MI Podiatrist Discusses Contributing Factors for DFUs
“Two common contributing factors to the high incidence of diabetic foot ulcers: peripheral neuropathy and peripheral arterial disease,” said Jonathon Rayman, DPM. The nerve damage resulting from peripheral neuropathy can cause numbness in the hands and feet, leading to foot ulcers. “For example, if they step on something like a needle or a rock they might not notice it, or they’re wearing a pair of shoes that are too tight or are starting to cause a blister. They don’t feel it and just continue walking, and they develop an infection or a wound that goes deep through the skin,” Dr. Rayman explained.

Peripheral arterial disease, another common condition in people with diabetes, is linked to foot ulcers because it causes the arteries in the legs and feet to calcify. That calcification can lead to gangrene or infections as well,” Dr. Rayman said.

OH Podiatrist Discusses Pickleball Injuries
According to A County Medical Center (ACMC) podiatrist, Michelle Dunbar, DPM, feet can bear the brunt of the hard stops, starts, and stretches that are common in a pickleball court. “Aside from the acute injuries, pickleball players can experience heel bruising, which occurs when the pad of fat surrounding the heel bone is damaged due to repetitive impact or an overly aggressive step when lunging to hit the ball,” she said.

“Another area affected by strenuous or repetitive strain is our Achilles tendon. This band of tissues connects the calf and heel. As we age, the structure of the Achilles tendon weakens. People who have strained their Achilles tendon will experience pain in the back of the leg or near the heel.” Other symptoms may include stiffness or tenderness, especially in the morning, Dr. Dunbar said.

According to Saylee Tulpule, DPM, member of the Foot and Ankle Specialists of the Mid-Atlantic and spokesperson for the American Podiatric Medical Association, there are four factors to consider when shopping for a sturdy, warm winter boot: 1) Width to ensure adequate space for the forefoot—“especially for people with bunions. 2) Traction to prevent slippage—usually in the form of a thick, grippy rubber sole.

RE: Data iSight is Trying to Kill Out-Of-Network Medicine

For out-of-network doctors, some carriers like Aetna, Cigna, and United Healthcare have started to use a third-party company called Data iSight as a pricer/negotiator. Unlike Multiplan and other third-party negotiators, Data iSight is not actually doing any negotiating and are pricing the claims as they see fit and the big insurance companies that employ them are standing by whatever they say.
Data iSight is not determining the price according to the patients’ out-of-network reimbursement rate such as UCR rate or Medicare percentage, as per usual. Instead, they pay close to in-network rates and do not allow the patient the chance to use their paid for out-of-network benefits. It seems like both the patients and the out-of-network providers are getting stiffed.
