Canadian Podiatrist Returns to His Hometown
Dr. Neil Brown is the only doctor of podiatric medicine in Kelowna to do surgeries in his clinic. Okanagan born and raised, Brown went off to UBC Vancouver to become a pharmacist and then to Samuel Merritt College in Oakland, California to become a doctor of podiatric medicine. He did his surgery residency at Vancouver General Hospital and Medstar Washington Hospital and Georgetown University Hospital in Washington, DC.

Brown wanted to move back to Okanagan, so he opened Kelowna Podiatry Office at the Harvey Ellis Professional Building in Kelowna last year. To utilize his pharmacy degree, Brown also works one day a week as a pharmacist at Shoppers Drug Mart. His objective is to obtain courtesy staff privileges at Kelowna General Hospital so he can do more foot surgeries.

| RESPONSES/COMMENTS (CLINICAL) | |
From: Michael Rivera, DPM
Clearly, there is a significant supinatus of the forefoot, and/or a hypermobile 1st ray with navicular, cuneiform, and 1st metatarsal involvement. There is flattening of the talar head with talar-navicular beaking. The Cyma line is incongruent, with decreased CI angle, stable TC angle with an hypertrophied navicular process. I suggest an MRI or CT to R/O a tarsal coalition.
My suggestion is a Koutsigiannis procedure (medial Calcaneal slide), and/or an Evans with a plantarflexory Cotton procedure and a Lapidus arthrodesis. If there is no STJ coalition, an arthroereisis may be an opition. Additional ancillary exams should be performed prior to surgical correction. In addition, take hindfoot posterior calcaneal axial x-rays to assess the rearfoot position. Most importantly, perform a TAL or Gastroc. recession if there is equinus involvement.

Dr. Sherman is correct, and yes, the PPACA should have included EHR standardization, but with the won't-do-anything-Congress that only wants to repeal the PPACA, you can forget about anything useful happening.

From: Cynthia Ferrelli, DPM
I once discussed this question with a colleague of mine who sat on the NYS Board. It was the Board’s view that these post-operative appointments were really non-billable, even after the global period, since there usually is not much to report to ascertain a CPT 99212. Ever since that discussion, I have not charged for such visits. Usually, I will see the patient twice after a matrixectomy procedure, and we just give them a window of time to show up at the office, i.e., 1-3 PM, or whatever fits into their schedule.
The front desk tells them it's just a quick check and that when a room becomes available, I will just take a quick peek and make sure that everything is okay. We tell the patient ahead of time that unless there is some kind of problem, there will not be a co-pay. This way, we don't utilize a slot in the schedule book. This has worked very well for many years.
