NC Podiatrist Thankful to be Back on His Feet
 
Dr. Doug Milch doesn’t remember a thing about Sept. 21, 2014. But it’s a day he will never forget. That’s the fall day last year when Milch, an Asheville podiatrist and long-distance runner, was driving home on I-40 from a medical conference in Nashville with his wife, Debra Wright, also a podiatrist. He woke up in the intensive care unit at the University of Tennessee Medical Center in Knoxville, after hitting a tree.

Milch was in surgery in the ICU for one week. He had a badly damaged pelvis, which required three metal plates and 18 screws; then was on the trauma floor for a week. He returned to Asheville and spent a week in rehabilitation, and then was able to return home, but with strict, non-weight-bearing orders for 12 weeks. Milch said he has a new perspective on what other people with severe injuries and illnesses go through now that he has been there himself. He hopes that others who find themselves in these "hopeless" situations, will also just keep going.

For Christmas, think socks for kids. Knox Community Hospital Specialty Office of Podiatry is collecting socks at the office. All the socks will go to The Salvation Army, Interchurch and New Directions. The children pictured are the grandchildren of Dr. Bob Atwell and his office employees.

UK Podiatrist Runs Mobile Clinic
 
Typically, our feet will carry us for an amazing 150,000 miles during our lifetime, but we don't think about them until something goes wrong. That's where Rob Lowe comes in. Lowe, 46, originally from Stratford-On-Avon, spent 27 years in the Royal Navy, but has now set up in practice specializing in podiatry and exercise rehabilitation.

Although he left the British Navy in 2012, Rob still spends one day a week working with injured veterans under the banner of Help For Heroes in Devonport Dockyard's Endeavour Building. But his main work is looking after people's feet and helping sportsmen and women recover from injuries. Lowe's wife Caroline is also a podiatrist. She works for the NHS. His mobile clinic is a former NHS neo-natal ambulance and is fully-equipped, including a ramp for wheelchairs.

ACFAS Division 9 Holds Interactive Complications Seminar
 
On November 21, 2015, ACFAS Division 9 presented their Interactive Complications Seminar, which was held at New York-Presbyterian Queens. Five local residency programs presented six challenging surgical and medical complications, and the audience participated in insightful discussion and problem-solving. The event was moderated by the ACFAS Division 9 Board Members and the participating residency directors.

Royal Philips has announced that Lumify, its smart device ultrasound solution, is available for purchase by licensed healthcare providers or organizations in the U.S., and is offered as part of a new digital subscription model. The subscription includes a transducer, app, access to the online portal, warranty, and Philips’ support.

Five Steps to Improve the Patient Experience - Part 5

4. Make ease-of-access a top priority
 
This starts with a well-managed appointment scheduling system that has realistic templates. It means understanding demand and preparing for it by having enough available appointments, based on historical same-day demand. It means starting on time and ending on time. It means keeping the office and the phone lines open during the lunch break. This can be done by staggering staff schedules (start times, end times, and lunch breaks) between physicians and staff members.
 
Also build in accountability by tracking patient access. Know when the next available appointments are and require your appointment schedulers to communicate when demand shifts and appointment needs are being compromised. Your practice management system and EHR can provide critical performance data. Run system reports each month and analyze them. This builds in needed accountability that focuses on improving the patient experience and the service your practice provides.

Query: How Would This be Coded?
 
I took a patient to surgery and resected a coalition between the 3rd and 4th metatarsals, right foot. Additionally, I excised a fracture fragment (an intra-operative finding) from the dorsal base of the 4th metatarsal. What CPT and ICD-10 coding would be recommended?

Response: When coding removal of bone from the metatarsal, I would code it CPT 28122 (partial excision [craterization, saucerization, sequestrectomy, or diaphysectomy] bone [e.g., osteomyelitis or bossing]; tarsal or metatarsal bone, except talus or calcaneus). 
 
ICD-10 coding possibilities are: Q66.89 (other specified congenital deformities of feet) for the coalition. 
 
For the bone fragment as the intra-operative finding, if unrelated to the congenital code, use M21.6X1 (other acquired deformities of right foot).

Many of us felt that the Affordable Health Care act would be a panacea for our nation's healthcare problems. The Insurance companies felt like they would make out like bandits. Now, we are all having buyers remorse over the program. Doctors are not seeing as many patients. Patients who had good healthcare insurance found themselves being forced into policies with massive annual deductibles and very high co-payments, making them feel like having insurance is an expensive joke.
 
There is a light at the end of the tunnel. Just as the first automobiles were rickety death traps and the early airline industry was beset with inordinate amounts of crashes, Obamare is also in its infancy and will require a great deal of upgrading to work out all of the kinks. It will take a number of years with a great many tweaks. Only then will patients, physicians and the insurance industry have a well functioning healthcare system.

Dr. Malusky's statement, as written, is not the truth. No insurance company paid to be included, nor did they "bribe", to get in. In fact, insurance companies scrambled to create plans that met the PPACA guidelines. They wanted in, and the government needed them. It was beneficial to both. Do I trust the stated reasons UHC made, for possibly pulling out of the PPACA? What do you think?
