Twistable Running Shoes Help Build Strong Muscles: NY Podiatrist 
 
Running sneakers tend to have more control to brace for impact on the sole, while walking sneakers have a bit more flex and bend. Many walking sneakers have added foam, which is especially important for people who are heavier and put more weight on the shoe because it’ll return some of the force back to your body. Emily Splichal, DPM recommends only running in running sneakers, especially ones with more cushioning for long distance running.

Patients adjust to this policy, and our late-cancel and no-show rates have declined.

How can I find out what diagnosis codes can be used for CPT 29540? I have checked the Codingline archives and can’t find any helpful information.

Response: It depends on payer policy. Because you are in Pennsylvania, I am providing you Novitas’ information, the Medicare Carrier’s LCA strapping link. Although it is now retired as of 7/23/2020, it was a good place to look at ICD-10 coverage associated with CPT 29540. Look at the “Group 7 codes.” See link.
 
Lastly, different payers have different policies. Make sure you look up the specific payer’s medical policy to determine their covered diagnosis codes.

RE: Primary Prevention of Gout in Men by Modifying Obesity and Other Factors

A study by McCormick, N, et al. of 44,654 U.S. men followed up over 26 years estimated that 77% of confirmed gout cases may have been prevented had all men been of normal weight and adhered to a diet similar to the "Dietary Approaches to Stop Hypertension" (DASH) pattern, with no intake of alcohol or diuretics. However, among men with obesity, modification of the other factors would not have prevented gout.
 
The authors also suggest that addressing excess adiposity and other key modifiable factors has the potential to prevent the majority of incidents of gout cases among men. JAMA Netw Open. 2020;3(11):e2027421.

RE: EHRs, Insurance Carriers, and Prior Authorization

There are numerous times when E-prescribing that I am told that I need pre-authorization for medications. Oftentimes, this can be for ciclopirox - the generic version of Penlac. My EHR invariably is not able to handle the prior authorization and the office has to call in for it. The doctor is mandated to use the EHR. If one chooses not to, they get penalized by Medicare. These systems cost doctors money. Why doesn’t the government force the insurance companies to be compatible and up-to-date with the doctors' EHRs? 
 
I opine if we can’t get an electronic response from the insurance company, then they shouldn’t be allowed to require a prior authorization in the first place. Furthermore, prior authorization on generic medications should not be allowed unless someone can give a logical explanation as to why it’s needed.

I remember Dennis from when he first went into practice. He was an extremely intelligent, capable, and soft spoken man. The profession was lucky to have him. I was sorry to learn of his death and especially the difficulties he suffered in his last years. This event and the Covid pandemic remind us all that "tomorrow is not promised." Dennis delayed his retirement only to find that so little of it was to be enjoyed as he had planned.
 
In this troubling time, I remind everyone that there is a whole world outside of podiatry. If it is your intention to explore it, and you can, do it! That "someday" may never come. Review your finances with a professional, if necessary. See what's left on your "bucket list" and take the plunge. I always said, "podiatry is what I do; it is not who I am." Tomorrow is not promised, so open those doors to the future as soon as you can.
