Chief White House Economic Advisor Receives Giant Foot from NY Podiatrist
 
Chief White House Economic Advisor Larry Kudlow recently received a giant foot from Rock Positano, DPM, MSc, MPH, in appreciation for Kudlow’s intervention (at Dr. Positano’s request) to allow Medicare recipients continued access to podiatric services, which would have been drastically impacted if a recent CMS proposal altering podiatrists’ reimbursement had been enacted.

Dr. Positano, the Founder and Director of the Non-Surgical Foot and Ankle Service and the Joe DiMaggio Sports Medicine Center at the Hospital for Special Surgery in New York, and Deputy Board Chairman of the New York College of Podiatric Medicine, had taken podiatrists’ grave concerns directly to Washington. Mr. Kudlow promised to display his gift prominently!

NY Podiatrist Featured as Game Changer for Veterans Healthcare
 
Wilma Hunt-Watts, DPM was featured as an international game changer in the social-impact focused magazine, The Introducer. The November 2018 issue's theme was "Veteran Advocates and Entrepreneurs." Dr. Watts shared information about her work with the Dept. of Veterans Affairs (DVA) and her current focus as telemedicine ambassador for veterans.

Anna Tien, DPM, a board-certified podiatrist, has joined the Silver Cross medical staff. Tien attended medical school at California College of Podiatric Medicine in Oakland. She completed her podiatry residency at Jesse Brown Veterans Affairs Medical Center in Chicago.

NZ Podiatrist Weighs in on Mall's "No Barefoot" Policy
 
A podiatrist has weighed in on the debate over whether it's acceptable to go barefoot at the mall. Rachelle McDonald, an Auckland woman, is furious after she was ejected from Sylvia Park shopping center in Mt Wellington because she was barefoot. Masterton podiatrist Adam Philps has given his expert opinion on the debate that has split the nation as a result of McDonald's story. He says while it's fine to go barefoot at home, "I wouldn't go around a mall without my shoes on."

He goes on to say that his daily practice has made him aware of the potential dangers of being barefoot in busy public areas. "The things I see every day in my practice [such as] infections in legs caused by cracks in skin around the toes..." He says that the more sub-tropical areas of New Zealand have "potential for bugs, hookworms getting into your feet and the like." Aside from the potential for bugs and infections, Philps understands the mall's policy, because "it's more of a health and safety thing. They don't want customers to be getting glass in their foot, or picking up a bug from a urine soaked floor."

Steps to Improve Patient Relationships and Increase Staff Efficiency – Part 4

4. Connect With Patients and Prospects on Social Media
 
Healthcare has been slower than many industries to adopt social media. Healthcare providers have a real opportunity to catch up and connect with patients in new ways. Social media sites such as Facebook and Twitter are powerful places to build your brand and engage in conversations. Seventy one percent of online adults used Facebook and 23 percent used Twitter as of 2014, and that number is continuing to grow. By meeting patients where they are, you can improve current relationships and gain new patients.
 
Social media channels also present opportunities to educate patients. Remind them to use sunscreen before the Fourth of July. Tell them about Saturday hours. In addition to these tips, patients also appreciate a “happy birthday” message. Personal messages like that can help forge bonds between your team and your patients. They remind patients they’re important to you.

Query: A Second AFO
 
I have a Medicare patient who has severe progressive deformity posterior tibial tendon dysfunction. Three years ago, she was dispensed a Richie brace. However, her deformity today has gotten so bad that she can no longer wear this brace. I want to make her a different type of custom molded brace-gauntlet. Can I get paid for this second AFO?

Response: Medicare’s AFO LCD stipulates that if the patient's diagnosis has changed, a second AFO dispensed three years after you were paid for their initial device should be reimbursed. You should eventually be paid and you will need to appeal an ever growing number of appeals for this type of circumstance.
 
Medicare is very likely to deny this claim for reasons cited as “Same or Similar.” That is, a similar device was already dispensed (some other type of device listed in the AFO LCD) and paid for within the reasonable lifetime (5 years). However, your notes must clearly document the fact that the patient’s deformity has worsened, that the initial device is no longer medically appropriate (state why) and that the new device is medically necessary (state why). If you follow that workflow, your appeal of the initial denial should be successful.
 
I suggest you obtain an ABN from the patient. Be sure and cite “Same and Similar” as your rationale for the claim denial and be as accurate as possible on what the patient’s financial responsibilities will be, should your appeals be denied.

Look at some of the panels on CNN, Fox News, and MSNBC. Some of the men wear ties and others do not. That's enough for me. I take the most comfortable way out and do not wear ties at all anymore. I just hope that they never do a survey showing that doctors with tuxedos make more money because that will never happen in my office in my lifetime.

I thought “BBE”, i.e. “bare below the elbow” had been adopted as appropriate infectious disease prevention protocol in many places, especially in the UK. Wouldn’t that apply to our offices as well?
 
From MDLinx November 14, 2018: Neckties: Yet another reason to forego the necktie: Studies have shown that neckties and other items of clothing quickly become contaminated with bugs such as MRSA and C. difficile.
 
Worries about clothing contamination have fueled a new policy in the UK National Health Service hospitals banning neckties and jackets. Healthcare workers engaged in direct patient care are, instead, required to wear re-processable garments.

In response to Dr. Smith’s article concerning proper attire in the office, it has always been my view and experience of 55 years in practice, especially running courses in “Practice Administration”, that proper business attire sends a positive message to your patients of respect for them, yourself, and your profession. Wearing a white lab coat requires a business shirt and tie, or surgical scrubs. Nothing short of that is acceptable. When you visit a lawyer, his dress code is a suit and tie. Why would anyone want to present with a lesser appearance as a podiatric physician? The key is to “Dress for Success!”
