachusetts General Hospital (MGH), has co-edited the recently published text, The Massachusetts General Hospital Approach to Transcatheter Arterialization of the Deep Veins for Advanced Limb Salvage.

Dr. Rose-Sauld is a 2016 graduate of Barry University School of Podiatric Medicine. She came to MGH after completing her residency at Palmetto General Hospital in Hialeah, Florida in 2019. She holds a Harvard Medical School appointment as an associate professor. Since joining MGH, she has integrated herself into the vascular services department of the Wound Care Center, bringing needed podiatric expertise to the wound care program. She is a founding member of the MGH LEAPP program.

Guam Podiatrist Supports Proposed Medical Complex    
 
Gov. Lou Leon Guerrero met with a full room of healthcare workers Monday night for an informational meeting on a new medical complex, one of several such meetings the governor has held to drum up support for the project. Teresa Borja, DPM, a local podiatrist, stated that medical professionals arguing against the medical complex may be retired by the time the complex is up and running. "I want this opportunity for a medical complex. I want that for myself. I want that for my 14-year-old son. I want that for my mother, who is 85," Borja said.

She also stated that placing the new hospital at Eda Agaga won't be a problem for her. "It's 20 minutes from my Tamuning location from where I will be living," Dr. Borja said. "Think about the people that are opposed to this Eda Agaga location. They're older than I am. They're like ten years older than I am. Think about the future. This location will address people that don't have the access."

Query: Nail Avulsion And Matrixectomy
 
I am resident and had a conversation among my peers about the coding for a surgical nail procedure. We have a patient that suffers from onychocryptosis, and in the clinic we perform a digital block using lidocaine. We then perform a partial nail avulsion of the medial border of the hallux. To complete the treatment, we apply phenol to destroy the matrix of the offending portion of the nail. Our question: Can we bill for CPT 11730 and CPT 11750?

Response: No, you cannot. The CPT 11730 is inherent in the procedure with CPT 11750. This would be like billing for an exostectomy of the 1st metatarsal when doing a McBride or similar. It is part of what is required to do the “bigger” procedure. This unbundling may have been happening in the past but it would equate to double billing essentially the same procedure.

RE: Foot Injuries Associated with Pickleball

Over my career, I have treated foot and ankle injuries associated with the popularity of certain sports. As a profession, whave addressed our role in treating sports injuries. The best example was jogging and marathon running. Podiatry recognized that it has a role to play to preventing and treating running-related injuries. The same applies to all other sports; and many of our orthotic labs offer orthoses designed for basketball, tennis, skiing, jogging, etc.
 
The new "game" in town is called pickleball. It is a cross between table tennis, regular tennis, and badminton. It is taking America and many other countries over by storm. Pickleball courts are being built all over, either in parks or in indoor facilities, and you have to literally get into a queue in order to experience this new sport. Many indoor tennis facilities are converting their courts to this new sport. A physical therapist whose practice is at a gym above an indoor pickleball court said that he is seeing a great deal of injuries associated with this new sport. It seems easy and people are not taking it seriously. These can either be long-term overuse injuries or acute injuries.

The G2211 code became more widely used and prominent during the COVID-19 crisis for podiatrists practicing wound healing. During that time, due to limited access, podiatrists became the most common medical contact for many patients with chronic non-healing wounds, and therefore became the primary source of much of the medical care. This included direct care to the patient as well as serving as the key resource for referral to other specialties. 
 
As that crisis has now passed, it would appear that it is less commonly used because of the increased access to multiple specialties. Podiatrists are no longer serving as much as a key provider coordinating care for the patient. This is an add-on code and not a stand-alone. It's used for E/M services.

In the past, I have opined in this space on the need for DPMs to use extenders for nail care. As I stated before, back in 2008, I inquired about this with our state society (PPMA). I was told that it was not within our license for a DPM to supervise nail care. For a DPM to bill for nail care services, the DPM MUST perform the service personally; the use of an extender would be insurance fraud.  
 
Our PPMA representative even told me of a DPM who, at that time, was being taken to court for insurance, i.e. Medicare fraud, because he used a nail technician under his supervision and billed Medicare for nail debridement. My question to Ms. McCormick is how can we even discuss the merits of using such ancillary personnel if it is illegal, at least in Pennsylvania, to use nail care extenders? Or has the law changed that allows for this practice?
