OFAMA Opposes Bill to Limit "Step Therapy"
According to Jimelle Rumberg, executive director of the Ohio Foot and Ankle Medical Association, "State Senators Peggy Lehner, R-Kettering, and Charleta Tavares, D-Columbus, introduced House Bill 243 to limit 'step therapy.' That sounds like something of special interest to foot and ankle doctors, but in fact it’s the way health insurers second-guess all types of physicians by refusing to pay for medications they’ve prescribed. As an alternative, the insurer makes patients go through a number of “steps” trying cheaper medications instead of the one the physician has prescribed. Therefore, only if these other medications fail will the insurer allow the original prescription. That’s not good business or good medicine. Patient care should not be dictated by the whims of an insurance company trying to save a dollar."

"Our surgeons encounter this practice frequently. Many surgical patients, including amputees, need prescription pain medication over an extended period. Their doctors work closely with them to manage their pain and speed their rehabilitation. However, we often find insurers who want to transition the patient to an over-the-counter pain reliever much too quickly. That can slow recovery and cause unnecessary suffering. These decisions should be made exclusively by the doctor in consultation with the patient, not by an insurance underwriter. We understand the need for cost containment in the healthcare industry, but step therapy is not the way to achieve it," says Rumberg.

OR Podiatrist Conducts 4th Annual Holiday Health Event
While chatter about Black Friday shopping bargains filled the airwaves Wednesday, patrons of the Medford Gospel Mission were being treated to something better than the biggest of holiday specials. Our Hearts to Your Soles, a local effort under the charge of podiatrist Dr. Rich Owens and local RN Michelle Caswell, set up shop with more than 300 pairs of shoes, twice as many pairs of socks, and some basic medical care, including foot exams, and flu shots.

In its fourth year, the event focused on helping the valley's most vulnerable population, many of whom spend much of their lives walking from place to place. "I do construction and worked two days last week. I've had a broken back, so that basically means 24/7 pain," Adrian Henson said. "The doctor doing this is a big-hearted podiatrist."

Half of Foot Problems Exacerbated by Bad Shoes: Aussie Podiatrist
Switching the high heels for a pair of flats on a long night out is a choice your feet will thank you for at the end of the day. But both kinds of shoes can do a lot of damage in the long-term if they are not providing your feet with the support they need. Sydney podiatrist Karl Lockett said at least half of the foot problems he treated were caused or exacerbated by bad shoes.

“The biggest misunderstanding is that high heels are bad, flat shoes are good,” he said. “There’s not necessarily one good shoe for everybody; it’s about knowing what your requirements are. ”For those who insist on wearing heels, Lockett recommends a chunkier heel, regular calf stretching, and wearing good shoes while walking in between.

Five Steps to Improve the Patient Experience - Part 6
5. Commit to Timeliness
Be prompt in answering phone calls, don't keep patients on hold more than 30 seconds, and don't jockey them around from one person to the other. Also be diligent in returning phone calls and e-mails within a standard time frame. Communicate the established timeframe to providers, staff, and patients. Again, this reinforces accountability.
Timeliness typically suffers the most when patients are kept waiting for their appointments. Check them in promptly and let them know how long they are likely to wait. Most practice management systems can track patients through the system beginning at check-in. It is important that staff understand this feature and use it consistently. It is a great tool and provides reports that measure performance in work flow and also helps identify any hiccups that keep patients waiting unnecessarily. It's an opportunity for everyone to improve patient service and patient flow.
You cannot manage what you do not measure. If other people are measuring your performance based on the patient experience, it is time for your practice to understand the significance and measure its own performance. This requires a commitment on the part of physicians and staff, and will lead to improving the patient experience.

I may have a distorted view on this topic, but I find it interesting that the pretense of the ACA was that ALL individuals were supposed to participate or be “fined.” The theory that healthy individuals would not utilize care, but would pay into the system, is a basic premise of all insurances. Spread the risk, and have as many people not use the system as possible. In order to make people join up, they would be faced with a penalty for not doing so. However, when the penalty for not signing up is less than what signing up would cost, the incentive is reduced.

This code may seem ridiculous to cityfolk, but in places like here in Upstate New York, such incidences are commonplace. Farming is a way of life with many hazards. While I was in practice, I treated many injuries due to livestock, e.g. multiple metatarsal fractures from being stepped on by a cow or horse. Up here, I suppose a code for being caught in a subway door would seem ridiculous.
